Avaliação sorológica e molecular de pacientes com suspeita de arboviroses em uma unidade municipal de saúde de Goiânia-Goiás

Resumo: The arboviruses has been a great concern to human health in the worldwide. In Brazil the most prevalents arboviruses are Dengue (DENV), Zika (ZIKV) and Chikungunya (CHIKV), which presents similar sintomatology, as well as Mayaro fever (MAYV), therefore, it is essential to perform laboratorial diagnosis to confirm these infections. The present study aims to perform serological and molecular evaluation of patients with suspected arboviruses dengue-like, assisted at a public health unity in the city of Goiânia, Goiás. A total of 459 samples were collected from March, 30th, 2017 to June, 20th, 2018. For the serological evaluation, commercial Bahiafarma® rapid diagnosis kits were used to detect IgM and IgG antibodies for ZIKV, DENV and CHIKV. The molecular evaluation was performed by RT-qPCR and conventional PCR technique, to detect the viral genome. Subsenquently, 11 samples were select and submitted to the viral isolation in cell culture. As a serological results, samples IgM reagents were obtained, 0.7% for ZIKV and 8.5% for DENV, however no one for CHIKV. Regarding the molecular evaluation it was obtained a percentage of 12.6% positive samples for ZIKV; 28.3% positive samples for DENV; 2.2% positive samples for CHIKV and 17% positive for MAYV. Were obtained, respectively, sensitivity and specificity of 5% and 84% for Bahiafarma® anti-DENV IgM; specificity of 99% for Bahiafarma® anti-ZIKV IgM, and specificity of 42% for for Bahiafarma® anti-CHIKV IgM. Viral isolation in cell culture had no evident cytopathic effect, however, the 11 samples were positive for ZIKV and two samples were positive for MAYV after RT-PCR performed from the cell supernatant. Although arboviruses are benign in most cases, severe cases may occur with irreversible sequelae. Thus, it is essential that confirmation of arbovirus infection be performed by sensitive laboratory diagnostic methods.

No Brasil, as arboviroses de maior incidência, são Dengue (DENV), Zika (ZIKV) e Chikungunya (CHIKV), as quais apresentam sintomatologia semelhantes, assim como ocorre também na febre Mayaro (MAYV), sendo desta forma, imprescindível a realização de diagnóstico diferencial para a confirmação destas infecções. O presente estudo tem como objetivo, realizar avaliação sorológica e molecular de pacientes com suspeita de arboviroses, atendidos em uma unidade pública de saúde do município de Goiânia, Goiás. Foram, então, coletadas 459 amostras no período compreendido entre o dia 30 de março de 2017 e 20 de junho de 2018. Para avaliação sorológica, foram utilizados kits comerciais de diagnóstico rápido Bahiafarma® para detecção dos anticorpos IgM e IgG para ZIKV, DENV e CHIKV. A avaliação molecular foi realizada por meio da técnica de RT-qPCR e PCR convencional, para detecção do genoma viral. Posteriormente, 11 amostras foram selecionadas e submetidas à técnica de isolamento viral em cultivo de celular. Como resultados sorológicos, foram obtidas amostras IgM reagentes, sendo 0,7% para ZIKV e 8,5% para DENV, porém nenhuma para CHIKV. Com relação à avaliação molecular foi obtido um percentual de 12,6% amostras positivas para ZIKV; 28,3% positivas para DENV; 2,2% positivas para CHIKV e 17% positivas para MAYV.
