The genogram as a means to characterize the family structure of elderly patients with cognitive impairment in poverty contexts

Resumo: The evaluation of a family composition can be obtained by a specific tool called genogram, and contributes to the elderly adequate care planning. The goal of this study is to identify the family structure of elderly patients with cognitive impairment registered in Family Healthcare Centers of socially vulnerable regions. We realized individual home interviews based on assumptions of the quantitative investigation method and we obtained 45 genograms. Ethical issues in research were respected. Results show that the family composition of poor elderly patients is multigenerational; families with seven members were seen. Most part of the patients was 60 to 75 years old, had a low education level, were married and lived with a stable companion and their children. Some of them also lived with their grandchildren. The most common disease was arterial hypertension. Most elderly patients reported having a regular relation with the family members. The genogram showed to be a useful tool to delineate the family structure of poor elderly patients. It may be used by healthcare professionals, especially by nurses, since they work directly with the family members.

El objetivo de este trabajo ha sido de identificar la composición familiar de las personas de edad con alteraciones cognitivas inscritas en Unidades de Salud de la Familia de regiones de alta vulnerabilidad social. Basada en las premisas del método cuantitativo de investigación, se realizaron entrevistas individuales, domiciliarias y se trazaron los genogramas de 45 ancianos, respetando los principios éticos en investigación. Las familias de los ancianos pobres son multigeneracionales y en algunas hay hasta siete miembros. La mayoría de los ancianos tenía entre 60 y 75 años, bajo nivel de escolaridad, estaba casada y vivía con su esposo (a) / compañero (a) e hijos, algunos también vivían con los nietos. La enfermedad más citada fue hipertensión arterial. La mayoría de los ancianos afirmó tener una relación normal con sus familiares. El genograma demostró ser un instrumento útil para representar la estructura familiar de ancianos pobres y puede servir para que los profesionales de salud, en especial, de los enfermeros de Unidades de Salud de la Familia, por trabajar directamente con las familias de forma tal que dirijan sus actividades a la realidad de los ancianos pobres.A avaliação da composição familiar, obtida por meio de um instrumento denominado "genograma", contribui para o planejamento adequado do cuidado aos idosos. O objetivo com este trabalho foi identificar a composição familiar de idosos com alterações cognitivas cadastrados em Unidades de Saúde da Família (USFs) de regiões de alta vulnerabilidade social. Com base nos pressupostos do método quantitativo de investigação, foram realizadas entrevistas individuais, domiciliárias e confeccionados genogramas de 45 idosos. Os cuidados éticos em pesquisa foram observados. A composição familiar dos idosos pobres é multigeracional, sendo encontradas famílias com até sete membros. Os idosos, em sua maioria, possuíam idade entre 60 e 75 anos, baixa escolaridade, estavam casados e morando com esposo(a) /companheiro(a) e filhos. Alguns viviam com os netos também. A doença mais citada foi a hipertensão arterial. A maioria dos idosos relatou ligação normal com familiares.
