Resumo: | ABSTRACT: The adoption of preventive measures (PM) is the best way for the elderly to prevent falls, however they find it difficult to adopt these measures in their daily lives. Objective: to evaluate the effect of an educational intervention on the elderly's adherence to preventive measures against falls and beliefs in health. Materials and Method: quasiexperimental study, with a time-series design, carried out at the Community Center for Seniors in Cuiabá, from June to October 2019. The sample was determined considering error type I = 5%, error type II = 20%, proportion of adherence to falls MP = 64.36%, minimum difference before and after intervention = 4%, resulting in a minimum sample of 66 elderly people. 30% were added to the sample, predicting losses, and ended with 97 elderly people. Included: people aged 60 and over and who performed at least one of the physical activities. Excluded: people who had cognitive and mental difficulties. 109 elderly people were recruited (baseline), of whom 68 completed an intervention protocol. The educational intervention was based on the Health Belief Model (HBM) and carried out in six stages (T0 to T5). Validated instruments were applied for pre and post-intervention assessments: sociodemographic characteristics and health conditions, health beliefs and adherence to falls PM. Simple (unadjusted) and adjusted multiple linear regression analyzes and paired t and Wilcoxon tests for paired samples were used. Results: the majority of the elderly recruited is a woman (86.24%), a widow (37.61%), had 1 to 4 years of study, average age of 69.73 years, with health problems (93.58 %) and 46.79% fell in the last year. In simple linear regression, dimensions 3 (perception of the benefits of adhering to falls PM), 4 (perception of barriers to adhering to PM falls) and the total score of health beliefs was positively and significantly associated with the elderly's adherence to falls PM (p 0.002, p <0.001, p <0.001, respectively). In multiple linear regression, dimensions 3, 4 and the total score of beliefs in health remained positively associated with the adherence of the elderly. In the pre and postintervention comparison, there was a statistically significant increase in the mean scores of the constructs perception of susceptibility, perception of severity, perception of benefits, perception of barriers and in the total score of beliefs (p <0.0001, 0.0019, <0.0001, <0.0001 and <0.0001, respectively). The indicators of total adherence increased (p <0.0001) and partial adherence decreased (p <0.0001) significantly. When adding the total adherence to the partial, there was a significant increase in the adherence of the elderly to the preventive measures for falls after the educational intervention (p <0.0001). Conclusion: there was a positive association between the adherence of the elderly to the PM of falls with beliefs in health and that educational intervention based on the HBM was able to improve these beliefs and their adherence. It is recommended that nurses consider the health beliefs of the elderly about falls and their prevention in health care for these people and that educational interventions based on this model are used. |

Objetivo: evaluar el efecto de una intervención educativa en la adherencia de los ancianos a las medidas preventivas contra caídas y creencias en salud. Materiales y Método: estudio cuasiexperimental, con diseño de series de tiempo, realizado en el Centro de Convivência para Seniores de Cuiabá, de junio a octubre de 2019. La muestra se determinó considerando error tipo I = 5%, error tipo II = 20%, proporción de adherencia a las caídas MP = 64,36%, diferencia mínima antes y después de la intervención = 4%, resultando una muestra mínima de 66 ancianos. Se incorporó un 30% a la muestra, prediciendo pérdidas, y terminó con 97 ancianos. Se incluyen: personas de 60 años y más que hayan realizado al menos una de las actividades físicas. Excluidos: personas que tuvieran dificultades cognitivas y mentales. 