Diagnóstico citopatológico da leucoplasia pilosa oral em pacientes portadores do HIV-1; Análise comparativa entre a Citopatologia Convencional e a Citologia em Meio Líquido

Resumo: | Oral hairy leukoplakia was first described by Greenspan in 1984, and composes the group of oral lesions strongly associated with acquired immune deficiency syndrome, having an important diagnostic and prognostic value. It´s a white, assymptomatic, and non-removable lesion, with plane, corrugated or hairy surface, most frequently located on the lateral border of the tongue. This lesion is caused by Epstein-Barr virus, and its diagnosis can be made by detecting the typical cytopathic effects caused by the virus in epithelial cells. These effects are detectable by cytopathology, which can also diagnose its subclinical phase. Liquid-based cytology has some advantages when compared to conventional smears, but its diagnostic efficacy for oral hairy leukoplakia detection is unknown. The aim of this research was to compare the diagnostic efficaciousness of both techniques. The sample consisted of 45 patients infected with human immunodeficiency virus, with or without clinical evidence of lesion. They were submitted to oral exam and had material scraped from both sides of the tongue by using an endocervical brush. Liquid-based cytology samples were collected first. Conventional smears were prepared and the brush containing residual material was preserved for future analysis by polymerase chain reaction. For comparative analysis of both techniques, all slides were evaluated for diagnosis, cellularity and cell overlapping. Liquid-based cytology showed good diagnostic efficaciousness when compared to conventional cytology, even not detecting one case of oral hairy leukoplakia. Conventional slides showed higher cellularity and cell overlapping, advantages that contribute to diagnostic criteria. Conventional smears also showed lower processing time and cost. Liquid-based cytology has other advantages, as reduction of time of analysis, bacteria and the possibility of preparation of more than one slide per sample collected, providing material preserved for additional tests, and being more proper for research usage. The presence of Epstein-Barr virus was confirmed in all cases of oral hairy leukoplakia diagnosed by conventional cytology. Liquid-based cytology was not superior to conventional exfoliative cytology in detecting oral hairy leukoplakia lesions. The advantages of conventional cytology prove that this technique still must be commended for routine practice |

Trata-se de uma lesão branca, assintomática, não destacável, de superfície plana, corrugada ou pilosa, presente principalmente em borda lateral de língua, uni ou bilateralmente. Seu agente etiológico é o vírus Epstein-Barr, e seu diagnóstico pode ser feito por meio da identificação das alterações celulares representativas dos efeitos citopáticos da infecção viral no epitélio. Tais efeitos são vistos na citopatologia convencional, que constitui um excelente método diagnóstico desta lesão, podendo, inclusive, detectar sua fase subclínica. A citologia em meio líquido representa uma modificação sobre a técnica da citopatologia convencional, mas seu desempenho no diagnóstico da leucoplasia pilosa oral não é conhecido. O objetivo deste trabalho foi realizar uma análise comparativa entre as duas técnicas no diagnóstico citopatológico da leucoplasia pilosa oral. Participaram do estudo 45 pacientes infectados pelo vírus da imunodeficiência humana, com ou sem lesão clínica sugestiva desta lesão. Após anamnese e exame clínico, foram realizadas duas raspagens em cada borda da língua, utilizando uma escova endocervical ginecológica para cada raspagem. O material coletado na primeira raspagem era processado de acordo com a técnica da citologia em meio líquido. O material coletado na segunda raspagem foi estendido em esfregaços citopatológicos convencionais, reservando-se o material da ponta ativa da escova para a posterior realização da reação da polimerase em cadeia para identificação viral. As lâminas confeccionadas por meio de ambas as técnicas foram comparadas com relação ao diagnóstico obtido, celularidade e sobreposição celular. A citologia em meio líquido apresentou diagnóstico discordante em apenas um caso, onde não detectou a leucoplasia pilosa oral, apresentando excelência concordância diagnóstica com a citopatologia convencional. As lâminas confeccionadas por meio da citopatologia convencional apresentaram maior celularidade e maior sobreposição celular, características que representam vantagens, pois facilitam o diagnóstico citopatológico da leucoplasia pilosa oral. A citopatologia convencional também apresentou outras vantagens como menor tempo de processamento e baixo custo. As vantagens da citologia em meio líquido foram redução de agregados bacterianos, redução do tempo de análise das lâminas e fornecimento de material de reserva, característica esta que a torna muito útil na realização de pesquisas. A presença do vírus Epstein-Barr foi confirmada em todas as amostras com diagnóstico citopatológico de leucoplasia pilosa oral. Os resultados alcançados não demonstraram grandes vantagens ou um desempenho superior da citologia em meio líquido com relação à citopatologia convencional no diagnóstico da leucoplasia pilosa oral.
