Analisando a política de saúde brasileira, em que municípios respondem pela provisão dos serviços, sua estratégia metodológica foi direcionada para um estudo transversal dos orçamentos locais, na formação das receitas e despesas, considerando porte populacional e a inserção regional dos municípios. Ao final, a despeito do caráter equalizador das transferências condicionadas federais, identificamos assimetrias entre capacidades e condições locais que podem comprometer o acesso igualitário aos serviços de saúde, como definido na Constituição brasileira.In this paper we evaluate to what extent territorial inequalities affect capacity and local conditions for provision of services in a national public policy with decentralized implementation. Analyzing the Brazilian health policy, in which municipalities are responsible for the provision of services, their methodological strategy was directed to a cross-sectional study of local budgets in the formation of revenues and expenses considering population size and regional location of municipalities.
