Tracionamento cirúrgico-ortodôntico de incisivo central superior permanente: relato de caso

Resumo: | Introduction: Impacted teeth are those that, when the time comes to erupt into the oral cavity, remain enclosed in bone tissue and/or soft tissue, due to numerous local or systemic factors. The impacted upper central incisor is a less frequent abnormality when compared to the upper canines and has a great aesthetic and functional impact on the patient. Objective: The present study aims to report a conservative clinical treatment for impacted central incisor traction surgery, through a multidisciplinary approach. Case report: An 8-year-old male patient complained of delayed eruption of tooth 21 and reported that tooth 61 had already exfoliated about a year ago. After performing imaging tests, it was diagnosed that element 21 was included. In a multidisciplinary approach between the areas of ordodontics, surgery and pediatric dentistry, conservative surgical treatment was determined by performing traction of tooth 21, with gluing of an orthodontic button to the crown of the traction element and extraction of multiple teeth (53,55,15 and 63) in the surgical center of the Maria Aparecida Predrossian University Hospital. Surgery was performed without associated complications and after 5 days suture removal, as there was a good healing process. The patient was referred to the orthodontist for continued dental traction treatment. Conclusion: Surgical-orthodontic traction has a good prognosis, being a conservative option for treating impacted teeth in the aesthetic area. |

O incisivo central superior incluso é uma anormalidade menos frequente quando comparada aos caninos superiores sendo de grande impacto estético e funcional para o paciente. Objetivo: O presente estudo tem como objetivo relatar um tratamento clínico conservador de cirurgia de tracionamento de incisivo central incluso, através de uma abordagem multidisciplinar. Relato de caso: Paciente de 8 anos, sexo masculino, apresentava queixa de atraso na erupção do dente 21 e relatou que o dente 61 já havia esfoliado há cerca de um ano. Após realização de exames de imagens, foi observado que o elemento 21 apresentava-se incluso. Em abordagem multidisciplinar entre as áreas de ortodontia, cirurgia e odontopediatria, determinou-se um tratamento cirúrgico-conservador por meio da realização de tracionamento do dente 21, com colagem de botão ortodôntico à coroa do elemento tracionado e exodontia de múltiplos dentes (53,55,15 e 63) sob anestesia geral, no Hospital Universitário Maria Aparecida Predrossian. Foi realizada cirurgia sem complicações associadas e após 5 dias remoção de suturas, uma vez que apresentava bom processo cicatricial. O paciente foi encaminhado ao ortodontista para continuidade do tratamento de tracionamento do elemento dental em questão.
