Resumo: | The increase in life expectancy from technological and scientific advances related to health and improvement of quality of life has contributed to the growth of the elderly population in the world scenario, active and healthy aging being the great challenge in this process. Thus, it is evident the need to develop studies that address aspects of functionality and health of the elderly population. Aging causes physiological and metabolic changes that lead to decreased function and reduced muscle strength, central themes of this thesis. Functional capacity is influenced by genetic factors and has as a strong predictor physical performance, also being determined by sociodemographic, economic, psychological, presence of morbidities, lifestyle behaviors and nutritional factors. This thesis presents two scientific manuscripts as products. The first manuscript aimed to analyze the association between anthropometric markers and handgrip strength in the elderly. This is a cross-sectional study with 485 elderly of both sexes and age ≥ 60 years. The anthropometric markers evaluated were: Body Mass Index, Mid-upper Arm Circumference and Calf Circumference. Handgrip strength (HGS) was measured by means of a hydraulic dynamometer manual Jamar®, accepted as a reference instrument for assessing the grip force. Anthropometric markers were measured using standardized techniques and sociodemographic, economic and lifestyle data were collected by trained interviewers. Generalized linear models (GLM) with log link and gamma family were used to analyze the association of anthropometric markers with maximum HGF. Directed acyclic graph (DAG) was constructed to identify potential confounding factors. The calf circumference remained associated with maximal HGS, after adjusting for potential confounding factors. For each centimeter increased in the calf, HGS increased 4% (β= 1.04, 95%CI: 1.03;1.07) in men and 1% (β= 1.01, 95%CI: 1.00;1.02) in women. The second manuscript estimated the prevalence of poor physical performance and associated factors in elderly women. Physical performance was assessed by the Short Physical Performance Battery (SPPB), which provides an objective measure of physical performance, based on physical tests. The associated factors were assessed by a questionnaire that included sociodemographic, economic, lifestyle behaviors and weight and health conditions. The association between the independent variables and the low physical performance was estimated by adjusted Poisson regression models with robust variance, adopting the hierarchical input of variables in the model. The prevalence of poor physical performance among the elderly was 44.0%. The variables associated with poor physical performance were: age ≥ 70 years, low education, sedentary behavior, low handgrip strength and diabetes mellitus. In conclusion, the study results confirmed the association of calf circumference with HGS, regardless of potential confounding factors, in both sexes. The prevalence of low physical performance was high and associated with variables of different levels of determination among elderly women. This information can be useful in planning public policies for healthy aging. |

This information can be useful in planning public policies for healthy aging.O aumento da expectativa de vida proveniente dos avanços tecnológicos e científicos relacionados à saúde e melhoria da qualidade de vida tem contribuído para o crescimento da população idosa no cenário mundial, inclusive no Brasil, sendo o envelhecimento ativo e saudável o grande desafio nesse processo. Evidencia-se, assim, a necessidade do desenvolvimento de estudos que abordem aspectos da funcionalidade e de saúde da população idosa. O envelhecimento acarreta mudanças fisiológicas e metabólicas que levam à queda de função e redução da força muscular, temáticas centrais dessa tese. A capacidade funcional é influenciada por fatores genéticos e tem como forte preditor o desempenho físico, sendo também determinada por fatores sociodemográficos, econômicos, psicológicos, presença de morbidades, comportamentos do estilo de vida e fatores nutricionais. Essa tese apresenta como produtos, dois manuscritos científicos. O primeiro manuscrito teve o objetivo de analisar a associação entre marcadores antropométricos e força de preensão manual em idosos. Trata-se de um estudo transversal, com 485 idosos de ambos os sexos e idade ≥ 60 anos. Os marcadores antropométricos avaliados foram: Índice de Massa Corporal, Perímetro do Braço e Perímetro da Panturrilha. A força de preensão manual (FPM) foi mensurada por meio de dinamômetro hidráulico manual Jamar®, aceito como instrumento de referência para avaliação da força de preensão. Os marcadores antropométricos foram mensurados utilizando-se técnicas padronizadas e dados sociodemográficos, econômicos e do estilo de vida foram coletados por entrevistadores treinados. Para analisar a associação dos marcadores antropométricos com a FPM máxima utilizou-se modelos lineares generalizados (GLM) com link de log e família gama. Gráfico acíclico direcionado (DAG) foi construído para identificar potenciais fatores de confusão. O perímetro da panturrilha permaneceu associado à FPM máxima, após ajuste para potenciais fatores de confusão. Para cada centímetro acrescido na panturrilha, a FPM aumentou 4% (β= 1,04, IC95: 1,03;1,07) em homens e 1% (β= 1,01, IC95%: 1,00;1,02) em mulheres. No segundo manuscrito estimou-se a prevalência de baixo desempenho físico e fatores associados em mulheres idosas. O desempenho físico foi avaliado pelo instrumento Short Physical Performance Battery (SPPB), que fornece uma medida objetiva do desempenho físico, baseado na realização de testes físicos. Os fatores associados foram avaliados por questionário que incluiu variáveis sociodemográficas, econômicas, comportamentos do estilo de vida e condições de peso e saúde. A associação entre as variáveis independentes e o baixo desempenho físico foi estimada por modelos ajustados de regressão de Poisson com variância robusta, adotando-se a entrada hierárquica de variáveis no modelo. A prevalência de baixo desempenho físico entre as idosas foi de 44,0%. As variáveis associadas ao baixo desempenho físico foram: idade ≥70 anos, baixa escolaridade, comportamento sedentário, baixa força de preensão manual e diabetes mellitus. Em conclusão, os resultados do estudo confirmaram a associação do perímetro da panturrilha à FPM, independentemente de potenciais fatores de confusão, em ambos os sexos. A prevalência de baixo desempenho físico foi elevada e associada com variáveis de diferentes níveis de determinação entre as mulheres idosas.
