Diabetes Mellitus is a common disease and is a major public health problem in Brazil and worldwide. The disease is responsible for an important number of visits in primary care. In the territory of UBS White Corn on Fora / MG Judge The problem is growing, and patients although diagnosed and using previously prescribed medications, is not getting control of the disease and seek UBS to consult periodically increasing spontaneous demand, reducing quality of life in addition to being with the consequences caused by the disease. The problem led to the construction of a PES based action plan aimed at modifying habits that affect the quality of diabetic patient care, in order to ensure better care for sufferers. For the construction of this project works were used, magazines and papers for literature review. The intervention proposes a continuous follow-up, aiming the best results of blood glucose levels, with the possibility to correct the course and it is possible to impact the problem presented. It is hoped that with this proposal that diabetic patients acquire adequate knowledge about the disease, aimed at transforming the habits and lifestyles, as well as major consequences and conditions from the disease. With good monitoring and standardized follow-up during the consultation meeting and groups, home visits and group relationship, you can improve control of the disease by avoiding blood sugar fluctuations.
