Factors that predispose to the distraction of the team of nursing during the preparation and the administration of medicines

Resumo: | This research intended to exam the theory that distraction, during the preparation and administration of medication, can lead to errors. Direct and non-participant observations of the medication process (performed by nursing team in all nursing shifts) were carried out in the Intensive Care Unit of a philanthropic hospital. During observation stage 136 drugs were administered totaling 43 mistakes, namely: dose omission, infusion speed, duct, dose, dilution and schedule. During data collection 100 factors causing the nurses' distraction and that could lead to errors were identified: telephone or mobile phone ringing; interruption by other professionals; non-standardized changes of medical prescriptions; overlapping job tasks in standardized medication times; and lack of formal care planning. Although 21% of the errors related to the wrong preparation and administration times of medication were caused by the distracting event "non-standardized changes of medical prescriptions", there was not a significant correlation between the identified errors and the factors that could lead to distraction. Therefore it can be inferred that there is no direct link between the possible distracting factors and the errors related to the preparation and to the administration of medication. |

Se realizaron observaciones directas no participantes del proceso llevado a cabo por los enfermeros de todas las guardias de la Unidad de Cuidados Intensivos de un hospital filantrópico. Durante el período de observación se administraron 136 fármacos y se comprobaron 43 errores, entre ellos: omisión, velocidad de infusión, vía, dosis, dilución y horario. Durante la recogida de datos se identificaron 100 factores que podrían causar distracción de los enfermeros y predisposición a la equivocación: el teléfono fijo o celular que suena, interrupción de otros profesionales, cambios no estandarizados de recetas médicas, superposición de distintas tareas del mismo profesional en los horarios estandarizados para la medicación y ausencia de planificación formal de la asistencia. El 21% de los errores de la medicación relacionado a la preparación y a la administración de fármacos en el horario equivocado se debió a "cambios no estandarizados de recetas médicas"; sin embargo, no hubo ninguna correlación significante entre los errores identificados y los factores que podrían llevar a la distracción. Por consiguiente puede deducirse que, en la condición estudiada, no hay relación directa entre los posibles factores de distracción y los errores relacionados a la preparación y administración de fármacos.Nesta pesquisa, propôs-se testar a hipótese de que a distração, durante o preparo e a administração de medicamentos, pode conduzir ao erro. No Centro de Terapia Intensiva (CTI) de um hospital filantrópico foram realizadas observações não participantes e diretas das atividades do processo medicamentoso desempenhadas pela equipe de enfermagem em todos os plantões. No período de observação, foram administrados 136 fármacos, totalizando 43 erros, dentre os quais omissão, velocidade de infusão, via, dose, diluição e horário. Durante a coleta de dados, foram identificados 100 fatores que poderiam causar distrações da enfermagem predispondo seu erro: telefone fixo da instituição ou celular tocando, interrupção por outros profissionais, mudanças não padronizadas de prescrições médicas, sobreposição de diferentes tarefas pelo mesmo profissional nos horários padronizados para medicação e ausência de planejamento formal da assistência. Embora 21% dos erros de medicação relacionados ao preparo e à administração de medicamentos em horário errado tenham sido induzidos pelo evento que causa distração "mudanças não padronizadas de prescrições médicas", não houve correlação significativa entre os erros identificados e os fatores que poderiam conduzir à distração.
