Humanistic nursing: contribute for development of the nurse in neonatal unit

Resumo: | As nurses, working in neonatology, we believe in the necessity of reflection in order to transpose obstacles, leading to a job with an emancipatory perspective. Seeking to investigate the practice of the nurse’s assistances to the high-risk newborn (NB) at the UIN based on the humanistic theory, analyze the practice of the nurse’s assistance to the high-risk newborn and to elaborate a process of humanistic care to the NB, with the nurses´ collective participation. A qualitative research supported by the humanistic theory, by Paterson and Zderad developed at UIN of a maternity school in the city of Fortaleza-CE. Totaling a period, referring to data collection from March to August 2005. The subjects were ten nurses in activity at UIN. During a period of sixty (60) days, weekly meetings, which were filmed, took place for the data collection. Later, after the meetings, we used participant observation at the UIN to register the interdisciplinary communication among nurse/baby/family/equip. During the analysis, synthesis and description we tried to comprehend it under the humanistic theory by Paterson and Zderad. The data was organized and presented using frames. After the reflection of the theoretical expositions, themes extracted from speeches and the filming of the participants, and developed by the group were: demotivation, caring, interpersonal relationship environment. Regarding the demotivation theme, mental and physical fatigue sub themes, the re-work at UIN, lack of a perspective of changes and self-knowledge emerged. Regarding the caring theme, the sub themes were self-care and caring for the baby. in the environment theme, the environment of the UIN and the feelings of the nurse in the UIN environment were the emerged sub themes. In the interpersonal relationship theme, the sub theme was found to be the relationship with the interdisciplinary equip in the UIN. The process of humanistic caring for the NB was elaborated collectively by the nurse’s, after the comprehension of the meaning of better-being at the UIN, and included all stages of the humanistic theory. We tried to comprehend the relations among speech and existence, the environment, the ambiance around the person of each nurse. The defended thesis reaches the comprehension of the nurse’s experience significance, their better-being while caring for the NB at UIN, by the development of self-conscience, by understanding the others, by a more relating look to diversities, by the establishment of a wider vision of the reality lived through in their daily lives, by the transition of their egocentrism to sharing of their emotions, by their knowledge and their vocation; caring.Key words: Nursing process; Nurse-patient relations; Newborn; Neonatal nursing. |

Tuvimos el objetivo de investigar la práctica de la enfermera en la asistencia al RN de gran riesgo, después de la mejoría de su atención, basada en los presupuestos de la Teoría Humanística de Enfermería, y elaborar un proceso de atención humanística al RN con participación colectiva de las enfermeras. Se trata de una investigación cualitativa, basada en la Teoría Humanística de Enfermería de Paterson y Zderad, desarrollada en la UIN de una maternidad escuela, en la ciudad de Fortaleza-CE, en el período referente a la colecta de datos, de marzo a agosto de 2005. Los sujetos fueron diez enfermeras actuantes en la UIN. Para la colecta de datos fueron realizadas reuniones semanales en un período de 60 días, las cuales fueron grabadas. En el segundo momento, después de las reuniones, utilizamos la observación participante en la UIN para registrar la comunicación enfermera/niño/familia/equipo interdisciplinario. Durante el análisis, síntesis y descripción, intentamos comprenderla a la luz de la Teoría Humanística de Enfermería, de Paterson y Zderad. Los datos fueron organizados y presentados a través de cuadros. Las temáticas extraídas de las hablas y también de las grabaciones de las participantes, y después de los reflejos de las exposiciones teóricas desarrolladas por el grupo, fueron: desmotivación, atención, ambiente, relación interpersonal. De la temática desmotivación, emergieron los temas cansazo físico y mental, el nuevo trabajo en la UIN, falta de perspectiva para cambios, autoconocimiento; de la temática atención, los temas fueron auto cuidado y atención al niño; de la temática ambiente, emergieron los temas el ambiente de la UIN y sentimientos de la enfermera en el ambiente de la UIN; en la temática relación interpersonal, el otro tema fue la relación con el equipo interdisciplinario. El proceso de atención humanística al recién nacido fue elaborada colectivamente por las enfermeras después de la comprensión del significado del su estar mejor en UIN, y todavía incluyó todas las etapas de la Teoría Humanística. Intentamos comprender las relaciones entre el discurso y la existencia, el medio, el ambiente que envuelve las personas de cada enfermera. La tesis alcanza la comprensión de la enfermera sobre el significado de su experiencia, de su estar mejor al cuidar del RN en UIN, por el desarrollo de la autoconciencia, de la comprensión del otro, por lo mirar más relacionador de las diversidades, por el establecimiento de una visión más amplia de las realidades vividas en su rutina, por la transición de su egocentrismo para lo compartir de sus emociones, de su saber y de su vocación – lo cuidar.Palabras clave: Procesos de enfermería; Relación enfermero-paciente; Recién nacido; Enfermería neonatal.Como enfermeiras, atuando em Neonatologia, acreditamos ser necessária uma reflexão para transpor obstáculos, procurando opções que levem a um trabalho com perspectiva emancipatória. Objetivamos investigar a prática da enfermeira na assistência ao RN de alto risco na UIN com base na Teoria Humanística; analisar a prática da enfermeira na assistência ao RN de alto risco, após aprimoramento do seu cuidado, embasado nos pressupostos da Teoria Humanística de Enfermagem, e elaborar um processo de cuidado humanístico ao RN com participação coletiva das enfermeiras. Pesquisa qualitativa, apoiada na Teoria Humanística de Paterson e Zderad, desenvolvida na UIN de uma maternidade-escola, na cidade de Fortaleza-CE, perfazendo um período de março a agosto de 2005. Os sujeitos foram dez enfermeiras atuantes na UIN. Para a coleta de dados foram realizadas reuniões semanais, em um período de 60 dias, as quais foram filmadas. No segundo momento, após as reuniões, usamos a observação participante na UIN para registrar a comunicação enfermeira/bebê/família/equipe interdisciplinar. No decorrer da análise, síntese e descrição buscamos compreendê-la à luz da Teoria Humanística, de Paterson e Zderad. Os dados foram organizados e apresentados por meio de quadros. As temáticas extraídas das falas e das filmagens das participantes, após reflexões das exposições teóricas desenvolvidas pelo grupo, foram: desmotivação, cuidado, ambiente, relacionamento interpessoal. Da temática desmotivação, emergiram as subtemáticas cansaço físico e mental, o re-trabalho na UIN, falta de perspectiva para mudanças, autoconhecimento; da temática cuidado, as subtemáticas foram autocuidado e cuidado ao bebê; da temática ambiente, emergiram as subtemáticas o ambiente da UIN e os sentimentos da enfermeira ao atuar na UIN; da temática relacionamento interpessoal, a subtemática foi relacionamento com a equipe interdisciplinar na UIN. O processo de cuidado humanístico ao RN foi elaborado coletivamente pelas enfermeiras após a compreensão do significado do seu estar-melhor em UIN, e incluiu todas as etapas da Teoria Humanística. Tentamos compreender as relações entre o discurso e a existência, o meio, a ambiência que rodeia a pessoa de cada enfermeira.
