Avaliação do perfil de parasitemia por hemocultura seriada em indivíduos infectados cronicamente pelo Trypanosoma cruzi

Resumo: | Over a hundred years have passed since the discovery of Chagas disease and it is still a serious challenge to public health. Hemoculture has shown to be an important parasitological method aiming the isolation of Trypanosoma cruzi as well as in doubtful diagnosis and in the validation of the specific therapeutic efficacy. Its repetitive performance is another factor that increases its sensitivity. The aim of this study was to evaluate the parasitaemia profile through serial hemocultures from chronically T. cruzi infected individuals without previous treatment. 50 individuals from both genders (60% males) which had venous blood collected three fold with one month of minimum interval between the collections participated of this study. The technique used was of Chiari et al. & Luz et al. with LIT medium resulting in seven tubes per examination. Resulting in one performed hemoculture the sensitivity was of 32% in the first, 32% in the second sample and 34% in the third. When adding up the cumulative results of three samples the positivity increased to 58% (p=0,016). When we analyzed the parasitaemia profile of the positive hemocultures, 42% were of low parasitaemia (none of the three hemocultures were positive), 30% presented median parasitaemia (one hemoculture positive from the three performed), and 28% presented high parasitaemia (two or more positive hemocultures). When we related the parasitaemia level by hemoculture versus the number of positive tubes by hemocultures we found a direct relation between the parasitaemia and the number of positive tubes. Analyze the number of tubes positive blood culture proves to be sufficient to define the profile of individuals with parasitemia middle and high parasitemia, without the need for serial blood cultures. This can be expedited with the release of results when the clinician intends to make specific treatment. |

Sua realização repetidas vezes é outro fator que aumenta sua sensibilidade. O objetivo deste trabalho foi avaliar o perfil de parasitemia por hemoculturas seriadas em indivíduos cronicamente infectados pelo T. cruzi, sem tratamento específico prévio. Participaram 50 indivíduos de ambos os sexos (60% sexo masculino), que realizaram três coletas de sangue venoso, com intervalo mínimo de um mês. A técnica foi de Chiari et al. & Luz et al. com meio LIT, semeando sete tubos por exame. Com uma hemocultura realizada a sensibilidade foi de 32% na primeira amostra, 32% na segunda e 34% na terceira. Com os resultados cumulativos das três amostras a positividade aumentou significativamente para 58% (p=0, 016). Analisando o perfil de parasitemia por hemoculturas positivas, 42% dospacientes foram de baixa parasitemia (nenhuma das três hemoculturas positivas), 30% média parasitemia (uma hemocultura positiva das três realizadas), e 28% elevada parasitemia (duas ou as três hemoculturas positivas). Quando relacionado nível de parasitemia por hemocultura versus número de tubos positivos por hemocultura, encontramos relação direta entre parasitemia e número de tubos positivos. Pacientes que apresentaram uma única hemocultura positiva (média parasitemia) tinham um ou dois tubos positivos; com duas ou três hemoculturas positivas (elevada parasitemia) de três a seis tubos positivos por hemocultura. Analisar o número de tubos positivos por hemocultura demonstra ser suficiente para definir o perfil de parasitemia dos indivíduos com média e elevada parasitemia, sem necessidade de proceder à hemocultura seriada.
