Resumo: Maternal mediation is an important factor in child development and the standard meditational adopted may be affected by different factors of cognitive or emotional nature. This study evaluated and compared the standard of mediation in mothers of children with Down syndrome, before and after an intervention program on mediation, describing the variables such as stress, anxiety, depression, coping strategies, and beliefs and expectations regarding child development. The sample consisted of nine mothers and their children, 2-5 years (M = 3.9 years). The collection was conducted in stages: in the first were applied sociodemographic questionnaire, Child Behavior Checklist [CBCL 1½ / 5 years] Interview to identify Beliefs and Expectations, Scales Beck anxiety and depression, Inventories of Stress Symptoms for adults Lipp (ISSL) and Coping Assessment Interview. In the second stage, the dyads were videotaped during three free and semi structured situations and evaluated by the Mediated Learning Scale (MLE Rate Scale). In the third stage, with subsample of six participants, it was applied a proposal for intervention in Maternal Mediation, based on MISC - Mediational Intervention for Sensitizing Caregivers with six criteria - Intentionality, Meaning, Transcendence, Behavior Regulation, Praise and Emotional involvement, organized in three workshops. In the fourth stage the dyads were videotaped after intervention at (20 days and 3 months). The data analysis showed that the majority of mothers (n=7) responded to diagnosis with negative feelings, but overcoming them over time. There were predominated (n=8) of positive expectations regarding the development; beliefs around the time of diagnosis, overall had religious character. In the evaluation by the scales Beck and ISSL, three mothers had mild level anxiety and one in moderate level; two with stress resistance phase (prevalence of psychological symptoms). Reported stressors were prejudice/social exclusion, routine appointments, in addition to behavioral problems, development and health of children. In the evaluation of coping strategies, the group predominantly adopted positive adaptive families, Self-confidence (n=8) Support Seeking (n=7), Problem Solving (n=7), Information Seeking (n=7), Accommodation (n=6) and Negotiation (n=1) compared to maladaptive, Avoidance (n=3), Isolation (n=2), Submission (n = 2) and Abandonment (n=1). It was found a significant difference between the means of mediational pattern obtained in the pre-intervention (2.0) and post-intervention (2.5) (Wilcoxon test, p=0,001). Of the six criteria of mediation, five improved in the segment sessions, with transcendence the one which obtained the highest Gain Rate (TG 78%). Mothers presented a good standard of mediation, in association with positive expectations of child development and greater repertoire of adaptive coping strategies of the diagnosis of disability and its stressors. Even so, the intervention helped to improve the quality of mediation between dyads, being thoroughly evaluated by participants. As a result, it is believed that brief and systematized interventions that enable mothers the feeling of competence in their ability as a mediator can help them identify opportunities for mediation of child development, also contributing to the maternal face of disability.

Cette étude a évalué et comparé le modèle de médiation chez les mères d'enfants atteints du Syndrome de Down, avant et après la proposition d'intervention sur la médiation, et il a décrit des variables telles que le stress, l'anxiété, la dépression, les stratégies d‟adaptation au handicap, et les croyances et les attentes en ce qui concerne le développement de l‟enfant. Neuf mères et leurs enfants de 2 à 5 ans (M = 3,9 ans) ont participé à l'étude . Les données ont été recueillies par étapes: dans la première étape, nous avons apliqué un questionnaire sociodémographique (Child Behavior Checklist [CBCL 1½ / 5 ans]), une interview pour identifier les croyances et les attentes, les Inventaires de Beck pour l'anxiété et pour la dépression, l'Inventaire de Symptômes de Stress pour les Adultes de Lipp (ISSL), et une interview pour l'évaluation de l‟adaptation. Dans la deuxième étape, les dyades ont été filmées dans trois situations libres et semi-structurées et elles ont été évaluées par l‟Échelle d'Apprentissage Médiée (MLE Rate Scale). Dans la troisième étape, avec un souséchantillon de six participants, nous avons fait une Proposition d'Intervention sur la Médiation Maternelle, basée sur le MISC - Mediational Intervention for Sensitizing Caregivers (Intervention Médiationnel pour Sensibiliser les Soignants), avec six critères – l‟ Intentionnalité, le Sens, la Transcendance, la Régulation du Comportement, l‟ Éloge et l'Implication Émotionnelle – et organisée en trois ateliers. Dans la quatrième étape, les dyades ont été filmées après l'intervention (20 jours et 3 mois). L'analyse des données a montré que la majorité des mères (n = 7) a réagi au diagnostic avec des sentiments négatifs, mais elles les ont surmontés au fil du temps (n = 6). Il y a eu la prédominance (n=8) des attentes positives en ce qui concerne