Comparação da prevalência de parasitos entéricos em gatos errantes e domiciliados em Goiânia-Goiás, análise da acurácia de técnicas parasitológicas e avaliação da copro-PCR para o diagnóstico de Toxoplasma gondii

Resumo: | Cats are backers of zoonotic agents to man, including intestinal parasites, including Toxoplasma gondii. The aim of this study was to evaluate the prevalence of enteric parasites in stray cats and cats resident in the period from March 2015 to May 2016; analyzing the accuracy of various laboratory methods for diagnosis of enteric parasites and verify Copro-PCR performance in conventional parasitological confirmation of diagnosis and differential diagnosis of T. gondii oocysts. Samples of faeces were collected processed by methods Willis, Sheather, Faust and Hoffman-Janer-Pons-Lutz (HJPL) and Copro-PCR amplification using T. gondii B1 gene. The accuracy analysis was performed by determining the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and kappa (k). We collected 65 samples of feces of stray cats captured by the CCZ of Goiania-GO and a Non Governmental Organization protector of animals and 84 samples of resident cats in Goiânia-GO, with a total of 149 samples. The prevalence of enteric parasites in cats roving was 60% (39/65) and the resident cats was 17% (14/84). In positive samples of stray cats, it was observed that 43.5% (17/39) of the animals were infected by monoparasitism, 30.7% (12/39) had eggs of Hookworm; 5.1% (2/39) T. gondii/H. hammondi oocysts; 5.1% (2/39) Toxocara cati; 2.6% (1/39) oocysts Cystoisospora sp .; and 56.5% (22/39) of the animals had multiple parasitic infections, and 18% (7/39) were infected by Hookworm, Cystoisospora sp. and T. gondii/H. hammondi; 13% (5/39) T. gondii/H. hammondi and Cystoisospora sp.; 10.2% (4/39) T. gondii / H. hammondi and Hookworms; 10.2% (4/39) for Hookworms and Cystoisospora sp.; 5.1% (2/39) for Hookworms and T. cati. In the group of resident cats, monoparasitism was observed, and 57.1% (8/14) were infected by Hookworm; 21.4% (3/14) T. gondii/H. hammondi; 14.3% (2/14) T. cati; 7.2% (1/14) of Cystoisospora sp. The accuracy analysis for Hookworms and T. gondii / H. hammondi the Faust technique performed better, and Cystoisospora sp., the HPJL technique obtained higher accuracy. In Copro-PCR of the 23 samples identified as T. gondii/H. hammondi, 6 were diagnosed as T. gondii, demonstrating the ability to perform the technical differential diagnosis. In addition to Copro-PCR it was also possible to identify the presence of genetic material of T. gondii 16.1% (24/149) of the fecal samples analyzed. 12.1% (18/149) of the positive samples by Copro-PCR were negative in parasitological examinations of conventional stool. 4% (6/149) of the samples were positive agreement by conventional parasitological examinations and Copro-PCR. Comparing the two groups, it was observed that stray cats have a high prevalence of enteric parasites in relation to pet cats. Copro-PCR is a tool capable of performing the differential diagnosis of coccidia and its high sensitivity positive samples detected even considered negative by the gold standard. These results allow us to conclude that cats have epidemiological importance in environmental contamination and transmission of zoonoses, both stray cats, as the resident, who had high parasitism in this study. |

Os objetivos deste estudo foram avaliar a prevalência de parasitos entéricos em gatos errantes e gatos domiciliados no período de março de 2015 a maio de 2016; analisar a acurácia de diferentes métodos laboratoriais para o diagnóstico destes parasitos entéricos e verificar a performance da Copro-PCR na confirmação do diagnóstico parasitológico convencional e diagnóstico diferencial de oocistos de T. gondii. As amostras de fezes coletadas foram processadas pelos métodos de Willis, Sheather, Faust e Hoffman-Janer-Pons-Lutz (HJPL) e Copro-PCR para T. gondii através da amplificação do gene B1. A análise de acurácia foi realizada determinando a sensibilidade, especificidade, valor preditivo positivo (VPP), valor preditivo negativo (VPN) e índice kappa (k). Foram coletadas 65 amostras de fezes de gatos errantes capturados pelo CCZ de Goiânia-GO e por uma Organização Não Governamental protetora de animais e 84 amostras de gatos domiciliados em Goiânia-GO, sendo um total de 149 amostras. A prevalência de parasitos entéricos nos gatos errantes foi de 60% (39/65) e nos gatos domiciliados foi de 17% (14/84). Nas amostras positivas dos gatos errantes, foi possível observar que 43,5% (17/39) dos animais estavam infectados por monoparasitismo, sendo 30,7% (12/39) apresentaram ovos de Ancilostomídeos; 5,1% (2/39) oocistos de T. gondii/H. hammondi; 5,1% (2/39) ovos de Toxocara cati; 2,6% (1/39) oocistos de Cystoisospora sp.; e 56,5% (22/39) dos