Resumo: | The establishment and maintenance of breastfeeding in premature and low birth weight newborns are complex, presenting greater challenges than in full-term and healthy newborns, as the breastfeeding process can be interrupted indefinitely. This project aims to address an institutional demand for improvements in breastfeeding assistance for premature infants and is aligned with the research line of organization and evaluation of healthcare systems. Overall Objective: To contribute to the improvement of the quality of breastfeeding management in premature and low birth weight newborns. Specific Objectives: Determine the current compliance with best practice recommendations; Identify barriers and facilitators to improve compliance with best practice recommendations; Implement strategies to address areas of non-compliance; Evaluate changes in compliance with evidence-based practice recommendations after the implementation of strategies. Methodology: Participatory research that used the implementation science framework and the JBI's audit and feedback tools named PACES and GRiP. The study was conducted in a university hospital in southeastern Brazil, with the participation of the multidisciplinary team and managers. The study was organized following the steps: situational diagnosis; protocol development; training; protocol implementation; monitoring and protocol review. Data were analyzed through descriptive statistics and content analysis. Results: Seven audit criteria were listed based on national and international guidelines, WHO and Ministry of Health manuals, and JBI Summaries. These criteria were assessed for compliance with evidence-based best practices. In the baseline audit, three of these criteria were found to be compliant, while the other four were non-compliant. Based on the situational diagnosis and baseline audit, barriers were identified, and eleven strategies were developed with the resources described for their execution. After the training, all the audited criteria showed compliance in the first follow-up audit, and five out of the seven audited criteria were compliant in the second follow-up audit. Products: Protocol for breastfeeding premature and low birth weight newborns. Flowcharts, process mappings, and infographics to guide staff work and parents' involvement in this process. Multidisciplinary and intersectoral training. Final Considerations: This project achieved the goal of improving evidence-based practice among the multidisciplinary team and enabled the development and implementation of the institution's first protocol for breastfeeding assistance to premature and low birth weight newborns, facilitated by the implementation of multifaceted strategies targeted at the identified barriers. The protocol and products are already implemented and generating positive impacts in the institution. This research has the potential to be applied at the regional and national levels as it systematically describes an innovative method with the necessary steps and resources for the implementation of the best scientific evidence. The research influenced changes in practice, but the maintenance of evidence-based care and adherence to the protocol require ongoing education for the neonatal team and other involved sectors to achieve better results. |

Esse projeto visa atender uma demanda institucional por melhorias na assistência à amamentação dos prematuros, e está alinhado a linha de pesquisa organização e avaliação dos sistemas de cuidados à saúde. Objetivo geral: Contribuir com a melhoria da qualidade do manejo da amamentação em recém-nascidos prematuros e de baixo peso. Objetivos específicos: Determinar a conformidade atual com as recomendações de melhores práticas; Identificar barreiras e facilitadores para melhorar a conformidade com as recomendações de melhores práticas; Implementar estratégias para abordar áreas de não conformidade; Avaliar as mudanças no cumprimento das recomendações de práticas baseadas em evidências após a implementação das estratégias. Metodologia: Pesquisa participativa que utilizou referencial da ciência da implementação e as ferramentas de auditoria e feedback do JBI nomeadas PACES e GRiP. Estudo realizado em hospital universitário no sudeste do Brasil, com participação da equipe multiprofissional e gestores. O estudo foi organizado seguindo-se as etapas: diagnóstico situacional; elaboração do protocolo; treinamentos; implementação do protocolo; monitoramento e revisão do protocolo. Os dados foram analisados por meio de estatística descritiva e análise de conteúdo. Resultados: Foram elencados sete critérios de auditoria com base em guidelines nacionais e internacionais, manuais da OMS e do Ministério da Saúde e por meio de Summarys da JBI. Tais critérios tiveram sua conformidade avaliada de acordo com as melhores práticas baseadas em evidências. Na auditoria de base três desses critérios se mostraram conformes, sendo os outros quatro não conformes. Por meio do diagnóstico situacional e auditoria de base foram verificadas barreiras e dessas foram elaboradas onze estratégias e descrito os recursos para executá-las. Após as capacitações pôde-se perceber conformidade de todos os critérios auditados na primeira auditoria de seguimento e conformidade de cinco dos sete critérios auditados na segunda auditoria de seguimento. Produtos: Protocolo para aleitamento materno de recém-nascidos prematuros e de baixo peso. Fluxogramas, mapeamentos de processo e infográficos para direcionar o trabalho de funcionários e a atuação dos pais nesse processo. Treinamentos multidisciplinares e intersetoriais. Considerações finais: Esse estudo alcançou o objetivo de melhorar a prática baseada em evidências da equipe multiprofissional e possibilitou a elaboração e implantação do primeiro protocolo de assistência à amamentação de recém-nascidos prematuros e de baixo peso da Instituição, possibilitada pela implementação de estratégias multifacetadas direcionada às barreiras identificadas. O protocolo e os produtos já estão implementados e gerando impactos positivos na instituição. Essa pesquisa tem potencial de ser aplicada a nível regional e nacional, pois descreve de forma sistematizada um método inovador com as etapas e recursos necessários para a implementação das melhores evidencias científicas.
