Atuação do ácido tranexâmico oral ou intravenoso em pacientes submetidos a reparo artroscópico de rupturas de manguito rotador no ombro: ensaio clínico randomizado

Objective: To determine the action of oral or intravenous tranexamic acid in improving intraoperative visual clarity in patients undergoing arthroscopic repair of rotator cuff tears. Methods: This is a prospective, randomized, double-blind clinical trial carried out in a private hospital in São Luís do Maranhão, Brazil, between July 2020 and September 2022, diagnosing rotator cuff tear and failure of conservative treatment for more than three months. Patients were randomized into three groups regarding the use of tranexamic acid: Control (GC), Oral (GC), and Intravenous (GI). Results: The groups' sociodemographic data, surgery time, and anesthesia time were similar. In addition, there was no difference concerning the intraoperative interventions performed to stop the bleeding, the blood count, coagulogram values , and the inflammatory response. The primary outcome was improved objective and subjective visual clarity, which showed no differences. Conclusion: TXA use did not demonstrate benefits concerning the control group, not interfering significantly in the number of intraoperative interventions, with the inflammatory response, blood count, and coagulogram, or in the objective and subjective visual clarity between the evaluated groups.
