Comparação dos efeitos da sedação consciente mínima realizada com extrato fixo de valeriana-lúpulo e midazolam na exodontia de terceiros molares inferiores retidos

Resumo: This prospective, split-mouth, triple-blind, randomized clinical trial aimed to analyze the comparison of the Valerian-Hop fixed extract and midazolam effects in minimal conscious sedation through the analysis of postoperative pain, anxiety, postoperative pain, vital signs and anterograde amnesia, in individuals who underwent impacted lower third molars extraction. The study was completed by 23 individuals aged between 18 and 25 years, of both sexes, with ASA I surgical risk classification and with impacted lower third molars positioned in a similar way bilaterally. The surgical interventions were performed by a single specialist in oral and maxillofacial surgery and traumatology, in two different sessions, with an interval of 21 days between them. Before all surgeries, each participant received an 8 mg dexamethasone capsule. Forty-five minutes before surgery, they randomly received, orally, 1 capsule of valerian 500 mg – Hops 120 mg or midazolam 15 mg for each side to be operated. Each participant received 12 paracetamol 750mg tablets to take, when necessary, postoperatively. The Modified Dental Anxiety Scale (MDAS) was applied at three moments (preoperative, before the first and second surgery), vital signs were analyzed in 7 moments, the sedation level was also observed and the participant took a visual analogue pain scale to answer at home, in addition to a questionnaire for the anterograde amnesia analysis. Our results showed that there was no significant difference between midazolam and Valerian-Hop considering anxiety score, in the three analyzed moments. The sedation level was higher with midazolam. Midazolam appeared to control postoperative pain better than valerian-hops. There was no statistically significant difference between treatments and the pain peak. There was also no association between the two treatments regarding your consumption and the 1st escape analgesics intake. The systolic and diastolic blood pressure values were higher when using Valerian-Hop, although the values were within the clinical normal range. Heart rate had higher values when we used midazolam only 45 minutes after administration, with no significant difference at other times. We did not find statistically significant results regarding oxygen saturation. Midazolam showed better results in relation to anterograde amnesia. We conclude that Valerian-Hops was able to establish comfort, few hemodynamic changes and few side effects, being an alternative to be used with anxious patients in preoperative period who need lower third molar extraction.

We conclude that Valerian-Hops was able to establish comfort, few hemodynamic changes and few side effects, being an alternative to be used with anxious patients in preoperative period who need lower third molar extraction.Este ensaio clínico, prospectivo, do tipo split-mouth, triplo-cego e randomizado teve como objetivo a comparação dos efeitos da sedação consciente mínima do extrato fixo de valeriana- lúpulo e do midazolam por meio de análise da dor pós-operatória, da ansiedade, sinais vitais e amnésia anterógrada, em indivíduos que foram submetidos a exodontias de terceiros molares inferiores retidos. Concluíram a pesquisa 23 indivíduos com idade entre 18 e 25 anos, de ambos os sexos, com classificação de risco cirúrgico ASA I e com terceiros molares inferiores retidos posicionados de maneira similar bilateralmente. As intervenções cirúrgicas foram realizadas por um único especialista em cirurgia e traumatologia buco-maxilo-facial, em duas sessões diferentes, com um intervalo de 21 dias entre elas. Antes de todas as cirurgias cada participante recebeu uma cápsula de dexametasona 8 mg. Quarenta e cinco minutos antes da cirurgia receberam de forma randomizada, via oral, 1 cápsula de Valeriana 500 mg – Lúpulo 120 mg ou midazolam 15 mg para cada lado a ser operado. Cada participante recebeu 12 comprimidos de paracetamol 750mg para tomar, se necessário, no pós operatório. A Escala de Ansiedade Odontológica Modificada (MDAS) foi aplicada em três momentos (consulta inicial, antes da primeira e da segunda cirurgia), os sinais vitais foram analisados em 7 momentos, o nível de sedação também foi observado e o participante levou para responder em casa uma escala analógica visual da dor, além de um questionário para análise da amnésia anterógrada. Nossos resultados mostraram que não houve diferença significativa entre midazolam e valeriana-lúpulo em relação ao escore de ansiedade, nos três momentos analisados. O nível de sedação foi maior quando utilizamos o midazolam. O midazolam pareceu controlar mais a dor pós-operatória do que a valeriana-lúpulo. Não houve diferença estatisticamente significativa entre as duas medicações e o pico da dor. Também não houve associação entre as medicações sedativas e o consumo da primeira tomada de analgésicos de escape. Os valores da pressão arterial sistólica e diastólica foram mais elevados ao utilizarmos valeriana-lúpulo, embora os valores estivessem dentro do padrão de normalidade clínica. A frequência cardíaca teve valores aumentados quando utilizamos o midazolam apenas 45 minutos após a administração, não havendo diferença significativa nos demais momentos. Não encontramos resultados estatisticamente significativos com relação a saturação de oxigênio. O midazolam apresentou melhores resultados com relação a amnésia anterógrada.
