Padrões alimentares de refeições e sintomas depressivos : estudo em uma coorte de estudantes universitários

Resumo: Depression is one of the most important mental disorders nowadays, with increasing prevalence worldwide. University students represent a risk group for the development of the disease, as they are subjected to new life situations, often stressful and challenging. In general, this population presents unhealthy eating habits, which may be associated with the development of depression. Thus, the central theme of this thesis is an approach to eating patterns and depressive symptoms among young college students. For this, two scientific articles were prepared. The first article aimed at verifying the incidence of depressive symptoms among college students and the sociodemographic, economic and lifestyle factors associated. We analyzed the four-year follow-up data from the ELESEU cohort, counted from university admission. The presence of depressive symptoms in the years following baseline was assessed by the Patient Health Questionaire-9. Incidence and incidence rate of depressive symptoms were calculated. Higher incidence was observed in female students; the incidence in the 2nd year of the course was higher for both sexes. Among men, the risk of developing depression during college life was higher among younger people and among those with moderate or high degrees of stress in the first year of the course. Among women, the risk was higher for those with high stress and who smoked and consumed alcohol at the same time when they entered university. In the second article, we sought to know the eating habits of college students in the first year of undergraduate education and associated factors. A total of 685 incoming students in the first semester of 2016 and 2017 years participated in the study. Food intake was assessed using the 24-hour Dietary Recall. Exploratory factor analysis was used to identify the dietary patterns (DP) of the main meals (breakfast, lunch and dinner). Principal component analysis was adopted for factor extraction. General linear model with logarithm link and gamma family was used to analyze the association between sociodemographic, economic, lifestyle and eating behavior variables with adherence to DP at each meal. Three DP were extracted for each meal. For breakfast, the standards were called “University Restaurant”, “Coffee and Tea” and “Snack”. For lunch, the “Traditional”, “Western” and “Vegetarian” standards were extracted, and for dinner, the “University Restaurant”, “Western” and “Fashion Diet” standards. Students who ate meals on campus had higher adherence to the “University Restaurant” and “Coffee and Tea” DP for Dinner, and the “University Restaurant” for Dinner, and lower adherence to the “Lunch” DP, “Traditional” lunch and dinner. “Western” and “Trendy Diet” dining, showing the influence of food availability on campus on student eating patterns. Finally, in the third article, it was analyzed whether the lunch dietary patterns established at baseline were associated with the development of depression during the follow-up study. It was observed that, among girls, the greater adherence to the “Vegetarian” lunch dietary pattern at baseline increased the risk of depression when compared to those with lower adherence to this dietary pattern. Among boys, there was no association between the DP adherence and depression risk.

Estudantes universitários representam um grupo de risco para o desenvolvimento da doença, por estarem submetidos a situações novas de vida, muitas vezes estressantes e desafiadoras. De modo geral, essa população apresenta hábitos alimentares não saudáveis, que podem se associar ao desenvolvimento da depressão. Assim, a temática central dessa tese é uma abordagem sobre padrões de consumo de refeições e sintomas depressivos entre jovens universitários. Para isso, foram elaborados três manuscritos científicos. O primeiro manuscrito teve o objetivo de verificar a incidência de sintomas depressivos entre estudantes universitários e os fatores sociodemográficos, econômicos e do estilo de vida associados, analisou-se os dados de quatro anos de seguimento da coorte do ELESEU, contados a partir do ingresso na universidade. A presença de sintomas depressivos nos anos seguintes à linha de base foi avaliada pelo Patient Health Questionaire-9. Foram calculadas a incidência e a densidade de incidência dos sintomas depressivos. Observou-se maior incidência em estudantes do sexo feminino; a incidência no 2o ano do curso foi maior para ambos os sexos. Entre os homens, o risco de desenvolver depressão durante o período de estudo foi maior entre os mais jovens e entre aqueles com graus moderado ou elevado de estresse no 1o ano do curso. Entre as mulheres, o risco foi maior para aquelas que apresentavam grau elevado de estresse e que fumavam e consumiam bebida alcoólica concomitantemente quando ingressaram na universidade. No segundo manuscrito, buscou-se conhecer os padrões alimentares (PA) de refeições dos estudantes universitários no primeiro ano da graduação e fatores associados. Participaram dessa análise 685 estudantes ingressantes nos anos de 2016 e 2017, no primeiro semestre letivo. O consumo alimentar foi avaliado por Recordatório Alimentar de 24 horas. Para extração dos fatores adotou-se a análise de componentes principais. Modelo linear generalizado com ligação logarítmica e família gama foi utilizado para analisar a associação entre variáveis sociodemográficas, econômicas, do estilo de vida e do comportamento alimentar à adesão aos PA em cada refeição. Os PA de cada refeição principal (desjejum, almoço e jantar) foram identificados por análise fatorial exploratória. Foram extraídos três PA para cada refeição. Para o desjejum, os padrões foram denominados “Restaurante Universitário”, “Café e chá” e “Lanche”. Para o almoço foram extraídos os padrões “Tradicional”, “Ocidental” e “Vegetariano” e, para o jantar, os padrões “Restaurante Universitário”, “Ocidental” e “Dieta da moda”. Estudantes que consumiam as refeições no campus apresentaram maior adesão ao PA “Restaurante Universitário” e “Café e chá” do Desjejum e “Restaurante Universitário” do Jantar, e menor adesão ao PA “Lanche” de desjejum, “Tradicional” de almoço e “Ocidental” e “Dieta da moda” de jantar, mostrando a influência da disponibilidade de alimentos no campus no padrão alimentar das refeições dos estudantes. Finalmente, no terceiro manuscrito foi analisado se os padrões alimentares de almoço estabelecidos na linha de base estavam associados ao desenvolvimento de depressão durante o período de seguimento dos estudantes. Observou-se que, entre as meninas, a maior adesão ao padrão “Vegetariano” de almoço na linha de base amentou o risco de depressão quando comparadas àquelas com menor adesão a esse padrão.
