Atuação fisioterapêutica em pacientes com dores crônicas na APS

Resumo: Chronic pain is one of the main causes of patients abandoning treatment in health units. These patients often associate pain with certain exercises proposed by the physiotherapist. A combination of factors can result in musculoskeletal discomfort and kinesiophobia, the aversion to movement due to fear of pain. Physiotherapy aims to demystify the association between pain and physical exercise, promoting the understanding that the practice of physical activity, conducted appropriately, can be an essential ally in the process of reducing pain. Systematically review published studies on chronic pain in primary health care (PHC), focusing on groups of elderly people with this condition. Electronic searches were carried out without publication year restrictions in the Medline databases via PubMed, Pedro and SciELO in June 2024. The keywords used included “Chronic pain”, “elderly”, “Chronic pain groups” and “APS”. The search strategies were adapted for each database investigated. Clinical trials and case studies involving people with chronic pain and elderly people receiving care in primary health care were included. A systematic review of the literature was carried out using the descriptors “Chronic pain”, “elderly”, “groups” and “APS” in the SciELO and Medline/PubMed databases. Two reviewers independently screened articles, including studies that deal with chronic pain in elderly patients in primary health care groups. The sample size, participant profile and primary outcomes were evaluated. The initial search with the descriptor “chronic pain” resulted in 412 articles in the SciELO database and 95 in Medline/PubMed. After inserting other descriptors, the final number of articles was reduced to six, all from Medline/PubMed.The increase in the prevalence of chronic pain in the population and its significant impact on the quality of life of individuals highlights the need to deepen physiotherapeutic activities in PHC. Understanding the context and importance of this topic is fundamental to support the intervention of physiotherapists in this clinical scenario, aiming to provide effective and comprehensive treatment to patients. Muitas vezes, esses pacientes associam a dor a determinados exercícios propostos pelo fisioterapeuta. Uma combinação de fatores pode resultar em desconforto musculoesquelético e cinesiofobia, a aversão ao movimento devido ao medo de sentir dor. A fisioterapia tem como objetivo desmistificar a associação entre dor e exercício físico, promovendo a compreensão de que a prática de atividade física, conduzida de maneira adequada, pode ser uma aliada essencial no processo de diminuição das dores. Revisar sistematicamente estudos publicados sobre dores crônicas na atenção primária à saúde (APS), focando em grupos de idosos com essa condição. Foram realizadas buscas eletrônicas sem restrições de ano de publicação nas bases de dados Medline via PubMed, Pedro e SciELO em junho de 2024. As palavras-chave utilizadas incluíram “Dores crônicas”, “idosos”, “Grupos de dores crônicas” e “APS”. As estratégias de busca foram adaptadas para cada base de dados investigada. Foram incluídos ensaios clínicos e estudos de caso envolvendo pessoas com dores crônicas e idosos atendidos na atenção primária de saúde. Realizou-se uma revisão sistemática da literatura com os descritores “Dores crônicas”, “idosos”, “grupos” e “APS” nas bases de dados SciELO e Medline/PubMed. Dois revisores efetuaram a triagem de artigos de forma independente, incluindo estudos que tratam de dores crônicas em pacientes idosos em grupos da atenção primária de saúde. Avaliou-se o tamanho amostral, o perfil dos participantes e os desfechos primários. A busca inicial com o descritor “dores crônicas” resultou em 412 artigos na base de dados SciELO e 95 na Medline/PubMed. Após a inserção de outros descritores, o número final de artigos foi reduzido para seis, todos da Medline/PubMed.O aumento da prevalência de dores crônicas na população e seu impacto significativo na qualidade de vida dos indivíduos evidenciam a necessidade de se aprofundar na atuação fisioterapêutica na APS. Compreender o contexto e a importância desse tema é fundamental para embasar a intervenção dos fisioterapeutas nesse cenário clínico, visando proporcionar um tratamento eficaz e abrangente aos pacientes.
