Immediate effects of the "Biofeedback and electrostimulation in treating anorectal paradoxical contraction of the muscles in women with blocked evacuation puborretal

Resumo: | This study was conducted to evaluate the effects of "biofeedback" and electrostimulation in reversing muscle contraction paradoxical puborectalis. The study involved 31 women from the Clinic of Coloproctology Walter CantÃdio University Hospital, Federal University of Cearà and the Center for Gastroenterology and Coloproctology of CearÃ, in the period August 2010 to July 2012, with 28 to 64 years of age, who met the Rome III criteria and presented, eletromanomÃtrico examination, presence of contraction to maneuver defecation. Were randomized into three groups: group I conventional treatment of constipation, group II conventional treatment of constipation associated with "biofeedback", Group III conventional treatment of constipation associated with electrical stimulation. At baseline and after six weeks of treatment, we assessed the subjective and objective parameters of anorectal function by means of the scoring system for constipation Wexner scale to Bristol stool consistency, visual analog scale and anorectal electromanometry. All patients demonstrated improvement in bowel satisfaction, stool frequency, effort and feeling of incomplete evacuation, stool-type modifications and improvement in quality of life (p <0.05). On examination there was increased pressure eletromanomÃtrico average voluntary contraction in group III (p = 0.043), decreased sensitivity threshold in group II (p = 0.025) and in group III (p = 0.012) and decreased rectal capacity maximum in group II (p = 0.005). Only 19.4% (n = 6) normalized dynamic defecation, and 80.6% (n = 25) expressed clinical improvement and not instrumental. The results confirmed that the "biofeedback" and electrostimulation are valuable treatment options and impact on quality of life of these patients. The "biofeedback" was more effective than electrical stimulation, which in turn is more effective than conventional treatment for constipation. Further randomized, controlled, long-term are needed to confirm the results. In the future, studies with the combination of therapeutic modalities may be a therapeutic option more sustainable and effective. |

Foram avaliadas 31 mulheres provenientes do AmbulatÃrio de Coloproctologia do Hospital UniversitÃrio Walter CantÃdio da Universidade Federal do Cearà e do Centro de Coloproctologia e Gastroenterologia do CearÃ, no perÃodo agosto de 2010 a julho de 2012, com 28 a 64 anos de idade, que preenchiam os critÃrios de Roma III e apresentavam, ao exame eletromanomÃtrico, presenÃa de contraÃÃo à manobra evacuatÃria. Foram randomizadas em trÃs grupos: grupo I tratamento convencional da constipaÃÃo, grupo II tratamento convencional da constipaÃÃo associado ao âbiofeedbackâ, grupo III tratamento convencional da constipaÃÃo associado à eletroestimulaÃÃo. No inÃcio e apÃs seis semanas do tratamento, foram avaliados os parÃmetros subjetivos e objetivos da funÃÃo anorretal, por meio do sistema de pontuaÃÃo para constipaÃÃo de Wexner, escala de Bristol para consistÃncia de fezes, escala analÃgica visual e eletromanometria anorretal. Todas as pacientes demonstraram melhora da satisfaÃÃo intestinal, frequÃncia evacuatÃria, esforÃo e sensaÃÃo de evacuaÃÃo incompleta, modificaÃÃes do tipo de fezes e melhora da qualidade de vida (p<0,05). Ao exame eletromanomÃtrico houve aumento da pressÃo mÃdia de contraÃÃo voluntÃria no grupo III (p=0,043), diminuiÃÃo do limiar de sensibilidade no grupo II (p=0,025) e no grupo III (p=0,012) e diminuiÃÃo da capacidade retal mÃxima no grupo II (p=0,005). Somente 19,4% (n=6) normalizaram a dinÃmica evacuatÃria, e 80,6% (n=25) expressaram melhora clÃnica e nÃo instrumental. Os resultados confirmaram que o âbiofeedbackâ e a eletroestimulaÃÃo sÃo opÃÃes terapÃuticas valiosas e de impacto na qualidade de vida destes pacientes. O âbiofeedbackâ foi mais eficaz do que a eletroestimulaÃÃo, que à por sua vez mais eficaz do que tratamento convencional para constipaÃÃo. Maiores estudos randomizados, controlados e em longo prazo sÃo necessÃrios para confirmar os resultados. No futuro, estudos com a associaÃÃo das modalidades terapÃuticas pode ser opÃÃo terapÃutica mais duradoura e eficaz.
