Na maioria das vezes, médicos prescrevem antipsicóticos típicos e atípicos associados. Para mostrar a magnitude desse problema, uma pesquisa foi desenvolvida no Hospital público Galba Velloso na cidade de Belo Horizonte. OBJETIVO: estimar a frequência e a dosagem de antipsicóticos típicos e atípicos injetáveis em uma amostra de 1171 pacientes esquizofrênicos internados entre 2010-2013, destinados a serem tratados apenas com atípicos. METODO: Com base nas prescrições dos pacientes esquizofrênicos internados, selecionamos os 105 pacientes que utilizaram antipsicóticos atípicos com antipsicóticos convencionais. RESULTADO: Dos pacientes selecionados, 59 utilizaram risperidona, 27 a olanzapina, 11 a clozapina, 5 a quetiapina e 3 a ziprasidona. Dezessete indivíduos receberam pelo menos uma ampola de haloperidol injetável de ação rápida após o início do antipsicótico atípico, 10 receberam olanzapina IM e nenhum recebeu clorpromazina. O Haloperidol injetável foi geralmente administrado em associação com a prometazina. Os resultados agregados indicam que a olanzapina e a ziprasidona IM foram mais eficazes do que haloperidol e a prometazina IM na redução da agitação e comportamento agressivo nas primeiras 2 horas, com menor risco de efeitos colaterais extrapiramidais de curto prazo. CONCLUSÃO: O uso de haloperidol injetável de ação rápida para pacientes que usam exclusivamente atípicos orais foi mais frequente do que a olanzapina IM, mas não resultou em incremento na necessidade do uso de anticolinérgico.Introduction: Combining antipsychotics in schizophrenic patients is not a rare practice. Most of the time, doctors prescribe typical and atypical associated antipsychotics. To show the magnitude this problem, a research was developed at the Public Hospital Galba Velloso in the city of Belo Horizonte. Objective: to estimate the frequency and dosage of typical and atypical injectable antipsychotics in a sample of 1171 schizophrenic patients hospitalized between 2010-2013, intended to be treated only with atypicals. Method: Based on the prescriptions of interned schizophrenic patients, we selected the 105 patients who used atypical antipsychotics with conventional antipsychotics. Results: Of the selected patients, 59 used risperidone, 27 olanzapine, 11 clozapine, 5 quetiapine and 3 ziprasidone. Seventeen subjects received at least one quick-acting injectable haloperidol vial after the onset of the atypical antipsychotic, 10 received olanzapine IM and none received chlorpromazine. Injection Haloperidol was generally given in combination with promethazine. Aggregated results indicate that olanzapine and ziprasidone IM were more effective than haloperidol and promethazine IM in reducing agitation and aggressive behavior in the first 2 hours, with a lower risk of shortterm extrapyramidal side effects.
