DISTÚRBIOS DO OLFATO E PALADAR APÓS A FASE AGUDA DA COVID-19: UM ESTUDO OBSERVACIONAL SOBRE O IMPACTO NEUROPSIQUIÁTRICO.

Resumo: | Introduction: The influence of the chemosensory disorders on quality of life has been studied since before the pandemic, since other viral infections have olfactory and gustatory disfunctions. However, the persistence of these disorders and the relation with neuropsychiatric symptoms during and after the COVID-19 pandemic has highlighted the need for studies on the impact on the mental health of the affected individuals. Objective: To estimate the frequency of chemosensory impairment after the acute phase of COVID-19 and its relationship with the development of neuropsychiatric disorders. Methods: This is a cross-sectional descriptive study based on primary data collected through a questionnaire made available on the Survey Monkey online platform between May and July 2023. The questionnaire was based on questions about sociodemographic information, acute infection data, olfactory disorder data, assessment and memory assessment. They were eligible for the study, individuals living in the state of Mato Grosso do Sul, aged over 18 years and with a previous diagnosis of COVID-19. Results: The study included 495 participants who met the eligibility criteria. The majority was residents of Campo Grande (82,7%), aged up to 39 years (44,9%) and female (70.9 %). Regarding the prevalence of chemosensory disorders, 62,4% had a decline in smell, 61% had a decline in taste and 13.3% had parosmia. Regarding the association between olfactory loss and the occurrence of neuropsychiatric symptoms, there was a greater occurrence symptoms in individuals with olfactory loss (regardless of the time of the disorder) in 7 of the 9 aspects surveyed: feeling sad or depressed, feeling impatient with people around them, inability to enjoy things as before, feeling hopelessness about the future, feelings anxious or nervous, feeling muscle tension and experience intense anxiety leading to panic. The same association with olfactory disorder was observed when were questioned about self-perceived memory problems. Conclusion: Chemosensory are a highly relevant factor in Covid-19 since they have a high prevalence and are related to a negative impact on individual’s quality of life. In addition, it is suggested that loss of smell and taste is related to greater occurence of neuropsychiatric disorders and memory deficits. |

No entanto, a persistência destes distúrbios e a relação com sintomas neuropsiquiátricos durante e após a pandemia de COVID-19 evidenciou a necessidade de estudos acerca do impacto gerado na saúde mental dos indivíduos afetados. Objetivo: Estimar a frequência de comprometimento quimiossensorial após a fase aguda da COVID-19 e o seu relacionamento com o desenvolvimento de transtornos neuropsiquiátricos. Metodologia: Trata-se de um estudo descritivo transversal fundamentado em dados primários coletados por meio de um questionário disponibilizado a partir da plataforma online Survey Monkey entre maio e julho de 2023. O questionário foi formulado a partir de questões sobre informações sociodemográficas, dados da infecção aguda, dados do distúrbio olfatório e do palada, avaliação do impacto na qualidade de vida, avaliação neuropsiquiátrica e avaliação de memória. Os dados sobre qualidade de vida e distúrbios neuropsiquiátricos foram baseados em questionários padronizados. Foram elegíveis ao estudo indivíduos moradores do estado do Mato Grosso do Sul, com idade superior a 18 anos e com diagnóstico prévio de doença COVID-19. Resultados: Foram incluídos no estudo 495 participantes que atenderam aos critérios de elegibilidade. A maioria era residente de Campo Grande (82,7%), possuía até 39 anos (44,9%) e era do sexo feminino (70,9%). Em relação à prevalência dos distúrbios quimiossensoriais, 62,4% apresentaram declínio do olfato, 61% declínio do paladar e 13,3% revelaram parosmia. Em relação à associação entre a perda olfatória e ocorrência de sintomas neuropsiquiátricos, foi verificada maior ocorrência dos sintomas nos indivíduos com perda olfatória (independente do tempo do distúrbio) em 7 de 9 aspectos pesquisados, sendo eles: sensação de tristeza ou depressão, sensação de estar impaciente com as pessoas ao redor, incapacidade de gostar pelas coisas como antigamente, sensação de desesperança em relação ao futuro, sensação de ansiedade ou nervosismo, sensação de tensão muscular e experiência de ansiedade intensa gerando pânico. Destaca-se a mesma associação com o distúrbio olfatório foi observada quando questionados sobre a auto-percepção de problema de memória. Conclusão: Os distúrbios quimiossensoriais constituem um fator de grande relevância na infecção pela Covid19 uma vez que possuem alta prevalência e estão relacionados à um impacto negativo na qualidade de vida dos indivíduos.
