Caracterização clinico-patológico e densidade de mastócitos em pacientes com câncer gástrico avançado

Resumo: Gastric cancer is the second most common cause of cancer-related mortality worldwide, mainly due to chronic infection by the bacteria Helicobacter pylori. Epidemiological, clinical, pathological and molecular investigations seeking a better understanding of these tumors may help establish a better prognosis for the disease and the development of better therapeutic possibilities. In particular the tumor microenvironment has been studied to better understand the ways of interacting with the immune system and neoplasms. Within this context, the aim of this study was a retrospective survey of the epidemiology and pathology of patients with gastric cancer in a hospital in Goiânia and evaluate pathological, clinical aspects as well as the density of mast cells in some selected cases. To do so, we reviewed medical records of 220 patients with advanced gastric cancer in the period 2005-2009 of Araújo Jorge Hospital according to established inclusion criteria. Data were collected as gender, age, histological type, grade of differentiation, site of cancer incidence according to the topography of the stomach and palliative surgeries performed to determine the clinical-pathological profile of patients. Blocks were selected with the biopsy some cases (29 cases) for cutting and staining of slides for immunohistochemistry and hematoxylin-eosin to evaluate the density of mast cells present in the samples. The results showed that most patients (62.96%) were males, with the majority of cases of gastric cancer has been found in elderly over 60 years (62%). Tumors were located preferably in the antrum of the stomach (32%) or the antrum / body region (28%). Regarding the degree of differentiation, the majority (46%) of the tumors are grade 3 (poorly differentiated) and, with respect to the histological subtype, the intestinal type was more prevalent totaling 49.38% of cases. It was also observed that 52.38% of patients underwent gastrectomy as palliative surgery. In studies of marking the mast cell carcinomas showed a significantly lower density of mast cells when compared to the control - the free margin of the disease (p = 0.01). However, no difference was observed in the density of mast cells for different ages, gender, histological type. Differences in topography and degree of differentiation. These studies demonstrated the necessity and the importance of investigating the role of mast cells in tumor regions.

Investigações epidemiológicas, clínicas, patológicas e moleculares buscando um melhor entendimento destes tumores permitem estabelecer um melhor prognóstico para a doença bem como o desenvolvimento de melhores possibilidades terapêuticas. Em especial o microambiente tumoral tem sido estudado para melhor entender as vias de interação das neoplasias com o sistema imune. Dentro deste contexto, o objetivo deste trabalho foi realizar um levantamento retrospectivo do perfil epidemiológico e anatomopatológico de pacientes com câncer gástrico em um hospital de Goiânia e avaliar aspectos patológicos, clínicos bem como a densidade de mastócitos em alguns casos selecionados. Para tanto, foram revisados 220 prontuários de pacientes com câncer gástrico avançado no período de 2005 a 2009 do hospital Araújo Jorge de acordo com critérios de inclusão estabelecidos. Coletou-se dados como sexo, idade, tipo histológico, grau de diferenciação, local de incidência do câncer de acordo com a topografia do estômago e cirurgias paliativas realizadas, para determinação do perfil-clínico patológico dos pacientes. Selecionou-se os blocos com a biópsia de alguns casos (29 casos) para corte e coloração das lâminas por imunohistoquímica e hematoxilinaeosina para avaliação da densidade de mastócitos presente nas amostras. Os resultados mostraram que a maioria dos pacientes (62,96%) eram do sexo masculino, sendo que a maioria dos casos de câncer gástrico foi encontrado em idosos com mais de 60 anos (62%). Os tumores localizaram-se preferencialmente no antro do estômago (32%) ou na região antro/corpo (28%). No que tange ao grau de diferenciação, a maioria (46%) dos tumores são de grau 3 (pouco diferenciados) e, com relação ao subtipo histológico, o tipo intestinal foi o mais prevalente totalizando 49,38% dos casos. Observou-se também que 52,38% dos pacientes fizeram a gastrectomia como cirurgia paliativa. Nos estudos de marcação dos mastócitos, os carcinomas apresentaram uma densidade significativamente menor de células mastocitárias quando comparado com o controle – margem livre de doença (p=0,01). Entretanto, não observou-se diferença na densidade de mastócitos para as diferentes idades, gênero tipo histológico. Houve diferença na topografia e grau de diferenciação.
