Perfil sorológico à LID-1 e PGL-I em pacientes com reação hansênica tipo I e tipo II e em pacientes não reacionais

Resumo: Introduction: Leprosy Type 1 ( T1R) and type 2 reactions ( T2R) are considered the major complicators in the clinical management of leprosy patients. Leprosy reactions can occur before, during or after multidrug therapy (MDT). T1R is associated with exacerbated Th1 type cell mediated immunity (CMI) whereas T2R is associated with Th2 type immunity characterized by strong antibody production and in some cases transitory CMI to M. leprae antigens. Objective: Assess the serological reactivity to LID-1 fusion protein and to PGL-I M. leprae antigens among multibacillary (MB), paucibacillary (PB) leprosy patients that developed reaction at diagnosis or during MDT compared to reaction free MB and PB patients. Material and Methods: Paired serum samples from 73 leprosy patients were tested by ELISA to detect IgG antibodies to LID-1 and IgM responses to PGL-I. Leprosy patients included MB that developed T1R or T2R at diagnosis or during MDT and PB patients that developed T1R at diagnosis or during MDT compared to reaction free MB and PB patients. For T1R and T2R patients, samples collected during reactions and in the absence of reactions were tested and for reaction free patients samples collected at diagnosis and after MDT were tested. Results: The decline of anti LID-1 and anti PGL-I antibodies levels was significant among MB patients with T1R at diagnosis (p <0,05). Among MB patients with T2R at diagnosis and non reactional MB leprosy patients, the decline of antibodies was significant only to LID-1 (p <0,05). Higher levels of anti LID-1 antibodies were observed in MB patients who developed T2R at diagnosis compared with unreactional patients (p = 0.020) and patients who developed T1R at diagnosis (p = 0.008). Moreover, different antibody levels were seen in patients who developed T2R during MDT compared with unreactional MB patients after MDT (p = 0.020). Conclusions: Our data suggest that high anti LID-1 antibody levels may be associated with higher risk of T2R in MB patients.

Já a RT2 está associada com a resposta imunitária do tipo Th2, caracterizada por forte produção de anticorpos e em alguns casos ativação transitória da CMI. Objetivo: Avaliar a reatividade sorológica a proteína de fusão LID-1 e ao PGL-I do M. leprae em pacientes com hanseníase multibacilares (MB) e paucibacilares (PB) com reação hansênica e em pacientes MB e PB que não desenvolveram reação. Materiais e Métodos: Foram testadas amostras um total de 73 pacientes com hanseníase MB que desenvolveram RT1 ou RT2 ao diagnóstico ou durante MDT e em pacientes PB que desenvolveram RT1 ao diagnóstico ou durante MDT comparados com pacientes MB e PB que não desenvolveram reação hansênica ao diagnóstico nem durante MDT. A presença de IgG anti LID-1 e IgM anti PGL-I foi testada por ELISA. Resultados: O declínio dos níveis de anticorpos anti LID-1 e anti PGL-I foi significativo dentre os pacientes MB que estavam em RT1 ao diagnóstico (p< 0.05).