109 personas mayores fueron reclutadas (línea de base), de los cuales 68 completaron un protocolo de intervención. La intervención educativa se basó en el Modelo de Creencias en Salud (MCS) y se llevó a cabo en seis etapas (T0 a T5). Se aplicaron instrumentos validados para las evaluaciones pre y posintervención: características sociodemográficas y condiciones de salud, creencias de salud y adherencia a las caídas MP. Se utilizaron análisis de regresión lineal múltiple simple (no ajustada) y ajustada y pruebas de t pareada y de Wilcoxon para muestras pareadas. Resultados: la mayoría de los ancianos reclutados son mujeres (86,24%), viudas (37,61%), tenían de 1 a 4 años de estudio, edad media de 69,73 años, con problemas de salud (93,58%). %) y cayó un 46,79% en el último año. En regresión lineal simple, las dimensiones 3 (beneficios percibidos de la adhesión a las caídas de la MP), 4 (percepción de las barreras para la adhesión a las caídas de la MP) y la puntuación total de las creencias de salud se asoció positiva y significativamente con la adherencia de los ancianos a las caídas MP (p 0,002, p <0,001, p <0,001, respectivamente). En regresión lineal múltiple, las dimensiones 3, 4 y el puntaje total de creencias en salud permanecieron asociadas positivamente con la adherencia de los ancianos. En la comparación previa y posterior a la intervención, hubo un aumento estadísticamente significativo en las puntuaciones medias de los constructos percepción de susceptibilidad, percepción de severidad, percepción de beneficios, percepción de barreras y en la puntuación total de creencias (p <0,0001, 0,0019, <0, 0001, <0,0001 y <0,0001, respectivamente). Los indicadores de adherencia total aumentaron (p <0,0001) y la adherencia parcial disminuyó (p <0,0001) significativamente. Al sumar la adherencia total a la parcial, hubo un aumento significativo en la adherencia de los ancianos a las medidas preventivas de caídas tras la intervención educativa (p <0,0001). Conclusión: hubo asociación positiva entre la adherencia de los ancianos al MP de caídas con creencias de salud y que la intervención educativa basada en el MCS fue capaz de mejorar estas creencias y su adherencia. Se recomienda que los enfermeros consideren las creencias de salud de los ancianos sobre las caídas y su prevención en la atención de la salud de estas personas y que se utilicen intervenciones educativas basadas en este modelo.A adoção de medidas preventivas (MP) é a melhor forma de os idosos prevenirem quedas, contudo eles encontram dificuldades em adotar essas medidas no seu cotidiano. Objetivo: avaliar o efeito de uma intervenção educativa na adesão dos idosos às medidas preventivas de quedas e crenças em saúde. Materiais e Método: estudo quase experimental, com delineamento tempo-série, realizado no Centro de Convivência para Idosos de Cuiabá, de junho a outubro de 2019. A amostra foi determinada considerando erro tipo I = 5%, erro tipo II = 20%, proporção de adesão às MP de quedas = 64,36%, diferença mínima antes e pós-intervenção = 4%, resultando em amostra mínima de 66 idosos. Acresceram-se 30% à amostra, prevendo perdas, sendo finalizada com 97 idosos. Incluídos: pessoas com 60 anos e mais e que realizavam pelo menos uma das atividades físicas. Excluídos: pessoas que apresentavam dificuldade cognitiva e mental. Recrutaram-se 109 idosos (baseline), dos quais 68 finalizaram o protocolo de intervenção. A intervenção educativa foi embasada no Modelo de Crenças em Saúde (MCS) e realizada em seis tempos (T0 a T5). Instrumentos validados foram aplicados para avaliações pré e pós-intervenção: características sociodemográficas e condições de saúde, crenças em saúde e adesão às MP de quedas. Análises de regressão linear simples (não ajustada) e múltipla ajustada e os testes t pareado e Wilcoxon para amostras pareadas foram utilizados. Resultados: a maioria dos idosos recrutados é mulher (86,24%), viúva (37,61%), possuía de 1 a 4 anos de estudo, média de idade de 69,73 anos, com problemas de saúde (93,58%) e 46,79% caíram no último ano. Na regressão linear simples, as dimensões 3 (Percepção dos benefícios em aderir às MP de quedas), 4 (Percepção das barreiras para aderir às MP de quedas) e o escore total das crenças em saúde associaram-se positivamente e significativamente com a adesão dos idosos às MP de quedas (p 0,002, p < 0,001, p < 0,001, respectivamente). Na regressão linear múltipla, as dimensões 3, 4 e o escore total de crenças em saúde permaneceram associados positivamente com a adesão dos idosos. Na comparação pré e pós-intervenção, houve aumento estatisticamente significativo nos escores médios dos construtos percepção de suscetibilidade, percepção de severidade, percepção de benefícios, percepção de barreiras e no escore total de crenças (p <0,0001, 0,0019, <0,0001, <0,0001 e <0,0001, respectivamente). Os indicadores de adesão total aumentaram (p <0,0001) e de adesão parcial diminuíram (p <0,0001) significativamente. Ao somar a adesão total à parcial houve aumento significativo da adesão dos idosos às medidas preventivas de quedas pós-intervenção educativa (p <0,0001). Conclusão: verificou-se associação positiva da adesão dos idosos às MP de quedas com as crenças em saúde e que intervenção educativa embasada no MCS foi capaz de melhorar essas crenças e a adesão destes.
