Chronic renal failure and pregnancy: hopes and possibilities

Resumo: The objective of this study is to get to know the meaning of pregnancy and maternity for women at a fertile age with or without children, with a diagnostic of chronic renal failure undergoing dialysis therapy. The approach used was qualitative, based on a dialectic methodology, which is a descriptive exploratory research. Individual interviews were used with a semi-structured outline. After identifying women aged 12-49,women were chosen at random, including 6 groups of dialysis and 19 interviews were held with women who were in dialysis therapy at the Nephrology Institute in the North of the State of Minas Gerais. Interviews were then processed by discourse analysis, showing 4 categories:The meaning of maternity; Mother one way or another; Children - the great challenge - and What now? As a result of the research, we found a conflict between the dream of pregnancy and the awareness of its risks; the choice, for many of the women, for maternity without the risks of pregnancy; the hope for some to be "healed" and to realize the dream of pregnancy and maternity. Some of these women, however, have a hope for healing from the disease, which would make it possible to realize the desire to be a mother or, even, to be able to share more closely in the development of their children. It is concluded that chronic renal failure represents an obstacle in the desire of women to become pregnant, since they are aware of the risk that pregnancy can be both to them and to the child. It is recommended that, in caring for women with chronic renal failure, and submitted to dialysis therapy, there should be medical, social and psychological monitoring to provide guidance to the women and their partners.

Se utilizó el enfoque cualitativo con orientación metodológica de la dialéctica, caracterizándose como investigación exploratoria descriptiva. Como instrumento se aplicó la entrevista individual con guión semiestructurado. Después de la identificación de las mujeres de 12 a 49 años se efectuó un sorteo incluyendo los 6 seis grupos de diálisis y se realizaron 19 entrevistas con las mujeres bajo tratamiento dialítico en el Instituto de Nefrología del Norte del Estado de Minas Gerais. Las entrevistas fueron sometidas al análisis del discurso; quedaron evidentes cuatro categorías: El significado de la maternidad; Madre de una forma u otra; Hijos: el gran reto y ¿Y ahora? Como resultado de la investigación se realza el conflicto entre el sueño de la gestación y la conciencia de sus riesgos; la opción, para muchas de las mujeres, por la maternidad sin los riesgos de la gestación; la esperanza, para algunas,de que "las curen" y de realizar el sueño de la gestación y de la maternidad. Parte de dichas mujeres, sin embargo, tienen esperanza en la cura de la enfermedad, lo que permitiría realizar el deseo de ser madres o de poder compartir más de cerca el crecimiento de sus hijos. Se concluye que la insuficiencia renal crónica representa un obstáculo en el anhelo de las mujeres de quedarse embarazadas ya que son conscientes del riesgo que un embarazo puede representar tanto para ellas como para sus hijos. Se recomienda que en la atención a las mujeres portadoras de insuficiencia renal crónica sometidas a tratamiento dialítico se efectúe seguimiento médico, social y psicológico para que ellas y sus compañeros reciban la debida orientación.O objetivo com este estudo é conhecer o significado da gravidez e da maternidade para as mulheres em idade fértil que tenham filho, ou não, com diagnóstico de insuficiência renal crônica e submetidas ao tratamento dialítico. Utilizou-se abordagem qualitativa, com orientação teórico-metodológica da dialética, caracterizando-se como pesquisa descritiva exploratória. Foi utilizado como instrumento a entrevista individual com roteiro semi-estruturado. Após a identificação das mulheres de 12 a 49 anos, foi realizado sorteio, incluindo os seis grupos de diálise e realizadas 19 entrevistas com as mulheres que se encontravam em tratamento dialítico no Instituto de Nefrologia do Norte de Minas. As entrevistas foram submetidas à análise do discurso, evidenciando quatro categorias: O significado da maternidade; Mãe de um jeito ou de outro; Filhos: o grande desafio; e E agora? Destacam-se, como resultado da pesquisa, o conflito entre o sonho da gestação e a consciência de seus riscos; a opção, para muitas das mulheres pela maternidade sem os riscos da gestação; a esperança, para algumas, de serem "curadas" e de realizarem o sonho da gestação e da maternidade. Parte dessas mulheres, no entanto, tem esperança na cura da doença, o que possibilitaria realizar o desejo de ser mãe ou até mesmo de poder compartilhar mais de perto o desenvolvimento de seus filhos. Concluiu-se que a insuficiência renal crônica representa um obstáculo ao desejo das mulheres de engravidar, uma vez que elas têm consciência do risco que uma gravidez pode representar tanto para elas como para o filho.
