Análise das características intervenientes na conformação do ambiente de prática do enfermeiro no espaço hospitalar

Resumo: | ABSTRACT: Introduction: Seeking improvements in health care, in order to ensure patient safety and the quality of care provided, requires investing in professional nursing practice by promoting favorable work environments. Analyzing the work environment in which nurses are developing their professional practice should be a priority for nursing managers, and the first step is to verify whether or not there are factors that facilitate or hinder professional practice. In order to address the characteristics that are present in the nurse's practice environment and in understanding its influence for professional practice, the question is: how do nurses assess their practice environment? Do sociodemographic and professional factors influence nurses' perception of their practice environment? Objective: To analyze the nurse's practice environment in the hospital space. Method: A cross-sectional, analytical study, with a quantitative approach, developed in a public hospital under municipal management, maintained by the Unified Health System, in the city of Cuiabá - MT, Brazil. The population accessed consisted of 117 professional nurses, assigned to the hospital, the locus of the research and, in exercise, with the nursing service. Data collection took place in the last semester of 2019, using three instruments, namely: sociodemographic and professional profile; economic classification criteria ABEP (2018) of professionals and the Practice Environment Scale - PES. The collected data were analyzed and described using Statistics / Data Analysis (Stata) version 12. Descriptive statistics were used with the elaboration of tables of absolute (n) and percentage (%) of categorical variables and calculation of position measurement. and dispersion (median, standard deviation, minimum and maximum values) for continuous quantitative variables. For the correlation analysis of the dependent and independent variables, we used the Fisher's Correlation test. The study was approved by the Ethics Committee on Health Research with Human Beings of the Federal University of Mato Grosso - Campus Rondonópolis, under the opinion number 3.432.757. Results: The characterization of the nurses' sociodemographic and professional profile found a predominance of females (82.52%), married / stable partners (45.63%), the average age was 39.58 years and 65.05% nurses declared themselves to be heads of household. The health institution studied was classified as an unfavorable environment for professional practice, obtaining an average of 2.35 of the general PES score. The analyzes revealed that 62.14% of nurses perceived their practice environment as unfavorable, the only dimension perceived for having favorable characteristics was the “collegial relations between nurses and doctors” with 68.93%. The nature of the bond (p <0.004), the increase in working time at the institution (p <0.025) and the type of hospitalization unit that is filled (p <0.044) have a significant relationship with the nurses' perception of the environment professional practice. The largest proportion of nurses noticed a lack of professional recognition on the part of management, limitations in actions for professional development and an insufficient number of nursing staff to provide quality care to patients. Conclusion: Finally, it is concluded that the nurses participating in the research believe they have few favorable characteristics present in the professional practice environment that contribute to their participation in the discussion of hospital issues; little access to the fundamentals of nursing focused on the quality of care; skill, leadership and support from nursing coordinators / supervisors to nurses / nursing staff; lack of adequacy of the team and resources. Recognizing the problem, it is recommended to consider these findings, in order to subsidize the managers of health institutions to the conditions of planning, adequacy of conducts and actions that seek the best working conditions for nurses to develop professional practice, reinforcing the importance of presence of favorable characteristics in the nursing practice environment. |

Analizar el ambiente de trabajo en el que las enfermeras están desarrollando su práctica profesional debería ser una prioridad para los gerentes de enfermería, y el primer paso es verificar si existen factores que faciliten o dificulten la práctica profesional. Para abordar las características que están presentes en el entorno de práctica de la enfermera y para comprender su influencia en la práctica profesional, la pregunta es: ¿cómo evalúan las enfermeras su entorno de práctica? ¿Los factores sociodemográficos y profesionales influyen en la percepción de las enfermeras sobre su entorno de práctica? Objetivo: analizar el entorno de práctica de la enfermera en el espacio hospitalario. Método: Estudio analítico transversal, con enfoque cuantitativo, desarrollado en un hospital público de gestión municipal, mantenido por el Sistema Único de Salud, en la ciudad de Cuiabá - MT, Brasil. La población visitada estaba compuesta por 117 enfermeras profesionales, asignadas al hospital, el lugar de la investigación y, en ejercicio, con el servicio de enfermería. La recolección de datos tuvo lugar en el último semestre de 2019, utilizando tres instrumentos, a saber: perfil sociodemográfico y profesional; Criterios de clasificación económica ABEP (2018) de profesionales y la Escala de Práctica del Medio Ambiente - PES. Los datos recopilados se analizaron y describieron utilizando Estadísticas / Análisis de datos (Stata) versión 12. Las estadísticas descriptivas se utilizaron con la elaboración de tablas de absoluto (n) y porcentaje (%) de variables categóricas y el cálculo de la medición de posición. y dispersión (mediana, desviación estándar, valores mínimos y máximos) para variables cuantitativas continuas. Para el análisis de correlación de las variables dependientes e independientes, utilizamos la prueba de correlación de Fisher. El estudio fue aprobado por el Comité de Ética en Investigación de Salud con Seres Humanos en la Universidad Federal de Mato Grosso - Campus Rondonópolis, con el número de opinión 3.432.757. Resultados: La caracterización del perfil sociodemográfico y profesional de las enfermeras encontró un predominio de mujeres (82.52%), parejas casadas / estables (45.63%), la edad promedio fue de 39.58 años y 65.05% las enfermeras se declararon jefas de hogar. La institución de salud estudiada se clasificó como un entorno desfavorable para la práctica profesional, obteniendo un promedio de 2.35 del puntaje general de PSA. Los análisis revelaron que el 62.14% de las enfermeras percibieron su entorno de práctica como desfavorable, la única dimensión percibida por tener características favorables fue las "relaciones colegiales entre enfermeras y médicos" con el 68.93%. La naturaleza del vínculo (p <0.004), el aumento del tiempo de trabajo en la institución (p <0.025) y el tipo de unidad de hospitalización que se llena (p <0.044) tienen una relación significativa con la percepción del ambiente por parte de las enfermeras. practica profesional. La mayor proporción de enfermeras notó una falta de reconocimiento profesional por parte de la gerencia, limitaciones en las acciones para el desarrollo profesional y un número insuficiente de personal de enfermería para brindar atención de calidad a los pacientes. Conclusión: Finalmente, se concluye que las enfermeras que participan en la investigación creen que tienen pocas características favorables presentes en el entorno de práctica profesional que contribuyan a su participación en la discusión de temas hospitalarios; escaso acceso a los fundamentos de enfermería centrados en la calidad de la atención; habilidad, liderazgo y apoyo de coordinadores / supervisores de enfermería a enfermeras / personal de enfermería; falta de adecuación del equipo y los recursos. Reconociendo el problema, se recomienda considerar estos hallazgos, a fin de subsidiar a los gerentes de las instituciones de salud a las condiciones de planificación, adecuación de conductas y acciones que busquen las mejores condiciones de trabajo para que las enfermeras desarrollen prácticas profesionales, lo que refuerza la importancia de presencia de características favorables en el entorno de práctica de enfermería.CAPESIntrodução: Buscar melhorias na assistência à saúde, a fim de garantir a segurança do paciente e a qualidade do cuidado prestado requer investir na prática profissional de enfermagem promovendo ambientes de trabalho favoráveis. Analisar o ambiente de trabalho em que os enfermeiros estão desenvolvendo sua prática profissional deve ser prioridade dos gestores de enfermagem, sendo que o primeiro passo é verificar se existem ou não fatores que facilitam ou que dificultam a prática profissional. No intuito de abordar as características que estão presentes no ambiente de prática do enfermeiro e na compreensão de sua influência para a prática profissional, questiona-se: como os enfermeiros avaliam seu ambiente de prática? Os fatores sociodemográficos e profissionais influenciam na percepção dos enfermeiros sobre seu ambiente de prática? Objetivo: Analisar o ambiente de prática do enfermeiro no espaço hospitalar. Método: Estudo com delineamento transversal, analítico, com abordagem quantitativa, desenvolvido em um hospital público sob a gestão municipal, mantido pelo Sistema Único de Saúde, no município de Cuiabá – MT, Brasil. A população acessada foi constituída por 117 profissionais enfermeiros, lotados no hospital, lócus da pesquisa e, em exercício, junto ao serviço de enfermagem. A coleta dos dados aconteceu no último semestre de 2019, por meio de três instrumentos, sendo: perfil sociodemográfico e profissional; critério de classificação econômica ABEP (2018) dos profissionais e o Practice Environment Scale – PES. Os dados coletados foram analisados e descritos por meio do Statistics/Data Analysis (Stata) versão 12. Utilizou-se de estatísticas descritivas com elaboração de tabelas de frequências absoluta (n) e percentual (%) das variáveis categóricas e cálculo de medida de posição e dispersão (mediana, desvio-padrão, valores mínimo e máximo) para variáveis quantitativas continuas. Para a análise de correlação das variáveis dependentes e independentes utilizamos o teste de Correlação de Fisher’s. O estudo foi aprovado pelo Comitê de Ética em Pesquisa em Saúde com Seres Humanos da Universidade Federal de Mato Grosso – Campus Rondonópolis, com número do parecer 3.432.757. Resultados: A caracterização do perfil sociodemográfico e profissional dos enfermeiros constatou a predominância do sexo feminino (82,52%), casados/união estável (45,63%), a idade média foi de 39,58 anos e 65,05% dos enfermeiros autodeclararam ser chefes de família. A instituição de saúde estudada foi classificada como um ambiente desfavorável para a prática profissional, obtendo-se média de 2,35 do escore geral do PES. As análises revelaram que 62,14% dos enfermeiros perceberam seu ambiente de prática como desfavorável, a única dimensão percebida por possuir características favoráveis foi a “relações colegiais entre enfermeiros e médicos” com 68,93%. Conclusão: Por fim, conclui-se que os enfermeiros participantes da pesquisa julgam possuir poucas características favoráveis presentes no ambiente de prática profissional que contribuam para a participação destes na discussão dos assuntos hospitalares; pouco acesso aos fundamentos de enfermagem voltados para a qualidade do cuidado; habilidade, liderança e suporte dos coordenadores/supervisores de enfermagem aos enfermeiros/equipe de enfermagem; falta de adequação da equipe e dos recursos.
