Adolescence: an analisis of the pregnancy decision

Resumo: | This study aims to analyze the adolescents' decision for pregnancy considering the significance that such patients attribute to the phenomenon. It is a descriptive study with a qualitative approach, based on the theoretical and methodological principles of Symbolic Interactionism and Grounded Theory. To obtain data, sixteen teenagers aged twelve to sixteen years old underwent a semi-structured interview. All patients were attending a prenatal clinic of a Federal Institution in Rio de Janeiro, Brazil. Approval from the Ethics Research Committee was acquired. All women were informed about confidentiality, patient selection and aim of the study and an informed consent was obtained. Results showed two main categories that describe the meaning of such decision: 1) "Finding yourself pregnant", whose subcategories express the desire of getting pregnant and the planning of the pregnancy; and 2) "Choosing for pregnancy", whose subcategories express the feelings of power and autonomy that follow pregnancy. By integrating these categories, we reached a representative core category of the social process: Searching for Autonomy and Power. We conclude that to honor this decision and to recognize it as a teenager's right, represent the starting point of a nursing care based on the principles of bioethics and humanization. This implies the need of trained professionals, conscious of what reproductive rights represent and who understand that these patients have the right to practice their sexuality with safety and freedom. |

Los datos fueron obtenidos en entrevistas semiestructuradas con dieciséis adolescentes de doce a dieciséis años atendidos en una clínica prenatal de una Institución Gubernamental Federal de Río de Janeiro - Brasil, con la aprobación del Comité de Ética de Investigación. Se les presentó a las mujeres una solicitación para participar en el estudio; se les avisó sobre la confidencialidad, cómo habían sido seleccionadas y el objetivo del estudio y después firmaron el respectivo consentimiento informado. Los resultados evidenciaron dos categorías que describen los significados de esta decisión: 1) "Descubriendo que está embarazada" cuyas subcategories expresaron el deseo de quedarse o no quedarse embarazada, la planificación o no del embarazo; 2) "Optando por el embarazo" cuyas subcategories expresan el descubrimiento del poder y la experiencia de autonomía que resultan del embarazo. De la integración de estas categorías surgió la categoría central representativa del proceso social: "Buscando Autonomía y Poder". Se concluye que respetar esta decisión, al reconocerla como un derecho de las adolescentes, representa el punto de partida para una atención de enfermería basada en los principios de la bioética y de la humanización. Implica la necesidad de que haya profesionales especializados, conscientes de lo que representan los derechos reproductivos y que entiendan que los adolescentes tienen el derecho de practicar su sexualidad con seguridad y libertad con derecho a adquirir información sobre medios, posibilidades, estrategias, riesgos y ventajas para decidir sobre su prole.Neste estudo, o objetivo foi analisar a decisão da adolescente pela gravidez, com base no significado que ela atribuiu ao fenômeno. Trata-se de pesquisa descritiva com abordagem qualitativa baseada nos pressupostos teórico-metodológicos do Interacionismo Simbólico e da Grounded Theory. Os dados foram obtidos por meio de entrevistas semiestruturadas, realizadas com 16 adolescentes dos 12 aos 16 anos, atendidas no ambulatório de pré-natal de uma instituição pública federal na cidade do Rio de Janeiro. O estudo ocorreu no período de janeiro a outubro de 2005. Os resultados evidenciaram duas categorias que descrevem os significados para as adolescentes dessa sua decisão: 1. Descobrindo-se grávida, cujas subcategorias expressaram o querer e o não querer engravidar, o planejamento ou não da gravidez; 2. Optando pela gravidez, cujas subcategorias expressaram a descoberta do poder e a experiência de autonomia, advindos com a gravidez. Da integração dessas categorias emergiu a categoria central representativa do processo social básico: Buscando autonomia e poder. Concluiu-se que respeitar essa decisão, reconhecendo-a como um direito das adolescentes, representa o ponto de partida para um cuidado de enfermagem sensível, pautado nos princípios da bioética e da humanização.
