Viçosa-MG is located in the area of mining forest, 223 km from Belo Horizonte, the state capital and has a population of 72,220 inhabitants. The Health Team San Sebastian Family, in Viçosa-MG observed a considerably high number of individuals with high blood pressure and that there was no specific care focused on this group. Thus, considering the risks of hypertension, this study aimed to create an action plan to improve the control of blood pressure of hypertensive patients followed for this ESF. For this work we used the Situational and Strategic Planning made the situational diagnosis. Initially meetings were held with the team to define the actions and the implementation schedule to be performed on the nodes critics: lack of education of patients, unhealthy lifestyle habits, lack of renewal of drug prescription and need for staff training. The intervention plan involved actions aimed at health education, lectures on healthy habits, awareness campaigns, renewal of prescription and staff training. All actions count on a multidisciplinary team of health. It is expected that with the development of this work at the end of a year, the target of this proposal patients can present more controlled blood pressure levels, proper body weight, and quality of life, without abandoning the treatment, regardless of the stage of disease.
