Avaliação da qualidade de vida de pacientes com doença renal crônica em tratamento hemodialítico no Norte de Mato Grosso

Resumo: | Currently DRC affects about 35 million of the world's population. It is known that patients who require hemodialysis treatment do this blood filtration procedure for 4 hours at least three times a week, and once the patient is diagnosed with CKD and undergoing treatment, conservative or substitutive it is necessary that he Adapts to the numerous changes in living and eating habits. In this context, this study aimed to evaluate the quality of life of patients with CKD in hemodialysis treatment in the north of Mato Grosso. It is a descriptive and exploratory study, with a quantitative approach. To do so, a literature review, field research and semi-structured interview application were carried out to complete the socio-economic questionnaire (Appendix B) and the application of the Kidney Diseases Quality of Life-SF questionnaire (Annex A). Field research was carried out in the second half of 2016, at the Clinic of Renal Treatment (CTR), in Sinop - MT, after approval of the project by the Ethics and Research Committee with Human Subjects (Plataforma Brasil - CEP / CONEP - 58062016.2 .0000.5541). The study included 30 patients over 18 years, of both sexes, diagnosed with chronic renal disease of any etiology without signs of discomfort during the period of data collection. Patients younger than 18 years, undiagnosed with chronic kidney disease, were not eligible to respond to the questionnaire and who refused to participate in the study. All data were organized for analysis using the Excel software and later revised, coded and tabulated using the Graphpad Prism 5.0 software for the graphs construction. Afterwards the results were described and discussed for the final elaboration of the Work of Course and scientific article for publication. The results showed that the main dimensions changed were: physical aspects, the burden of renal disease and work status, all of them with a score below 50%. Based on the assumption that hemodialysis treatment alters the daily life routine of these patients, we expected that the respondents presented a greater impairment of the analyzed dimensions, however, 15 of the 19 dimensions surveyed presented high scores, that is, they showed a good state of health. This suggests that as difficult as it is to deal with this type of disease and its treatment, when the patient has social support (family and friends), good care and relationship with the multiprofessional team facilitates their coping and promotes a better quality of life For the chronic renal patient undergoing hemodialysis. In view of the above, it is concluded that because it is in direct contact with the chronic renal patient, it is of the utmost importance that the nurse assess the quality of life of these patients continuously, knowing how to listen to the main complaints, doubts and fears and from this can intervene In an adequate manner, with the help of a multidisciplinary team, emphasizing that as limiting as the CKD and its treatment are, the patient and the team can create coping strategies in an attempt to make the disease burden and hemodialysis treatment lighter. |

Sabe-se que os pacientes que necessitam de tratamento hemodialítico fazem este procedimento de filtração sanguínea durante 4 horas pelo menos três vezes na semana, e uma vez que o paciente é diagnosticado com DRC e submetido ao tratamento, conservador ou substitutivo é necessário que ele se adapte as inúmeras alterações dos hábitos de vida e alimentares. Neste contexto, este estudo visou avaliar a qualidade de vida de pacientes com DRC em tratamento hemodialítico no norte de Mato Grosso. Constitui-se de um estudo descritivo e exploratório, com abordagem quantitativa. Para tanto, foi realizada revisão literária, pesquisa de campo e aplicação de entrevista semi-estruturada para preenchimento do questionário sócio-econômico e aplicação do questionário Kidney Diseases Quality of Life-SF (KDQOL-SF). A pesquisa de campo foi realizada no segundo semestre de 2016, na Clínica de Tratamento Renal (CTR), na cidade de Sinop – MT, após aprovação do projeto pelo Comitê de Ética e Pesquisa com Seres Humanos (Plataforma Brasil – CEP/CONEP - 58062016.2.0000.5541). Foram incluídos no estudo 30 pacientes acima de 18 anos, de ambos os sexos, diagnosticados com doença renal crônica de qualquer etiologia sem sinais de desconforto durante o período da coleta dos dados. Foram excluídos pacientes menores de 18 anos, não diagnosticados com doença renal crônica, não aptos a responder o questionário e que se recusaram a participarem do estudo. Todos os dados foram organizados para análise utilizando o programa Excel e posteriormente revisados, codificados e tabulados utilizando o programa Graphpad Prism 5.0 para a construção dos gráficos. Em seguida os resultados foram descritos e discutidos para elaboração final do Trabalho de Curso. Os resultados obtidos demonstraram que as principais dimensões da qualidade de vida alteradas foram: aspectos físicos, o fardo da doença renal e o status de trabalho, ficando todas com escore abaixo de 50%. Partindo-se do pressuposto que o tratamento hemodialítico altera a rotina diária de vida destes pacientes, nós esperávamos que os entrevistados apresentariam um maior comprometimento das dimensões analisadas, entretanto, 15 das 19 dimensões pesquisadas apresentaram escores elevados, ou seja, demonstraram um bom estado de saúde. O que sugere que por mais difícil que seja lidar com esse tipo de doença e seu tratamento, quando o paciente tem apoio social (família e amigos), bom atendimento e relacionamento com a equipe multiprofissional facilita o seu enfrentamento e promove uma melhor qualidade de vida para o paciente renal crônico em tratamento hemodialítico.
