Introduction: Although pharmacological therapy aims to improve the quality of life of patients, polymedication is directly associated with an increased likelihood of the patient developing drug-related problems (DRPs). In addition to harming the patient, DRPs generate expenses and overload the public health system, with a greater number of hospitalizations and the use of more medication by the patient, which can lead to death. Objective: To analyze the use of medicines by patients with chronic diseases and point out the importance of the pharmaceutical professional for the health education of the population, through pharmacotherapeutic monitoring. Methodology: A case study of a user with chronic diseases is reported, attended at Unidade Básica de Saúde (UBS) in Barra do Garças/MT. Data were collected through a descriptive exploratory study with a quantitative approach and the use of the direct observation technique. Results: The user had his pharmacotherapy analyzed, and based on the data, the importance of health education for the improvement of the patient was discussed, in addition to the need to review the patient's prescriptions, when compared with the recommendations of the Medical Guidelines. Conclusion: The importance of pharmaceutical assistance in the control of DRPs and polypharmacy is evident, since it has extremely positive results for the patient's quality of life.
