The practice of nurse midwives in public health policy in humane delivery care in Rio de Janeiro

Resumo: Based on the dialectic approach, the purpose of this study was to analyze the nursing practice in public policies to make birth more humane, identifying the nurses' objective and subjective concepts in birth assistance. There were interviews with nurse-midwives, and direct observation of the reality lived in this birth setting. The investigation was conducted in three public maternity hospitals of the city of Rio de Janeiro in which childbirth assisted by a nurse has been institutionalized under the rules of the policy of humanization. The analysis of the content of the interviews was compared to observations of the setting. In this confrontation, the practice of nurse-midwives arose from the contradiction between the policies of humane treatment and the patterns resulting from the biomedical model that prevails in nursing training in the institutional context.

Fue empleado el método dialéctico, con entrevistas semiestructuradas a las enfermeras obstétricas y observación directa de la atención al parto. La investigación se llevó a cabo en tres maternidades públicas de la ciudad de Río de Janeiro donde, por medidas gubernamentales de la política de Humanización al Parto, fue institucionalizado el parto asistido por enfermeras. A partir del análisis de contenido, los núcleos de significados de las entrevistas se compararon con las anotaciones en los registros de campo. En esta comparación la práctica de las enfermeras obstétricas surge como resultado de la contradicción entre la defensa de las propuestas de la política de humanización y su actuación influenciada por el modelo biomédico que aún prevalece tanto en la formación profesional como en el contexto institucional.Considerando a abordagem dialética, este estudo objetivou analisar a práxis da enfermeira na política pública de humanização ao parto e nascimento, identificando suas concepções objetivas e subjetivas, no âmbito de sua assistência ao parto. Para tal, foram realizadas entrevistas semi-estruturadas com enfermeiras obstetras, e a observação direta da assistência ao parto nessas maternidades. A pesquisa desenvolveu-se em três maternidades públicas do Município do Rio de Janeiro, nas quais o parto assistido pela enfermeira foi institucionalizado sob as diretrizes governamentais da Política de Humanização. A partir da análise de conteúdo, os núcleos de significados das entrevistas foram confrontados com os registros do diário de campo.
