Employing Risk Assessment techniques in industries has become an increasingly disseminated practice nowadays. Its application is being stimulated by several supervising and regulative associations on behalf of insurance companies as well as companies themselves, investors and other stakeholders. In general, relevant Risk Assessment regulations and guidelines mention the necessity of evaluating risks involved an activity, however there is no clear definition of which methodology to be applied in which cases and there are no criteria established for the acceptance of such type of work. Facing this gap, it is common to find results of Risk Assessment studies that appear to be very questioned by personnel involved in their implementation. In some cases there may produced some extremely substantial works in defining measures for reduction of risks, however these efforts turn into even more modifications and building sites than has been planned before, and thus causing a reduction in investment gains. In other cases, simplified results can bring doubts about the effective mitigation of the risks involved in the facility. This dissertation intends to propose orientations to be used in the initial research of hazards and risks assessment during the operational life cycle phase of an offshore petroleum production unit. The results have been obtained during an FPSO case study, by comparison between two evaluation studies during project and operational evaluation phases.
