Mortalidade e morbidade dos recém-nascidos de muito baixo peso no Hospital das Clínicas de 2004 a 2010

Resumo: | Introduction: the progress of science, especially in the area of health care, has allowed newborns increasingly immature and smaller birth weights to have higher survival. Consequently, the chances of morbidity occurring increase and can even affect adulthood. Objectives: know the intra-hospital mortality and morbidity of very low weight newborns treated at Hospital das Clinicas -UFMG. Methods: data were collected retrospectively from medical records of newborns with weight 500-1499 grams, from January/2004 and December/2010 and were submitted to descriptive, bi and multivariate statistical analysis. There was an association between the independent variables (maternal and neonatal characteristics) and the outcome, death, at the level of statistical significance of 0.25 in the bivariate and 0.05 in the multivariate. It excludes patients with major malformations, those with birth weight lower than 500 g and gestational age less than 24 weeks because they were at the limit of viability in the referred hospital in addition to the infants not born in HCUFMG or transferred from other units. Deaths that occurred up to 12h of life were excluded in the analysis of morbidities. Results: during the study period, 653 newborns with birth weight of less than 1500 grams were born. 77 patients were excluded by malformations, 12 weighing less than 500 grams, 10 patients with gestational age less than 24 weeks, 2 home births, 17 patients, whose data could not be obtained (losses, 2.6%),as well as 36 deaths occurring up to 12 hours. At the end, 499 patients remained. The mean and standard deviation for gestational age was 30 weeks ± 2.68 and for weight 2.68 and 1116g ± 258. Mortality was 22.24%. The risk factors for death in this population were the birth weight and use of amines to shock treatment. Conclusion: birth weight and amines use were predictors of mortality in this population. |

Consequentemente, as possibilidades de ocorrerem morbidades aumentam e podem repercutir, inclusive, na vida adulta. Objetivos: conhecer a mortalidade e morbidade intrahospitalar dos recém-nascidos de muito baixo peso atendidos no Hospital das Clínicas - UFMG. Métodos: Os dados foram coletados retrospectivamente a partir de prontuários médicos dos RN com peso 500-1499 gramas, no período de janeiro/2004 a dezembro/2010 e foram submetidos à análise estatística descritiva, bi e multivariada. Verificou-se associação entre as variáveis independentes (características maternas e neonatais) e o desfecho, óbito, ao nível de significância estatística de 0,25 na bivariada e 0,05 na multivariada. Excluiu os pacientes com malformações maiores, aqueles com peso ao nascimento inferior a 500g e idade gestacional inferior a 24 semanas por estarem no limite de viabilidade no referido hospital além dos RN não nascidos no HC-UFMG ou transferidos de outras unidades. Os óbitos que ocorreram até 12h de vida foram excluídos na análise de morbidades. Resultados: No período estudado, nasceram 653 RN com peso ao nascimento inferior a 1500 gramas. Foram excluídos 77 pacientes por malformações, 12 com peso inferior a 500 gramas, 10 pacientes com idade gestacional inferior a 24 semanas, 2 partos domiciliares, 17 pacientes, cujos dados não foi possível obter (perdas, 2,6%), além de 36 óbitos ocorridos com até 12 horas de vida. Ao final, restaram 499 pacientes. A média e desvio padrão para idade gestacional foi de 30 semanas±2,68 e para o peso, 1116g±258. A mortalidade foi 22,24%. Os fatores de risco para óbito nessa população foram o peso ao nascimento e uso de aminas para tratamento de choque.
