A terapia comunitária integrativa como cuidado complementar em uma unidade de hemodiálise

Resumo: | People who need to be subjected to hemodialysis, in addition to physical discomfort due to physiological changes, experiencing a number of limitations related to addiction treatment, with loss of autonomy, insecurity and constant fear of death. This study aimed to evaluate the applicability of Community Therapy Integrative (TCI) technology as complementary care in a hemodialysis unit. Therefore, we sought to describe the performance of the wheels of TCI, considering all of the same steps, ie: site preparation, care, choice of topic, context, questioning, closure and evaluation and identify effects of ICT on people that characterized as elements of care in this action. We developed a search field type evaluative and interpretive held through an intervention. The study was conducted in a hemodialysis unit of the municipality of Lowland Grande / MT. There were four wheels TCI in the period April-July 2012, in all, 112 people were participating, with 68 people on hemodialysis, 11 family members, 16 employees of the unit, 2 therapists and 15 students of undergraduate and postgraduate courses. The central part of the data collection was the wheel itself TCI, being recorded by video recording, audio recorder, in addition to notes taken in the field diary. The forms of evaluation and assessment of wheels TCI filled by community therapists were also used as a data source. Data were organized and analyzed qualitatively based on thematic analysis, being presented and discussed according to the stages of ICT and highlighting the elements of care present in each of them, namely: (1) the careful organization of space; ( 2) receiving care in the TCI, (3) careful step in the choice of topic, (4) care at this stage of contextualization, (5) care in this problematization or sharing experiences, (6) the elements Care present in the closing stage; (7) present in step careful assessment TCI. The study was approved by the Research Ethics Committee of the University Hospital Júlio Müller, under Protocols and 817/CEP-HUJM/2010 70081/CEP-HUJM/2012. In conclusion, the study confirmed the assumption that ICT is a powerful strategy for the care of people in hemodialysis, their families and workers of the unit. It is a practice of care that is of importance in this environment because it provides people who experience conflict situations, such as people undergoing hemodialysis and family, the opportunity to express their feelings and thoughts about her life and promote reflection on their values and beliefs about their condition and treatment, and workers, especially nurses, the opportunity to review postures for care, so make it a practice actually more human, affective and effective. Especially, in the context of hemodialysis TCI promotes dialogue from speaking, listening and sharing experiences among all those involved in care: the workers, who are being cared for and their families. |

Esse estudo objetivou avaliar a aplicabilidade da Terapia Comunitária Integrativa (TCI) enquanto cuidado complementar em uma unidade de hemodiálise. Para tanto, buscou-se descrever a realização das rodas de TCI, considerando todas as etapas da mesma, ou seja: preparação do local, acolhimento, escolha do tema, contextualização, problematização, encerramento e avaliação; identificar os efeitos da TCI nas pessoas que os caracterizam como elementos do cuidado presentes nesta ação; ressaltar as possibilidades e limites da TCI no contexto da hemodiálise. Foi desenvolvida uma pesquisa de campo do tipo avaliativa e interpretativa, realizada por meio de uma intervenção. O estudo foi desenvolvido em uma unidade de hemodiálise do município de Várzea Grande/MT. Foram realizadas quatro rodas de TCI, no período de abril a julho de 2012, com 112 pessoas participantes, sendo 68 pessoas em hemodiálise, 11 familiares, 16 trabalhadores da unidade, dois terapeutas comunitários e 15 estudantes de graduação e de pós-graduação. A produção dos dados empíricos foi realizada, principalmente, a partir da realização das rodas de TCI, sendo registradas por meio de gravação em vídeo, gravador de áudio, além das anotações realizadas em diário de campo. Os formulários de avaliação e apreciação das rodas de TCI preenchidos pelas terapeutas comunitárias também foram utilizados como fonte de dados. Os dados foram organizados e analisados qualitativamente com base na técnica de análise temática, sendo apresentados e discutidos segundo as etapas da TCI e destacando os elementos de cuidado presentes em cada uma delas, ou seja: (1) o cuidado na organização do espaço; (2) o cuidado no acolhimento da TCI; (3) o cuidado na etapa da escolha do tema; (4) o cuidado presente na etapa da contextualização; (5) o cuidado presente na problematização ou compartilhamento de experiências; (6) os elementos de cuidado presentes na etapa de encerramento; (7) o cuidado presente na etapa de avaliação da TCI. A pesquisa foi aprovada pelo Comitê de Ética em Pesquisa do Hospital Universitário Júlio Müller, sob protocolos nº 817/CEP-HUJM/2010 e 70081/CEPHUJM/2012. Como conclusão, o estudo confirmou o pressuposto de que a TCI é uma prática potente para o cuidado complementar para pessoas em hemodiálise, aos seus familiares e aos trabalhadores da unidade. É uma prática de cuidado que se reveste de importância nesse ambiente, pois fornece às pessoas que vivenciam situações conflitantes, tais como as pessoas em hemodiálise e seus familiares, a oportunidade de expressar seus sentimentos e pensamentos sobre sua vida e promover uma reflexão sobre seus valores e crenças a respeito da sua condição e o tratamento. Para os trabalhadores da unidade, especialmente os de enfermagem, a TCI se revelou como uma oportunidade de rever posturas para o cuidado, de modo a torná-lo uma prática realmente mais humana, afetiva e efetiva.
