Distúrbios irruptivos e defeitos de desenvolvimento do esmalte: uma revisão de escopo

Resumo: The establishment of correct occlusion depends on the development of teeth in the correct number and position, on the potential for teeth to erupt spontaneously, and on the teeth erupting on their eruption path without obstacles. The objective of this study was to identify evidence in the literature of the association of eruption disturbances and developmental defects of enamel, through a scoping review. Studies which were primary, carried out with humans on the topic, in the format of original research and case reports were selected to compose the review. Studies were excluded if they were: off-topic, secondary, carried out with animals, including patients with systemic alterations, preterm birth, and malnourished, intubated and/or fissured people, as well as deciduous successor teeth with pulp involvement, ankylosed teeth, third molars, and teeth with dental trauma, retention due to lack of space due to early loss of deciduous teeth and/or with pericoronal cyst.The consulted databases, in February/2024, were: MEDLINE (PUBMED), Embase, Scopus, Web Of Science e Biblioteca Virtual em Saúde (BVS). No filter was used and there were no language limitations in the search. After removing duplicates, a total of 730 articles were screened, 295 of which were selected by title, and 162 were selected by abstract. Finally, 60 studies were selected for the review, 11 of which were recovered from the analysis of the references of the selected articles. The studies were published between 1959 and 2024, 12 cross-sectional or case-control, and 48 case reports, carried out in Germany, Brazil, China, Denmark, Spain, United States of America, France, Greece, India, England, Iran, Israel, Italy, Japan, Malta, Poland, Sweden, Thailand, Taiwan, Tunisia or Turkey. The enamel development defects found were mostly Amelogenesis Imperfecta, followed by Enamel Hypoplasia, Molar-Incisor Hypomineralization and Fluorosis. Eruption disturbances were mostly Retention, followed by Impaction, Ectopic Eruption and Incomplete Eruption. Incisors, canines, premolars, and first or second molars were affected. The studies included in this review reinforce the hypothesis on association between developmental defects of enamel and eruption disturbances. More observational studies are needed to better elucidate the influence of developmental defects of enamel on eruption disturbances.

O objetivo deste estudo foi identificar na literatura as evidências da associação de defeitos de desenvolvimento do esmalte e distúrbios irruptivos, através de uma revisão de escopo. Estudos primários em humanos, no formato de pesquisas originais ou relatos de casos foram incluídos. Foram excluídos estudos que não contemplam o tema em geral, secundários, realizados com animais, incluindo pacientes com alterações sistêmicas, prematuros, mal nutridos, entubados e/ou fissurados, bem como dentes permanentes sucessores de decíduos com envolvimento pulpar, anquilosados, com traumatismo dentário, com retenção devido à falta de espaço por perda precoce de dente decíduo, com cisto pericoronal e/ou terceiros molares. As bases de dados consultadas, em fevereiro/2024, foram: MEDLINE (PUBMED), Embase, Scopus, Web Of Science e Biblioteca Virtual em Saúde (BVS). Não foi utilizado nenhum filtro. Não houve limitações de idiomas na pesquisa. Retiradas as duplicatas, um total de 730 artigos foram recuperados, sendo que 295 foram selecionados pelo título, e 162 foram selecionados pelo resumo. Por último, foram selecionados 60 estudos para a revisão, sendo que 11 foram recuperados a partir da análise das referências dos artigos selecionados. Os estudos foram publicados entre 1959 e 2024, sendo 12 transversais ou caso-controle, e 48 relatos de caso, realizados na Alemanha, Brasil, China, Dinamarca, Espanha, Estados Unidos da América, França, Grécia, Índia, Inglaterra, Irã, Israel, Itália, Japão, Malta, Polônia, Suécia, Tailândia, Taiwan, Tunísia ou Turquia. Os defeitos de desenvolvimento do esmalte encontrados foram na maioria amelogênese imperfeita, seguida de hipoplasia do esmalte, hipomineralização molar-incisivo e fluorose. Os distúrbios irruptivos foram na maioria retenção, seguido de impacção, erupção ectópica e erupção incompleta. Incisivos, caninos, pré-molares, e primeiro ou segundo molares foram acometidos. Os estudos incluídos na presente revisão reforçam a hipótese da associação dos defeitos de desenvolvimento do esmalte com a ocorrência dos distúrbios irruptivos.
