Seu diagnóstico costuma ser tardio, levando a tratamentos caros e agressivos, à mutilação e até mesmo amputação do membro. O HPV também está associado a uma lesão proliferativa benigna denominada de condiloma acuminado, que tem alta morbidade. Existem fortes evidências de que pacientes com condiloma acuminado apresentam maior chance de desenvolver tumores malignos quando comparados aos pacientes sem essa lesão. Por isso, este estudo pretendeu avaliar a presença e a genotipagem do HPV por Reação em Cadeia da Polimerase (PCR), em 23 amostras de câncer peniano e 86 de condiloma acuminado, além de verificar a carga viral do HPV-6/11 e HPV-16 e o estado físico do HPV-16 por PCR em tempo real. A prevalência da infecção por HPV foi de 52% nas amostras de carcinoma e 80% nas amostras de condiloma. Múltiplas infecções com vírus de alto e baixo risco carcinogênico foram encontradas em 12% das amostras, sendo algo frequente em lesões benignas. Foi encontrado ampla variação de carga viral tanto do HPV-6/11, quanto do HPV-16. Pode-se observar a presença frequente de altas cargas virais em lesões mais graves, entretanto a grande variação encontrada pode dificultar a utilização da carga viral como marcador no diagnóstico precoce do câncer peniano. A maioria das amostras de condiloma acuminado infectadas pelo HPV-16 apresentou algum grau de integração (75%), assim como todas as amostras de carcinoma peniano, sugerindo que a integração é um fenômeno frequente e que pode constituir um fator importante para a carcinogênese, mas não único.Penile cancer is represented by 2% of the cases of cancer associated with Human Papillomavirus (HPV). Their diagnosis is usually late, leading to expensive and aggressive treatments, mutilation and even limb amputation. HPV is also associated with a benign proliferative lesion called condyloma acuminata, which has high morbidity. There is strong evidence that patients with condylomata acuminata are more likely to develop malignant tumors when compared to patients without condyloma. Therefore, this study aimed to evaluate the presence and genotyping of HPV by Polymerase Chain Reaction (PCR), in 23 penile cancer and 86 condylomata acuminata samples, as well as to verify HPV-6/11 and HPV viral load -16 and the physical state of HPV-16 by real-time PCR. The prevalence of HPV infection was 52% in the carcinoma samples and 80% in the condyloma samples. Multiple infections with high and low risk carcinogenic viruses were found in 12% of the samples, being frequent in benign lesions. A wide range of viral load was found for both HPV-6/11 and HPV-16. It can be observed the frequent presence of high viral loads in more serious lesions, however the great variation found may hinder the use of viral load as a marker in the early diagnosis of penile cancer.
