Resumo: The Pelvic incidence (PI) is the most important pelvic parameter. It is considered a fixed, anatomical, parameter that is not influenced by the pelvis's position. However, the projection of the pelvis, a three-dimensional structure, in the radiographic, two-dimensional examination, may be influenced by its positioning in relation to the x-ray bulb during image acquisition. Our aim was to describe a new way of measuring PI using computed tomography (CT) and to evaluate the reliability and reproducibility of this measurement by comparing it with the total and lumbosacral spine radiogragphys. A retrospective study was carried out in which patients who underwent radiography of the total spine, lumbosacral spine and computed tomography (CT) of the total abdomen, pelvis or lumbar spine were included, with a maximum interval of 3 months between examinations. Two independent examiners performed PI measurement, with each measurement being permorded twice, with an interval of two months betwenn assesmentes. To measure the pelvic incidence on CT, the image overlay technique was applied. SURGIMAP, Excel and IBM-SPSS 20.0 software were used to calculate IP, data tabulation and statistical analysis, respectively. The intra and inter-examiner validity and reproducibility were verified using the intraclass correlation coefficients (ICC), considering CI = 95%. During the study the examinations of 55 patients of both genders with an average age of 58.7 years (± 19) were analyzed. The mean PI in the analysis of both examiners at the two different times was 54.85° (± 13.73) on the total spine radiography, 54.06° (± 11.67) on the lumbosacral spine radiography, and 49, 96° (± 9.85) in the measurement using CT. There was good reliability and reproducibility both intra (ICC examiner 1 0,913 and ICC examiner 2 0,871) and inter-examiner (ICC avaliation 1 0,906 and ICC avaliation 2 0,824). High agreement was also found with the radiographs of the whole spine and lumbosacral spine. These findings suggest that it is possible to calculate PI using CT usind the image overlay technique, eith good reproducibility both intra and inter-observer.

É considerado um parâmetro anatômico, fixo, que não sofre influência da posição da pelve, sendo utilizado no planejamento cirúrgico e no cálculo da lordose normal. Entretanto, a projeção da pelve, uma estrutura tridimensional, no exame radiográfico, bidimensional, pode sofrer influência do posicionamento da mesma em relação a ampola de raio-x durante a aquisição da imagem. Foi objetivo desse estudo descrever uma nova maneira de medir a IP utilizando a tomografia computadorizada (TC) e avaliar a confiabilidade e reprodutibilidade dessa medida comparando com a radiografia de coluna total e de coluna lombossacra. Realizou-se um estudo observacional e analítico em base dados de dados existentes na qual foram avaliados pacientes que realizaram radiografia de coluna total, coluna lombossacra e tomografia computadorizada (TC) de abdome total, pelve ou coluna lombar, com um intervalo máximo de 3 meses entre os exames. A mensuração da IP foi realizada por dois examinadores independentes, sendo que cada medida foi realizada duas vezes, com intervalo de dois meses entre as avaliações sem identificação do paciente. Para medir a incidência pélvica na TC, aplicou-se a técnica de sobreposição de imagens. Foi utilizado os softwares SURGIMAP, Excel e IBM-SPSS 20.0 para cálculo da IP, tabulação dos dados e análise estatística, respectivamente. Verificou-se a validade e reprodutibilidade intra e inter-examinadores pelo coeficiente de correlação intraclasse (CCI), considerando IC =95%. Durante o estudo foram avaliados exames de 55 pacientes de ambos os gêneros com idade média de 58,7 anos (±19). A média da IP na análise de ambos os examinadores nos dois momentos distintos foi de 54,85° (±13,73) na radiografia de coluna total, 54,06° (±11,67) na radiografia de coluna lombossacra, e 49,96° (±9,85) na medida utilizando a TC. Houve boa confiabilidade e reprodutibilidade tanto intra (ICC examinador 1 0,913 e ICC examinador 2 0,871) como inter-examinadores (ICC avaliação 1 0,906 e ICC avaliação 2 0,824). Encontrou-se também alta concordância com as radiografias de coluna total e lombossacra.
