Resumo: | Psoriasis is a common chronic inflammatory skin disease, genetic and immunological genetic basis of great importance in clinical practice. Psoriasis can be treated in several ways, with topicals medicines, and / or phototherapy treatment. The use of immunosuppressive drugs in systemic treatment of psoriasis may increase the prevalence of Candida in the oral cavity of psoriatic patients. Studies show that the prevalence of Candida in the tongue and saliva has been higher in psoriatics patients. Candidiasis is a fungal infection most common in human oral, and Candida albicans the main species found. Oral candidiasis may present different clinical forms and can manifest itself in one or more ways, and involve only one or more regions of a mucosal surface. Some factors predispose to the development of candidiasis, such as debilitated patients due to some severe systemic disease, patients using corticosteroids and others. The diagnosis is made, in most of the cases, according to the clinical appearance of lesions. However, some cases are not confirmed in a laboratory examination. Cytopathology oral is defined as a study and interpretation of cellular characteristics that are spilled naturally or artificially in the oral mucosa. It is a non invasive diagnosis method with a high specificity and sensitivity, providing its utilization such as routine method for the diagnosis of lesions such as candidiasis. Due to the scarce literature on the prevalence of oral candidiasis in patients with psoriasis, this study tested the hypothesis that psoriasis is a predisposing factor for oral candidiasis, independent of immunosuppressive therapy. Material and Methods: This is a study of interagency and cross - analytical and case control. The study sample consisted of 392 patients, 196 of the psoriasis group (GP) and 196 control group (CG). The GP was made of patients with clinical and / or histopathological psoriasis, from the Dermatology ambulatory HUAP-UFF, HCFF-UFRJ, IPRDA-SCM. The GC was made of volunteers, clinically healthy and normal oral examination; selected from campaigns for registration of bone marrow donors conducted in the State of Rio de Janeiro by Hemorio and diagnosis oral laboratory. These patients were submitted to clinical examination, sialometry, scraping the back and edges of the tongue, in patients using dentures, held scraped the contact area of the prosthesis later. Then these smears were fixed and stained for analysis. Results: Most patients with psoriatic candidiasis were white women aged 51-60 years. Most patients had some psoriatic lesion and the fissured tongue, candidiasis, geographic tongue and fissured tongue associated with the most prevalent manifestations. Oral candidiasis prevailed in late-onset psoriasis. No statistical difference between treatment and severity of psoriasis, compared with the presence of oral candidiasis. The cytopathology has high sensitivity and specificity when compared with the physical examination. Conclusion: Psoriasis patients have a higher prevalence of oral candidiasis when compared to healthy subjects. Therefore, the analyzes conducted in this study confirm the hypothesis and show that psoriasis is a risk factor for oral candidiasis, regardless of immunosuppressive therapy |

A psoríase pode ser tratada de diversas maneiras, com medicamentos tópicos, sistêmicos e/ou fototerapia. Alguns autores afirmam que o uso de imunossupressores no tratamento sistêmico da psoríase, pode aumentar a prevalência de Candida na cavidade oral dos pacientes psoriásicos. Estudos mostram que a prevalência de Candida na língua e na saliva, tem sido maior nos pacientes com psoríase. A candidíase é a infecção fúngica oral mais comum em humanos, sendo a Candida albicans a principal espécie encontrada. A candidíase oral pode apresentar formas clínicas variadas, podendo se manifestar em uma ou mais formas, e acometer apenas uma ou mais regiões de uma superfície mucosa. Alguns fatores predispõem o desenvolvimento da candidíase, tais como: pacientes debilitados em virtude de alguma doença sistêmica grave, paciente em uso de corticosteróides, entre outros. O diagnóstico é realizado, na maioria das vezes, de acordo com o aspecto clínico das lesões, não sendo confirmado laboratorialmente. A citopatologia oral é definida como o estudo e a interpretação das características das células que se descamam, natural ou artificialmente da mucosa oral. É um método diagnóstico não invasivo, que apresenta alta especificidade e sensibilidade, o que viabiliza a sua utilização como metodologia de rotina para o diagnóstico de lesões como a candidíase. Devido à escassa literatura sobre a prevalência da candidíase oral nos pacientes com psoríase, este estudo testou a hipótese de que a psoríase é um fator predisponente à candidíase oral, independente da terapia imunossupressora. Material e Métodos: trata-se de um estudo interinstitucional e transversal - analítico e caso controle. A amostra deste estudo foi constituída por 392 pacientes, 196 do grupo psoríase (GP) e 196 do grupo controle (GC). O GP consistiu de pacientes com diagnóstico clínico e/ou histopatológico de psoríase, provenientes dos Ambulatórios de Dermatologia do HUAP-UFF, HCFF-UFRJ, IPRDA-SCM. O GC foi constituído de indivíduos voluntários, clinicamente saudáveis e com exame físico oral normal; selecionados das campanhas para cadastramento de doadores de medula óssea realizados no Estado do Rio de Janeiro pelo Hemorio e do ambulatório de diagnóstico oral. Estes pacientes foram submetidos ao exame clínico, sialometria, raspagem do dorso e bordas da língua; nos pacientes que utilizavam prótese total superior, realizou-se raspado da área de contato da prótese. Em seguida, estes esfregaços foram fixados e corados, para posterior análise. Resultados: a maioria dos pacientes psoriásicos com candidíase eram mulheres brancas, na faixa etária dos 51-60 anos. A maioria dos pacientes psoriásicos apresentou alguma lesão, sendo a língua fissurada, candidíase, e a língua geográfica associada à língua fissurada as manifestações mais prevalentes. A candidíase oral prevaleceu na psoríase de início tardio. Não foi observada diferença estatística entre o tratamento e a severidade da psoríase, com relação à presença de candidíase oral. A citopatologia apresenta alta sensibilidade e especificidade quando comparada com o exame físico. Conclusão: os pacientes psoriásicos apresentam maior prevalência de candidíase oral quando comparados aos indivíduos saudáveis, independente da terapia imunossupressora.
