Análise do Quadro de Pessoal de Enfermagem e sua Influência na Ocorrência de Eventos Adversos em Unidades de Internação de um Hospital de Médio Porte

Resumo: This dissertation aims to analyze the nursing staff and describe the occurrence of adverse events in the medical and surgical inpatient units of a medium-sized hospital. Descriptive, cross-sectional, prospective study with a quantitative approach carried out in a philanthropic hospital of medium complexity located in the municipality of Três Lagoas, state of Mato Grosso do Sul. Data were collected from August to November 2020, using the Classification System of Patients (SCP) of Fugulin, with complement in the area of care in wounded patients. The dimensioning of nursing staff was calculated according to the recommendations of the Federal Nursing Council and the Adverse Events (AEs) that occurred in the inpatient units, during the data collection period, were provided by the Patient Safety Center (PSC). Following the ethical precepts of resolutions 466/2012 and 510/2016, of the National Health Council, since the research participants signed the informed consent form and the data were collected only after approval by the Research Ethics Committee with Human Beings located at the Federal University of Mato Grosso do Sul Foundation, under nº 3.937.361. As a result of the elaboration of two articles, the first article was carried out in the medical clinic unit, 1,539 evaluations were carried out in 309 patients, most of whom, 71% of the sample, were aged 50 years or more. The Total Nursing Hours in the sector was 137.7 hours, with the largest workload for intensive care, 39.6 hours. The calculation of the DPE demonstrated the need for 30 nursing professionals, distributed among 16 nurses and 14 nursing technicians. The sector consists of 06 nurses, 10 nursing technicians and 04 nursing assistants, with a total of 20 professionals. Compared with the staff of nursing professionals made available to the sector, there was a deficit in the number of nurses and nursing technicians, in addition to the presence of nursing assistants, which is not recommended by COFEN. During the data collection period, only 04 (four) AEs were notified, half of which were related to food. The second article was carried out in the surgical inpatient unit, 1,523 evaluations were carried out in 399 patients. The total hours required for the provision of nursing care (THE) was 108.78 over a period of seven days in 24 hours, with prevalence for intermediate care (IM) 39.6 THE, followed by high dependency (AD) 32, 3 THE. According to the Nursing Dimensioning (DE), 21 professionals are needed, divided between seven nurses and 14 nursing technicians. The sector has 06 nurses, 12 nursing technicians and 02 assistants. In this unit, according to COFEN, nursing assistants are accepted, however we identified a deficit in the nurse category. During the period studied, there were nine notifications of Adverse Events (AEs), related to diet, communication, medication, surgery and falls. Note that, with the results of the articles, there is a greater deficit in the nurses category. A low rate of AE notifications was observed. It is concluded that DEP is below what is recommended by COFEN, and that EAs may be underreported. In this way, the relevance of the implementation of educational strategies for the awareness of the nursing team regarding the importance of AE notifications and the accomplishment of the SCP by the nurse for the adequacy of the DEP is perceived, providing quality in patient care and safety.

Pesquisa descritiva, transversal, prospectiva com abordagem quantitativa, realizada em um hospital filantrópico localizado no município de Três Lagoas, estado de Mato Grosso do Sul. Os dados foram coletados de agosto a novembro de 2020, em uma unidade de internação clínica e em uma unidade de internação cirúrgica, utilizando-se o Sistema de Classificação de Pacientes (SCP) de Fugulin, Gaidzinski e Kurcgant (2005), com complemento na área de cuidados em portadores de feridas. A partir da classificação obtida, calculou-se o Dimensionamento de Pessoal de Enfermagem (DPE), seguindo as recomendações da Resolução n 543, de 2017, do Conselho Federal de Enfermagem (COFEN) a fim de se identificar o quadro de pessoal de enfermagem requerido para cada um dos setores. Os Eventos Adversos (EAs) ocorridos nas unidades de internação, durante o período da coleta de dados, foram fornecidos pelo Núcleo de Segurança do Paciente (NSP). Os resultados estão apresentados em dois artigos. O primeiro artigo traz os dados obtidos na unidade de internação clínica, onde realizaram-se 1.539 avaliações em 309 pacientes, em que a maioria, 71%, apresentava idade igual ou superior a 50 anos. O Total de Horas de Enfermagem (THE) do setor foi de 137,7 horas, sendo que a maior carga de trabalho foi destinada ao cuidado de pacientes intensivos, 39,6 horas. O cálculo do DPE demonstrou a necessidade de 30 profissionais de enfermagem, em 24 horas, distribuídos entre 16 enfermeiros e 14 técnicos de enfermagem. O quadro de profissionais do setor era composto por 6 enfermeiros, 10 técnicos e 4 auxiliares de enfermagem, totalizando 20 profissionais. Dessa forma, observou-se deficit no quantitativo de enfermeiros e técnicos de enfermagem, além da presença de auxiliares de enfermagem, o que não é recomendado pelo COFEN. Durante o período de coleta de dados, houve a notificação de apenas quatro EAs, sendo metade destes relacionados à alimentação. Diante dos achados, recomendam-se investimentos na implementação de estratégias que possibilitem a aplicação diária eficiente do SCP e na promoção da cultura justa visando conscientizar a equipe de enfermagem sobre a importância da notificação dos EAs. O segundo artigo constitui-se de dados coletados na unidade de internação cirúrgica onde foram efetuadas 1.523 avaliações em 399 pacientes. O total de horas requeridas para a prestação dos cuidados de enfermagem foi de 108,78 horas, com maior carga de trabalho destinada a pacientes em cuidados intermediários, 39,6 horas. O DPE demonstrou a necessidade de 21 profissionais de enfermagem para o setor, divididos entre 7 enfermeiros e 14 técnicos/auxiliares de enfermagem. O setor disponibiliza 6 enfermeiros, 12 técnicos de enfermagem e 2 auxiliares, o que demonstra déficit de enfermeiros. No período de realização do estudo, houve nove notificações de Eventos Adversos (EAs) relacionados à dieta, comunicação, medicação, cirurgia e queda. Concluiu-se que há deficit para categoria profissional enfermeiro, número suficiente de técnicos/auxiliares de enfermagem e baixa notificação de EAs. Nota-se, com os resultados dos artigos, que há maior déficit de pessoal na categoria enfermeiros e baixa notificação de EAs, o que demonstra a necessidade de implementar a cultura justa e estratégias educativas para a conscientização da equipe de enfermagem sobre a importância das notificações de EAs e da realização da classificação diária de pacientes, pelo enfermeiro, para adequação do DPE e, consequentemente, melhoria da qualidade da assistência e da segurança dos pacientes hospitalizados.Fundação Universidade Federal de Mato Grosso do SulUFMSBrasilDownsizing Organizacional.
