Efeitos das Mudanças na Composição Corporal e na Concentração Sanguínea de Adipocinas na Redução do Risco Cardiometabólico Após a Cirurgia Bariátrica

Resumo: The remission of obesity-related diseases following bariatric surgery appears to result from the reorganization of metabolic and hormonal pathways involving adipokines. This study aimed to investigate the relationship between changes in body adiposity and serum adipokine levels, as well as the association between variations in adiponectin or resistin levels and cardiometabolic risk blood biomarkers before and after Roux-en-Y gastric bypass. A longitudinal and prospective study was conducted with bariatric surgery patients. Anthropometric, body composition and blood biochemical parameters were measured before and at 2 and 6 months post-surgery. The data were analyzed using ANOVA, Pearson or Spearman correlation, and simple linear regression with a significance level of p < 0.05. Among 36 mostly female patients aged 30 to 39 years, significant reductions in body weight (−26.8%), fat mass (−50%), waist circumference (−18%) and waist-to-height ratio (−22%) were observed post-surgery. Serum adiponectin levels increased (+107%), while resistin (−12.2%), TNF-α (−35%), and PAI-1 (−11.1%) decreased. Glucose, insulin, CRP, cholesterol, LDL-c, triglycerides, and vitamin D also decreased. Waist circumference variation showed a positive correlation with PAI-1 and TNF-α and a negative correlation with adiponectin. The total fat mass showed a positive correlation with PAI-1. Adiponectin variation correlated negatively with glucose, resistin, and CRP but positively with HDL-c. Resistin showed a positive correlation with insulin and CRP. In conclusion, 6 months post-bariatric surgery, reducing abdominal adiposity had a more significant impact on serum adipokine levels than total fat mass. Adiponectin increase and resistin decrease acted as endocrine mediators driving the remission of cardiometabolic risk biomarkers in individuals with obesity following Roux-en-Y gastric bypass.

Este estudo teve como objetivo investigar a relação entre as mudanças na adiposidade corporal e nas concentrações séricas de adipocinas, bem como a associação entre as variações na concentração de adiponectina ou resistina e biomarcadores sanguíneos de risco cardiometabólico antes e após a cirurgia de bypass gástrico em Y de Roux. Estudo longitudinal e prospectivo realizado com pacientes submetidos à cirurgia bariátrica. Parâmetros antropométricos, composição corporal e parâmetros bioquímicos sanguíneos foram mensurados antes e aos 2 e 6 meses após a cirurgia. Os dados foram analisados utilizando ANOVA, correlação de Pearson ou Spearman e regressão linear simples com um nível de significância de p <0,05. Entre 36 pacientes, em sua maioria mulheres, com idades entre 30 e 39 anos, observou-se reduções significativas no peso corporal (−26,8%), massa gorda (−50%), circunferência da cintura (−18%) e na relação cintura-altura (−22%) pós-cirurgia. As concentrações séricas de adiponectina aumentaram (+107%), enquanto resistina (−12,2%), TNF-α (−35%) e PAI-1 (−11,1%) diminuíram. Glicose, insulina, PCR, colesterol, LDL-c, triglicerídeos e vitamina D também sofreram redução. A variação da circunferência da cintura mostrou correlação positiva com PAI-1 e TNF-α e correlação negativa com adiponectina. A massa gorda total mostrou correlação positiva com PAI-1. A variação da adiponectina correlacionou-se negativamente com glicose, resistina e PCR, mas positivamente com HDL-c. A resistina mostrou correlação positiva com insulina e PCR. Em conclusão, 6 meses após a cirurgia bariátrica, a redução da adiposidade abdominal teve impacto mais significativo nas concentrações séricas de adipocinas do que a massa gorda total.
