National Health System: modalities of assistance and their interrelations, a nursing vision

Resumo: The present study has had as survey question the modalities of assistance of health institutions from the National Health System (SUS) and their interrelations. So, it has been objectified to identify the different modalities of assistance for the client from SUS offered by institutions/organizations and health services in the municipal district of Rio Grande, understand the relations which they have, and the way that has been processed their interrelations in the health services and health actions. It has been built a theorical referential to support the development of the survey, by the perspective of health as a system, and having as a reference the theory of systems with approach by Bronfenbrenner (l996), among others. To obtain information it has been used as methodological way the qualitative survey, descriptive, and explanatory. The collect of data was obtained from the method of semi-structured interview being developed from August to September of 2006, with fourteen subjects: coordinators of several services to provide entrance and routing for the clients from SUS in the municipal district of Rio Grande. The last question of the interview has been left opened, creating an opportunity for the interviewees to give pertinent suggestions. The process of analysis has taken into account the categories previously established: modalities of assistance, multi-professional team and interrelations between modalities of assistance. It has been evidenced, during the analysis and interpretation of data, the different modalities of assistance offered to the users of SUS, showing that each one presents its own peculiarities and the unknowledgement of others, and maybe this is the reason for not offering them, because they could help in the offer of more actions and services to this group of clients, bringing solutions for the repressed demand and reducing the cost of hospitalizations. It has been noticed the constant presence of the multi-professional team as a fundamental element in the execution of actions and services, standing out the strong participation of the nurses. Although it has been initially presupposed that the institutions/ organizations/ services offer the modalities in a disarticulated way, it has been noticed during the development of this work, that it does not happen this way. In the three health institutions there is a great effort by workers to get the routing of the clients for exams and for a possible hospitalization or for another kind of assistance to be offered by the institutions which integrate SUS. It has been evidenced, however, that there are not formal and computerized articulations, of reference and counter-reference that could facilitate the interrelations between the services of SUS.

El objetivo fue identificar las diferentes modalidades de atención al cliente del SUS ofrecidas por las instituciones/organizaciones/servicios de salud del municipio de Río Grande, así como comprender las relaciones que mantienen entre si y de que forma se procesan sus interrelaciones en lo servicios y acciones de salud. Se construyó una referencia teórica capaz de dar sustento al desarrollo de la investigación en la perspectiva de salud como un sistema, teniendo como punto de referencia la teoría de sistemas con un abordaje  en Bronfenbrenner (1996) entre otros. Para obtener la información se utilizó como método la investigación cualitativa, descriptiva y exploratoria. La recolección de datos se procesó a través del método de entrevista semi-estructurada, aplicado de agosto a septiembre de 2006, a catorce sujetos: coordinadores de los diversos servicios de entrada y orientación de clientes del SUS del municipio de Río Grande. La última pregunta de la entrevista era abierta, dejando espacio para  que los entrevistados propusieran sugerencias que encontraran pertinentes. El proceso de análisis llevó en consideración las categorías establecidas previamente: modalidades de atención, equipo multiprofesional e interrelaciones de las modalidades de atención. Se evidenció en el análisis e interpretación de los datos las diferente modalidades de atención ofrecidas a los usuarios del SUS., mostrando que cada una presenta peculiaridades propias y que el desconocimiento de otras, tal vez sea el motivo de no ofrecimiento, pues las mismas podrían auxiliar en la oferta de más acciones y servicios para estos clientes, trayendo soluciones para la demanda reprimida y disminuyendo el costo con la internación hospitalaria. Se percibió al ofrecer las modalidades de atención, la presencia constante del equipo multiprofesional como elemento fundamental en la ejecución de las acciones y servicios, destacando una fuerte participación del enfermero universitario. Sin embargo suponiendo inicialmente que las instituciones/organizaciones/servicios ofrecen las modalidades de forma desarticulada, se constató a lo largo de este trabajo que no siempre ocurre de esta manera. Existe de parte de los trabajadores de las tres instituciones de salud un gran empeño para conseguir el encaminamiento del cliente para exámenes así como para posible internación u otra atención a ser ofrecida por las instituciones integrantes del SUS. Se evidenció que no existen articulaciones formales e informatizadas de referencia y contrarreferencia que podrían facilitar las interrelaciones entre los servicios del SUS.O presente estudo teve como questão pesquisa as modalidades de atendimento das instituições de saúde do SUS e as suas inter-relações. Desta maneira, objetivou-se, identificar as diferentes modalidades de atendimento ao cliente do SUS oferecidas pelas instituições/organizações/serviços de saúde do município do Rio Grande, assim como compreender as relações que mantêm entre si e de que forma se processam as suas inter-relações nos serviços e ações de saúde. Construiu-se um referencial teórico capaz de dar sustentação para o desenvolvimento da pesquisa, na perspectiva da saúde como um sistema, tendo como ponto de referência a teoria de sistemas com abordagem de Bronfenbrenner (1996), dentre outros. Para obter as informações utilizou-se como caminho metodológico a pesquisa qualitativa, descritiva e exploratória. A coleta de dados processou-se através do método da entrevista semi-estruturada, aplicado de agosto a setembro de 2006, a quatorze sujeitos: coordenadores dos diversos serviços de entrada e encaminhamento de clientes do SUS do município do Rio Grande. A última questão da entrevista era aberta, deixando espaço para os entrevistados propor sugestões que julgassem pertinentes. O processo de análise levou em consideração as categorias estabelecidas previamente: modalidades de atendimento, equipe multiprofissional e inter-relações das modalidades de atendimento. Evidenciaram-se na análise e interpretação dos dados as diferentes modalidades de atendimento oferecidas aos usuários do SUS, mostrando que cada uma apresenta peculiaridades próprias e que o desconhecimento de outras, talvez, seja o motivo do não oferecimento, pois as mesmas poderiam auxiliar na oferta de mais ações e serviços para esta clientela, trazendo soluções para a demanda reprimida e diminuindo o custo com a internação hospitalar. Percebeu-se no oferecimento das modalidades de atendimento, a presença constante da equipe multiprofissional como elemento fundamental na execução das ações e serviços, destacando-se a forte participação do profissional enfermeiro. Embora pressupondo, inicialmente, que as instituições/organizações/serviços oferecem as modalidades de forma desarticulada, constatou-se ao longo deste trabalho, que nem sempre acontece dessa maneira. Existe por parte dos trabalhadores das três instituições de saúde um grande empenho para conseguir tanto o encaminhamento do cliente para exames como para a possível internação ou outro atendimento a ser oferecido pelas instituições integrantes do SUS.
