Evaluation and clinical laboratory of patients with suspected dengue served in Hospital of the State of Cearà reference

Resumo: Dengue is considered the most important arboviral disease and has a wide clinical spectrum, can present asymptomatically or as undifferentiated fever and may progress to severe forms such as dengue hemorrhagic fever and dengue shock syndrome. WHO suggested a new classification , because the diagnostic criteria were very strict and too based on laboratory results which hampered clinical management , and medical interventions are essential to conduct most appropriate to each case . The present study aimed to : identify laboratory abnormalities through specific and non specific in patients with suspected dengue fever treated at a referral hospital in the year 2012. The diagnosis was based on clinical, epidemiological and laboratory criteria . Among the laboratory tests used, the non specific tests: blood count, coagulation profile, liver function test, serum albumin and owned confirmation performed by specific tests, using molecular methods, virus isolation and serological methods to demonstrate the presence of antibodies IgM detecting the NS1 nonstructural glycoprotein, might be used as a marker during the acute phase of the disease as well as ELISA immunoassays, of the 95 patients, 65 ( 68.48 % ) had dengue fever according to the traditional classification, but to apply the new classification proposed by the WHO noted a change in profile with 53 ( 55 , 78 % ) patients with dengue with warning signs. Non specific tests in a constant change was observed in platelet and RBC indices as hematocrit, among the specific tests we observed a higher positivity of ELISA-IgM anti-dengue. Dengue fever is a difficult endemic infectious disease diagnosis. Therefore, it is necessary to invest in a deeper analysis of clinical and epidemiological data to arrive at a more accurate diagnosis in an early and proper treatment to each patient.

A OMS sugeriu uma nova classificaÃÃo clÃnica, pois os critÃrios de diagnÃsticos eram muito rÃgidos e demasiadamente baseados em resultados de laboratÃrio o que dificultava o manejo clÃnico, sendo que as intervenÃÃes mÃdicas sÃo imprescindÃveis para uma conduta mais adequada a cada casos. O presente estudo teve como objetivo, identificar as alteraÃÃes laboratoriais atravÃs de exames especÃficos e inespecÃficos em pacientes com suspeita clÃnica de dengue atendidos em um hospital de referÃncia no ano de 2012. O diagnÃstico foi baseado em critÃrios clÃnicos, epidemiolÃgicos e laboratoriais. Dentre os exames laboratoriais utilizaram-se os exames inespecÃficos: hemograma, coagulograma, prova de funÃÃo hepÃtica e dosagem de albumina sÃrica e possuiu sua confirmaÃÃo realizada por exames especÃficos, atravÃs de mÃtodos moleculares, isolamento viral e mÃtodos sorolÃgicos que demonstram a presenÃa de anticorpos da classe IgM, a detecÃÃo da glicoproteÃna nÃo estrutural NS1 que pÃde ser um marcador utilizada como um marcador durante a fase aguda da doenÃa assim como os imunoensaios ELISA. No presente estudo foram recrutados 95 pacientes e avaliados quanto as caracterÃsticas clÃnicas e individuais dos casos com suspeita de dengue, Dos 95 pacientes avaliados, 65 (68,48%) apresentaram dengue clÃssica segundo a classificaÃÃo tradicional, porÃm ao aplicar a nova classificaÃÃo sugerida pela OMS observamos uma modificaÃÃo no perfil com 53 (55,78%) pacientes com dengue com sinais de alarme. Nos testes inespecÃficos foi observado uma constante alteraÃÃo nos Ãndices hematimÃtricos como plaquetas e hematÃcritos, dentre os testes especÃficos foi observado uma maior positividade do ELISA â IgM anti-dengue. A dengue à uma doenÃa endÃmica infecciosa febril de difÃcil diagnÃstico diferencial.