animais apresentaram poliparasitismo, sendo que 18% (7/39) estavam infectados por Ancilostomídeos, Cystoisospora sp. e T. gondii/H. hammondi; 13% (5/39) por T. gondii/H. hammondi e Cystoisospora sp.; 10,2% (4/39) por T. gondii/H. hammondi e Ancilostomídeos; 10,2% (4/39) por Ancilostomídeos e Cystoisospora sp.; 5,1% (2/39) por Ancilostomídeos e T. cati. No grupo de gatos domiciliados, observou-se monoparasitismo, sendo que 57,1% (8/14) estavam infectados por Ancilostomídeos; 21,4% (3/14) por T. gondii/H. hammondi; 14,3% (2/14) por T. cati; 7,2% (1/14) por Cystoisospora sp. Na análise de acurácia para Ancilostomídeos e T. gondii/H. hammondi, a técnica de Faust apresentou melhor desempenho, e para Cystoisospora sp., a técnica de HPJL obteve maior acurácia. Na Copro-PCR das 23 amostras identificadas como T. gondii/H. hammondi, 6 eram realmente T. gondii, demonstrando a capacidade de realização de diagnóstico diferencial da técnica. Além disso a Copro-PCR também foi possível identificar a presença de material genético de T. gondii em 16,1% (24/149) das amostras fecais analisadas. Em 12,1% (18/149) das amostras positivas pela Copro-PCR, foram negativas nos exames parasitológicos de fezes convencionais. Em 4% (6/149) das amostras houve concordância de positividade pelos exames parasitológicos convencionais e Copro-PCR. Ao comparar os grupos, observou-se que os gatos errantes apresentam alta prevalência de parasitos entéricos em relação aos gatos de companhia. A Copro-PCR é uma ferramenta capaz de realizar o diagnóstico diferencial de coccídeos e sua sensibilidade elevada detectou amostras positivas mesmo consideradas negativas pelo padrão-ouro. Cats are backers of zoonotic agents to man, including intestinal parasites, including Toxoplasma gondii. The aim of this study was to evaluate the prevalence of enteric parasites in stray cats and cats resident in the period from March 2015 to May 2016; analyzing the accuracy of various laboratory methods for diagnosis of enteric parasites and verify Copro-PCR performance in conventional parasitological confirmation of diagnosis and differential diagnosis of T. gondii oocysts. Samples of faeces were collected processed by methods Willis, Sheather, Faust and Hoffman-Janer-Pons-Lutz (HJPL) and Copro-PCR amplification using T. gondii B1 gene. The accuracy analysis was performed by determining the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and kappa (k). We collected 65 samples of feces of stray cats captured by the CCZ of Goiania-GO and a Non Governmental Organization protector of animals and 84 samples of resident cats in Goiânia-GO, with a total of 149 samples. The prevalence of enteric parasites in cats roving was 60% (39/65) and the resident cats was 17% (14/84). In positive samples of stray cats, it was observed that 43.5% (17/39) of the animals were infected by monoparasitism, 30.7% (12/39) had eggs of Hookworm; 5.1% (2/39) T. gondii/H. hammondi oocysts; 5.1% (2/39) Toxocara cati; 2.6% (1/39) oocysts Cystoisospora sp .; and 56.5% (22/39) of the animals had multiple parasitic infections, and 18% (7/39) were infected by Hookworm, Cystoisospora sp. and T. gondii/H. hammondi; 13% (5/39) T. gondii/H. hammondi and Cystoisospora sp.; 10.2% (4/39) T. gondii / H. hammondi and Hookworms; 10.2% (4/39) for Hookworms and Cystoisospora sp.; 5.1% (2/39) for Hookworms and T. cati. In the group of resident cats, monoparasitism was observed, and 57.1% (8/14) were infected by Hookworm; 21.4% (3/14) T. gondii/H. hammondi; 14.3% (2/14) T. cati; 7.2% (1/14) of Cystoisospora sp. The accuracy analysis for Hookworms and T. gondii / H. hammondi the Faust technique performed better, and Cystoisospora sp., the HPJL technique obtained higher accuracy. In Copro-PCR of the 23 samples identified as T. gondii/H. hammondi, 6 were diagnosed as T. gondii, demonstrating the ability to perform the technical differential diagnosis. In addition to Copro-PCR it was also possible to identify the presence of genetic material of T. gondii 16.1% (24/149) of the fecal samples analyzed. 12.1% (18/149) of the positive samples by Copro-PCR were negative in parasitological examinations of conventional stool. 4% (6/149) of the samples were positive agreement by conventional parasitological examinations and Copro-PCR. Comparing the two groups, it was observed that stray cats have a high prevalence of enteric parasites in relation to pet cats. Copro-PCR is a tool capable of performing the differential diagnosis of coccidia and its high sensitivity positive samples detected even considered negative by the gold standard. These results allow us to conclude that cats have epidemiological importance in environmental contamination and transmission of zoonoses, both stray cats, as the resident, who had high parasitism in this study. |
