Leprosy in the city of Ouro Preto, Minas Gerais: epidemiologyc and operational aspects

Resumo: Leprosy is an infectious disease caused by the Mycobacterium leprae that still has a social historical image. The development of science has not been successful in changing people's fear and prejudice towards this illness. This is a descriptive and traversal study that aims to analyze the epidemiology of leprosy in the city of Ouro Preto between 2002 and 2008, in order to establish control strategies and identify possible homogeneous areas of risk transmission. Data were collected from the National Injury Surveillance System, at the Epidemiology Office of the city. Results show that all notifications were accomplished in the General Hospital of Ouro Preto. There is a possible cluster of the disease in the district of Santa Rita's since 72% of the notifications belonged to local residents. Such information suggests there is a remaining area of high disease incidence in old Vila Rica. The average rate of leprosy in Ouro Preto was 2.1/10000 while in the district of Santa Rita it was 24.5/10000, indicating a hyperendemic region. We also observed that cases in citizens under 15 years-old represented 2.9% of the total cases. Detection rate in 2002, 2006 and 2008 equals 0.5/10.000 citizens, showing active transmission of the disease. We highlight the importance of decentralizing health services in order to improve access to diagnosis and treatment of the disease as well as to promote new researches about its history and determinants.

Se trata de un estudio descriptivo transversal realizado con el objetivo de analizar la epidemiología de la lepra en el municipio de Ouro Preto de 2002 hasta 2008, buscando crear estrategias para controlar la enfermedad en la región y determinar las posibles zonas homogéneas con riesgos de trasmisión. Los datos fueron recogidos en el Sistema de Informação de Agravos e Notificações - SINAN, en el Sector de Epidemiología del municipio. Los resultados mostraron que todas las notificaciones se recibieron en la Policlínica Municipal de Ouro Preto. Hay una posible aglomeración de la enfermedad en el distrito de Santa Rita, ya que 72% de las notificaciones eran de residentes del distrito, lo cual sugiere una zona remanente de alta incidencia de la enfermedad en la antigua Vila Rica. La tasa de detección promedio de la lepra en el municipio fue de 2,1/10.000 habitantes y en Santa Rita 24,5/10.000, que caracteriza la región como hiperendémica. También se observó que los casos en niños menores de 15 años representó 2,9% del total de casos y que la tasa de detección en los años 2002, 2006 y 2008 fue 0,5/10.000 habitantes, mostrando trasmisión activa de la enfermedad. Se constató la necesidad de descentralizar los servicios de salud para mejorar el acceso y la resolución de problemas, además de realizar más investigación sobre la historia y sus factores determinantes.A hanseníase, doença infectocontagiosa causada pela Mycobacterium leprae, possui uma imagem social histórica, e o desenvolvimento da ciência não foi capaz de mudar a resposta de medo e preconceito da sociedade perante essa doença. Trata-se de um estudo descritivo do tipo transversal, cujo objetivo foi analisar a epidemiologia da hanseníase no município de Ouro Preto, no período de 2002 a 2008, visando formular estratégias para o controle da doença na região e identificar possíveis áreas homogêneas com riscos de transmissão. Foram coletados dados do Sistema de Informação de Agravos e Notificações (SINAN), no setor de Epidemiologia do município. Os resultados mostraram que todas as notificações foram realizadas na Policlínica Municipal de Ouro Preto. Há a existência de um possível cluster da doença no distrito de Santa Rita, uma vez que 72% das notificações eram de residentes desse distrito, o que sugere uma área remanescente de alta incidência da doença na antiga Vila Rica. A taxa de detecção média da hanseníase no município foi de 2,1/10 mil habitantes e, em Santa Rita, de 24,5/10 mil, caracterizando a região como hiperendêmica. Observou-se, ainda, que os casos em menores de 15 anos representaram 2,9% do total e que a taxa de detecção em 2002, 2006 e 2008 foi de 0,5/10 mil habitantes, demonstrando transmissão ativa da doença.
