Acupuntura combinada com auriculoterapia no tratamento e qualidade de vida de pacientes com Síndrome da Ardência Bucal (SAB): ensaio clínico de braço único

Resumo: Abstract: The Burning Mouth Syndrome (BMS) is part of the framework of chronic pain which is difficult to diagnose, refractory to treatment, more prevalent in women in pre and postmenopausal phase. Acupuncture and auriculotherapy have been suggested as options in the treatment of pain, since they promote analgesia favoring the reduction of symptoms with lower doses of drugs and allowing greater patient compliance to treatment, and positive impact on quality of life. However, the phobia to needles or fear of adverse reactions can trigger anxiety, nervousness, abandon treatment, generating stress, which can lead to the release of glucocorticoids, such as cortisol. Clinical trials investigating the effectiveness of acupuncture in the treatment of BMS are scarce in the literature. Objective: To investigate combined acupuncture and auriculotherapy in the treatment and quality of life of patients with BMS. Methods: Sixty patients with burning mouth were subjected to a thorough process of differential diagnosis of BMS. Of these, 12 met the inclusion criteria and agreed to participate. Eight patients have completed treatment with acupuncture and auriculotherapy with previously established protocol. The outcome variables were analyzed before and after treatment: pain / burning (VAS - visual analog scale), salivary flow (unstimulated sialometry), quality of life (OHIP14) and stress levels (salivary cortisol). Results: Significant reduction in the intensity of pain / burning in the first treatment sessions was observed; the low values of EVA (0-2) and subjective indicator of quality of life (μ = 5.37 ± 3.50) revealed improvement in quality of patients´ life. There was no relationship between salivary flow and intensity of pain / burning. Salivary cortisol levels were significantly lower before (P = 0.03) and during (P = 0.01) acupuncture / auriculotherapy sessions (1st, 11th), with P <0.005 for all comparisons to baseline upon awaking, revealing that the procedures under the study conditions, did not promote stress on the patients. Conclusions: Combined acupuncture / auriculotherapy was effective in reducing the intensity of burning and improving quality of life, not generating stress during treatment sessions. There was no relationship between salivary flow and intensity of burning mouth.

A acupuntura e auriculoterapia têm sido sugeridas como opção no tratamento da dor, visto que promovem a analgesia, favorecem a redução dos sintomas com menores doses de medicamentos e possibilitam maior adesão do paciente à terapêutica, atuando positivamente na qualidade de vida. No entanto, a fobia às agulhas ou o medo de reações adversas podem desencadear ansiedade, nervosismo, abandono do tratamento, causando estresse, o que pode gerar a liberação dos glucocorticóides, como o cortisol. Os ensaios clínicos que investigam a efetividade da acupuntura no tratamento da SAB são escassos na literatura. Objetivo: Investigar a efetividade da acupuntura associada à aurículoterapia no tratamento e qualidade de vida de pacientes acometidos pela SAB. Metodologia: Sessenta pacientes com ardência bucal, atendidos em um centro de referência integrante da rede de saúde pública no estado de Goiás, foram submetidos a um processo minucioso de diagnóstico diferencial da SAB. Desses, doze preencheram os critérios de inclusão e concordaram em participar do estudo. Oito pacientes finalizaram o processo com acupuntura e auriculoterapia, com protocolo previamente estabelecido. As variáveis de desfecho foram analisadas antes e após o tratamento: intensidade da dor/dormência (EVA – Escala Visual Analógica), fluxo salivar (sialometria não estimulada), qualidade de vida (OHIP14) e os níveis de estresse (cortisol salivar). Resultados: Uma redução significativa da intensidade da dor/ardência foi observada já nas primeiras sessões; os valores reduzidos da EVA (0 a 2) e do indicador subjetivo de qualidade de vida (µ= 5,37 ± 3,50) revelaram melhora na vida das pacientes. Não houve relação entre as variáveis fluxo salivar e intensidade da dor/ardência. Os níveis de cortisol salivar foram significativamente menores antes (P = 0,03) e durante (P = 0,01) as sessões de acupuntura e auriculoterapia (1a e 11a), com P<0,005 para todas as comparações com os níveis basais ao acordar, revelando que os procedimentos, dentro das condições utilizadas, não suscitaram estresse no paciente. Conclusões: A associação acupuntura/aurículoterapia foi eficaz na redução da intensidade da ardência e na melhora da qualidade de vida, não gerando estresse durante sua realização.
