Resumo: This study investigates how the pharmaceutical sector has become one of the largest and most influential economic sectors of our time. It presents figures and information that support this claim and aims to elucidate the factors that contributed to such a rise. Since Alexander Fleming's discovery of penicillin in 1928, pharmaceutical industries have evolved from small entities - often family-owned and operating locally - to multinational corporations valued at hundreds of billions of dollars with global influence. However, beyond the discovery and diversification of drugs, increased demand, and expansion of production capacity, part of this evolution is underpinned by complex, and in some cases perverse, strategies prioritizing profit maximization over public and individual health. The findings of this study reveal that, over the past two decades, the sector's revenues have quadrupled, reaching $1.48 trillion in 2022, an amount comparable to the Gross Domestic Product (GDP) of developed countries such as Spain. The twenty largest companies in the sector have a combined market value of $3.5 trillion and assets totaling $1.86 trillion, generating annual revenues of $820 billion and profits of $181.6 billion. For context, the assets of these companies alone are comparable to the GDP of all Sub-Saharan African countries. Companies, like Johnson & Johnson, have a market value exceeding the GDP of 184 nations. The study also examines and discusses questionable practices adopted by the pharmaceutical industry, including investing billions in lobbying and election financing, influencing regulatory agencies, financially supporting patient organizations, sponsoring authors of "Clinical Guidelines", manipulating and concealing drug research and tests, and directing massive investments to strengthen ties with prescribers, university hospitals, and academic institutions. Concrete examples of these actions are provided, backed by studies, data surveys, and court decisions, underscoring the alarming consequences of this reality. Lastly, the research analyzes pharmaceutical marketing as a primary sales boost strategy. Despite drugs not being ordinary commodities, susceptible to promotion under the lens of rampant consumption, companies invest billions annually in directto-consumer advertising. In the Brazilian context, the pharmaceutical sector stands out as one of the main investors in marketing. In recent years, several companies in the field have ranked among the highest individual advertising spenders. The emergence of digital marketing strategies driven by the internet, advanced algorithms, and social networks, combined with advertising campaigns harmful to public and collective health, underscore a concerning and challenging scenario.

Sin embargo, además del descubrimiento y diversificación de medicamentos, del aumento de la demanda y de la expansión de la capacidad productiva, parte de esta evolución está sustentada por estrategias complejas y, en algunos casos, perversas, que priorizan la maximización de las ganancias en detrimento de la salud pública e individual. Los hallazgos de este estudio revelan que, en las últimas dos décadas, los ingresos del sector se cuadruplicaron, alcanzando 1,48 billones de dólares en 2022, una cantidad comparable al Producto Interno Bruto (PIB) de países desarrollados, como España. Las veinte mayores empresas del sector tienen un valor de mercado combinado de 3,5 billones de dólares y activos que suman 1,86 billones de dólares, generando ingresos anuales de 820 mil millones de dólares y beneficios de 181,6 mil millones de dólares. Para contextualizar, solo los activos de estas empresas son comparables al PIB de todos los países del África Subsahariana. Empresas, como Johnson & Johnson, tienen un valor de mercado que supera el PIB de 184 naciones. El estudio también examina y discute prácticas cuestionables adoptadas por la industria farmacéutica, incluyendo la inversión de miles de millones de dólares en cabildeo y financiamiento electoral, influencia sobre organismos reguladores, apoyo financiero a organizaciones de pacientes, patrocinio a autores de "Directrices Clínicas", manipulación y ocultación de investigaciones y pruebas de medicamentos, y la orientación de inversiones masivas para fortalecer vínculos con prescriptores, hospitales universitarios e instituciones académicas. Se proporcionan ejemplos concretos de estas acciones, respaldados por estudios, recopilaciones de datos y decisiones judiciales, que resaltan las consecuencias alarmantes de esta realidad. Finalmente, la investigación analiza el marketing farmacéutico como una de las principales estrategias para aumentar las ventas. A pesar de que los medicamentos no son bienes comunes, susceptibles a la promoción bajo la lente del consumo desenfrenado, las empresas invierten miles de millones anualmente en publicidad dirigida directamente al consumidor. En el contexto brasileño, el sector farmacéutico se destaca como uno de los principales inversores en marketing. En los últimos años, varias empresas del área figuraron entre los mayores gastos individuales en publicidad. La emergencia de estrategias de marketing digital impulsadas por internet, algoritmos avanzados y redes sociales, junto a campañas publicitarias nocivas para la salud pública y colectiva, ratifican un escenario preocupante y desafiante.Este trabalho investiga como o setor farmacêutico tornou-se um dos maiores e mais influentes setores econômicos da atualidade. Apresenta números e informações que respaldam essa afirmação e busca elucidar os fatores que contribuíram para tal ascensão. Desde a descoberta da penicilina por Alexander Fleming em 1928, as indústrias farmacêuticas evoluíram de pequenas entidades - muitas vezes familiares e de atuação local - para corporações multinacionais avaliadas em centenas de bilhões de dólares com influência global. Entretanto, além da descoberta e diversificação de medicamentos, do aumento da demanda e da expansão da capacidade produtiva, parte dessa evolução é sustentada por estratégias complexas e, em alguns casos, perversas, que priorizam a maximização dos lucros em detrimento da saúde pública e individual. Os achados deste estudo revelam que, nas últimas duas décadas, as receitas do setor quadruplicaram, atingindo 1,48 trilhão de dólares em 2022, um montante comparável ao Produto Interno Bruto (PIB) de países desenvolvidos, como a Espanha. As vinte maiores empresas do setor possuem um valor de mercado combinado de US$ 3,5 trilhões e ativos totalizando US$ 1,86 trilhões, gerando receitas anuais de US$ 820 bilhões e lucros de US$ 181,6 bilhões. Para contextualizar, somente os ativos dessas companhias são comparáveis ao PIB de todos os países da África Subsaariana. Empresas, como a Johnson & Johnson, possuem um valor de mercado que supera o PIB de 184 nações. O estudo também examina e discute práticas questionáveis adotadas pela indústria farmacêutica, incluindo o investimento de bilhões de dólares em lobby e financiamento eleitoral, influência sobre órgãos reguladores, apoio financeiro a organizações de pacientes, patrocínio a autores de "Diretrizes Clínicas", manipulação e ocultação de pesquisas e testes de medicamentos, e o direcionamento de investimentos maciços para fortalecer laços com prescritores, hospitais universitários e instituições acadêmicas. São fornecidos exemplos concretos dessas ações, apoiados por estudos, levantamentos de dados e decisões judiciais, que ressaltam as consequências alarmantes desta realidade. Por fim, a pesquisa analisa o marketing farmacêutico como uma das principais estratégias de aumento de vendas. A despeito dos medicamentos não serem mercadorias comuns, suscetíveis à promoção sob a lente do consumo desenfreado, as empresas investem bilhões anualmente em publicidade dirigida diretamente ao consumidor. No contexto brasileiro, o setor farmacêutico destaca-se como um dos principais investidores em marketing. In the Brazilian context, the pharmaceutical sector stands out as one of the main investors in marketing. In recent years, several companies in the field have ranked among the highest individual advertising spenders. The emergence of digital marketing strategies driven by the internet, advanced algorithms, and social networks, combined with advertising campaigns harmful to public and collective health, underscore a concerning and challenging scenario.
