Maternal and child vulnerability: nonadherence factors to HIV vertical transmission prophylaxis

Resumo: This study aimed to identify nonadherence factors to HIV vertical transmission prophylaxis.The study is an integrative review through LILACS data basis from terms: HIV, pregnant women and antenatal care. The inclusion criteria were 9 unabridged Brazilian papers published from 1997 to 2007 by an electronic medium. Thematic analysis identified the following adherence factors: self-care, care for and by others. Nonadherence factors included: socioeconomic situation, femininity circumstances, access to information, silence, understanding of vulnerability situation, prejudice and healthcare services barriers. The conceptual framework for the analysis was that of vulnerability. The individual frame comprised maternal-child biological, clinical and behavioural susceptibility. The social included economical and sociocultural aspects. The programmatic frame covered the healthcare policy. In order to promote the adherence and to minimize these vulnerabilities, a multidisciplinary (biological, clinical, social and subjective) intervention is necessary in all healthcare service levels

El estudio consiste en una revisión integrativa por medio de la base de datos LILACS a partir de las palabras VIH and gestantes and cuidado prenatal. Los criterios de inclusión fueron: artículos nacionales de 1997 - 2007, con texto completo, en soporte electrónico, totalizando la muestra de nueve artículos. Por el análisis temático se verificaron factores de adhesión: cuidado de si, del otro y por el otro; y factores de no adhesión: situación socioeconómica, condiciones de femineidad, acceso a las informaciones, silencio, comprensión de la situación de vulnerabilidad, prejuicio y barreras en el servicio de salud. El cuadro conceptual del análisis fue de la vulnerabilidad. El plano individual estuvo formado por la susceptibilidad biológica, clínica y comportamental materno infantil; el plano social, por los aspectos económicos y socioculturales; y el plano programático, por la política de salud. Para promover la adhesión y minimizar esas vulnerabilidades se hace necesaria una intervención multidisciplinar en las dimensiones biológicas, clínica, social y subjetiva en todos los niveles de atención a la saludObjetivou-se com este trabalho identificar fatores de (não)adesão à profilaxia da transmissão vertical do HIV. O estudo consiste em revisão integrativa por meio da base de dados LILACS a partir das palavras HIV and gestantes and cuidado pré-natal. Os critérios de inclusão foram: artigos nacionais de 1997-2007, com texto completo, em suporte eletrônico, totalizando amostra de nove artigos. Pela análise temática, verificaram-se fatores de adesão: cuidado de si, do outro e pelo outro; e fatores de não adesão: situação socioeconômica, condições de feminilidade, acesso às informações, silêncio, compreensão da situação de vulnerabilidade, preconceito e barreiras no serviço de saúde. O quadro conceitual da análise foi o da vulnerabilidade. O plano individual foi constituído pela suscetibilidade biológica, clínica e comportamental materno-infantil; o plano social, pelos aspectos econômicos e socioculturais; e o plano programático, pela política de saúde.
