Resumo: INTRODUCTION : The HIV epidemic worldwide reveals the increase of new cases in individuals aged 50 years or older ( UNAIDS , 2010) . In Western Europe 12.9 % of new cases of HIV infection in 2007 were in this population . In Central Europe it is estimated that one in every ten new HIV infections contemplate the same age group , while the proportion of cases in Eastern Europe was 3.7% in the same year ( Lazzarus , Nielsen , 2010). In the U.S. 15 % of new infections are in people aged 50 years or more , these represent about 30 % of the population living with HIV in the country ( NGUYEN , HOLODNIY , 2008) . HIV infection in the elderly include issues related to gender bias , especially among women . This situation , coupled with the lack of perception of vulnerability can lead a greater number of older women to the HIV epidemic . OBJECTIVE : To describe older women infected with HIV analyze the epidemiology of women aged 50 or older with HIV attending the Reference Unit . METHODOLOGY : The study investigated the profile of HIV infection in women aged 50 years or more in the State of Goiás , with the analysis of a retrospective cohort of patients seen at the Hospital Reference . Information not found in the records were completed from the linkage with the database SINAN State. RESULTS : Older women infected with HIV have less education compared to younger ( p = 0.001 ) . The proportion of unmarried elderly women infected with HIV was statistically superior to the younger ( p = 0.003 ) . Late diagnosis , translated by counting the first T - CD4 less than 350 cells/mm3 was significantly higher in women aged 50 years or more ( p = 0.0001 ) as well as the manifestation of opportunistic infections during the first admission in Reference Unit (p = 0.001). The prevalence of opportunistic diseases in the general population of the study was 38.5 % . The distribution by age group showed that older women were more likely to express wasting syndrome ( p = 0.000 ) and toxoplasmosis ( p = 0.039 ) . There was a significant linear trend of increasing cases among older women ( p = 0.003 ) over the period observed . The probability of developing AIDS was significantly higher in older women compared to young (log rank = 10.5, p = 0.001 ) . The cumulative probability of survival according to age was different between the groups (log rank = 17.5, p = 0.000 ) and older women had lower cumulative probability of survival . CONCLUSION : The investigation of the epidemiological profile of HIV infection for women aged 50 or older has shown , the state level , the specificities and characteristics important to foster discussions about new implications of the HIV epidemic in Brazil .

Na Europa Ocidental 12,9% dos casos novos de infecção pelo HIV em 2007 foram nessa população. Na Europa Central estima-se que uma a cada dez novas infecções pelo HIV contemplem essa mesma faixa etária, enquanto que a proporção de casos na Europa Oriental foi de 3,7% no mesmo ano (LAZZARUS, NIELSEN, 2010). Nos EUA 15% das novas infecções são em indivíduos com 50 anos ou mais, estes representam cerca de 30% da população vivendo com HIV no país (NGUYEN, HOLODNIY, 2008). A infecção pelo HIV na população idosa compreende questões de preconceito relacionado ao sexo, em especial entre as mulheres. Essa conjuntura, associada a ausência da percepção de vulnerabilidade pode conduzir um número maior de mulheres idosas à epidemia do HIV. OBJETIVO: Caracterizar as mulheres idosas infectadas pelo HIV Analisar o perfil epidemiológico de mulheres com 50 anos ou mais de idade infectadas pelo HIV atendidas na Unidade de Referência. METODOLOGIA: O estudo investigou o perfil da infecção pelo HIV em mulheres com 50 anos ou mais no Estado de Goiás, com a análise de uma coorte retrospectiva composta por indivíduos atendidos no Hospital de Referência. As informações não encontradas nos prontuários foram completadas a partir do linkage com a base de dados do SINAN Estadual RESULTADOS: As mulheres idosas infectadas pelo HIV tem menor escolaridade em relação às mais jovens (p=0,001). A proporção de mulheres idosas não casadas infectadas pelo HIV foi estatisticamente superior às mais jovens (p=0,003). O diagnóstico tardio, traduzido pela contagem do primeiro T-CD4 menor que 350 cel/mm 3 foi significativamente maior em mulheres com 50 anos ou mais (p=0,0001) assim como a manifestação de doenças oportunistas já no primeiro atendimento na Unidade de Referência (p= 0,001). A prevalência de doenças oportunistas na população geral do estudo foi de 38,5%. A distribuição por grupo etário evidenciou que mulheres idosas apresentaram maior probabilidade de manifestar síndrome consumptiva (p=0,000) e neurotoxoplasmose (p=0,039). Houve uma tendência linear significativa de aumento de casos entre as mulheres idosas (p=0,003) ao longo do período observado. A probabilidade de desenvolvimento de aids foi significativamente maior nas mulheres idosas quando comparadas às jovens (log rank= 10,5; p=0,001). A probabilidade acumulada de sobrevida segundo a faixa etária foi diferente entre os grupos (log rank= 17,5;p=0,000) e mulheres mais velhas apresentaram menor probabilidade acumulada de sobrevida. CONCLUSION : The investigation of the epidemiological profile of HIV infection for women aged 50 or older has shown , the state level , the specificities and characteristics important to foster discussions about new implications of the HIV epidemic in Brazil .