le développement et les croyances autour du moment du diagnostic et elles avaient du caractère tout à fait religieux. Selon l'évaluation par des échelles de Beck et ISSL, trois mères avaient de l'anxiété au niveau légère et une mère l‟avait au niveau modéré; deux mères avaient du stress en phase de résistance (prédominance des symptômes psychologiques). Les facteurs de stress rapportés étaient le préjugé/l‟exclusion sociale, la routine des soins, et les problèmes liés au comportement, au développement et à la santé des enfants. L'évaluation des stratégies d'adaptation a trouvé la prédominance des familles adaptatives positives, la confiance en soi (n = 8), la recherche de soutien (n = 7), la résolution des problèmes (n = 7), la recherche d'information (n = 7), l‟ adaptation (n = 6) et la négociation (n = 1), par rapport à celles mésadaptés, comme la fuite (n = 3), l'isolement (n = 2), la submission (n = 2) et l‟impuissance (n = 1). Nous avons trouvé une différence statistiquement significative entre les moyennes du modèle de médiation obtenues dans la pré-intervention (2,0) et dans la post-intervention (2,5) (Test de Wilcoxon, p≤0,001). Cinq des six critères de médiation se sont améliorés dans les séances de suivi, et la transcendance a obtenu le Taux de Gain le plus élevé (TG 78%). Les mères avaient un bon modèle de médiation associé à des attentes positives sur le développement de l'enfant et une plus grande répertoire de stratégies d'adaptation au handicap et à ses facteurs de stress. Pourtant, l'intervention a contribué à l'amélioration de la qualité de la médiation et a été bien évaluée par les mères. Il est conclu que les interventions brèves et systématisées peuvent aider les mères à identifier les opportunités de médiation du développement de l'enfant, et qu‟elles peuvent aussi les aider à s‟adapter au handicap.A mediação materna é importante para o desenvolvimento infantil e o padrão mediacional adotado poderá ser afetado por fatores de natureza cognitiva ou emocional. Este estudo avaliou e comparou o padrão de mediação em mães de crianças com Síndrome de Down, antes e após proposta de intervenção sobre mediação, descrevendo variáveis como estresse, ansiedade, depressão, estratégias de enfrentamento da deficiência, e crenças e expectativas acerca do desenvolvimento da criança. Participaram nove mães e seus filhos, de 2 a 5 anos (M=3,9 anos). A coleta foi conduzida em etapas: na primeira foram aplicados questionário sociodemográfico, Child Behavior Checklist [CBCL 1½/5 anos], Entrevista para identificação de Crenças e Expectativas, Escalas Beck ansiedade e depressão, Inventário de Sintomas de Stress para adultos de Lipp (ISSL) e Entrevista de Avaliação do Enfrentamento. Na segunda etapa as díades foram filmadas em três situações livre e semiestruturadas, sendo avaliadas pela Escala de Aprendizagem Mediada (MLE Rate Scale). Na terceira etapa, com subamostra de seis participantes, realizou-se uma Proposta de Intervenção em Mediação Materna, baseada no MISC - Mediational Intervention for Sensitizing Caregivers, com seis critérios - Intencionalidade, Significação, Transcendência, Regulação do comportamento, Elogio e Envolvimento afetivo, organizado em três oficinas. Na quarta etapa as díades foram filmadas após intervenção (20 dias e 3 meses). A análise das entrevistas indicou que a maioria das mães (n=7) reagiu ao diagnóstico com 13 sentimentos negativos, superando-os com o passar do tempo (n=6). Houve predomínio (n=8) de expectativas positivas em relação ao desenvolvimento e as crenças em torno do momento do diagnóstico, em geral, tinham caráter religioso. Na avaliação pelas escalas Beck e ISSL, três mães apresentaram ansiedade em nível leve e uma em nível moderado; duas com estresse em fase de resistência (predomínio de sintomas psicológicos). Os estressores relatados foram preconceito/exclusão social, rotina de atendimentos, além dos problemas de comportamento, desenvolvimento e saúde da criança. Na avaliação das estratégias de enfrentamento, encontrou-se predominância das famílias adaptativas positivas, Autoconfiança (n=8), Busca de Suporte (n=7), Resolução de Problema (n=7), Busca de Informação (n=7), Acomodação (n=6) e Negociação (n=1), em comparação às mal adaptativas, como Fuga (n=3), Isolamento (n=2), Submissão (n=2) e Desemparo (n=1). Encontrou-se diferença estatisticamente significativa entre as médias do padrão mediacional obtidas na pré-intervenção (2,0) e pós-intervenção (2,5) (Teste Wilcoxon, p≤0,001). Dos seis critérios de mediação, cinco melhoraram nas sessões de segmento, sendo transcendência o que obteve maior Taxa de Ganho (TG 78%). As mães apresentaram bom padrão de mediação, associada a expectativas positivas do desenvolvimento infantil e maior repertório de estratégias de enfrentamento adaptativas à deficiência e seus estressores. Ainda assim, a intervenção contribuiu para melhorar a qualidade da mediação, sendo bem avaliada pelas participantes. Conclui-se que intervenções breves e sistematizadas podem ajudar as mães a identificar oportunidades de mediação do desenvolvimento da criança, contribuindo também para o enfrentamento da deficiência.TextREIS, Luciana Bicalho.
