Modulação autonômica cardíaca na recuperação pós-teste máximo de esforço, nível de atividade física e aptidão aeróbia em jovens adultos obesos

Resumo: | Excess body mass (BM) has been associated with the disorder in autonomic modulation, reflecting greater exposure to cardiovascular risks. The aim of this study was to analyze heart rate (HR) and cardiac autonomic modulation after maximal exercise test, physical activity level and aerobic fitness of young adults with different body mass indexes. Sixty (22.17 ± 2.89 years) untrained men were evaluated, being 24 eutrophics (E: 22.30 ± 1.77 kg ∙ m2), 15 overweight (S: 27.02 ± 1.41 kg / m²) and 21 obese (O: 34.03 ± 3.38 kg / m2) submitted to resting evaluation (anthropometric, hemodynamic variables, autonomic cardiac modulation), maximum progressive test (MPT) on cycloergometer (oxygen consumption peak [VO2peak]) And recovery (HR and RMSSD and SDNN of HR variability [HRV]). One-Way ANOVA was used for comparison between Tukey's post hoc groups for parametric data and the Kruskal-Wallis test, with Dunn's post hoc for non-parametric data. To compare the HR values and the HRV indices in the recovery between groups and between moments, mixed ANOVA with post hoc of Bonferroni was used. Also, Pearson's linear correlation was used for parametric and Spearman's Rank's data for non-parametric data. The level of physical activity (IPAQ) did not differ between groups (E: 1546 ± 1703; S: 1599 ± 1482; O: 2252 ± 3786 MET/min/wk). The Group O presented higher values of diastolic blood pressure (DBP) and mean arterial pressure (MAP) (75.82 ± 8.04 and 90.36 ± 9.71 mmHg) compared to the E groups (70.44 ± 6.47 And 84.84 ± 7.09 mmHg) and S (72.42 ± 6.26 and 86.69 ± 4.91 mmHg). The HR and values of the HRV at rest indexes were not different between the groups. The VO2peak was lower in the O group (28.94 ± 4.01 ml / kg / min) compared to the others (E: 37.25 ± 4.83, S: 36.97 ± 4.89 ml / kg / min). The anthropometric variables (body mass, body mass indice, neck circumference, waist circumference, waist hip ratio) were associated with SBP, DBP and MAP (r: 0.25-0.50) and inversely to VO2max (r: -0.45 a - 0.68). There were no differences in the absolute values, in the percentages of HR reduction and in the HRV indexes in the 10' post-PMT recovery between the groups. There was no vagal resumption after 600" postPMT. However, sympathetic withdrawal was observed after five minutes of active recovery, in groups E and O with oscillations in group S. The level of physical activity (r: -0.25 to -0.38) and DBP (r: 0, 25 to 0.39) were associated with HR at various moments of recovery, as well as HRV (RMSSD and SDNN) indices at recovery. Fcrep was correlated with all recovery moments (r: 0.30 to 0.49). From the results, it can be concluded that young obese adults in the present study already presented altered DBP and MAP values, as well as lower aerobic fitness in relation to eutrophic ones; however, this was not enough to cause impairment to cardiac autonomic modulation at rest and recovery of these subjects. |

Este estudo teve por objetivo analisar a frequência cardíaca (FC) e a modulação autonômica cardíaca pós-teste máximo de esforço, o nível de atividade física e a aptidão aeróbia de adultos jovens com diferentes índices de massa corporal (IMC). Foram avaliados 60 homens (22,17 ± 2,89 anos) não treinados, sendo 24 eutróficos (E: 22,30 ± 1,77 kg∙m2 ), 15 sobrepesos (S: 27,02 ± 1,41 kg/m2 ) e 21 obesos (O: 34,03 ± 3,38 kg/m2 ) submetidos a avaliações em repouso (variáveis antropométricas, hemodinâmicas, modulação autonômica cardíaca), teste progressivo máximo (TPM) em cicloergômetro (consumo máximo de oxigênio [VO2pico]) e recuperação (FC e índices RMSSD e SDNN da variabilidade da FC [VFC]). Utilizou-se Anova One-Way para comparação entre grupos com post hoc de Tukey para dados paramétricos e o teste de KruskalWallis, com post hoc de Dunn’s para dados não paramétricos. Para comparação dos valores da FC e dos índices da VFC na recuperação entre grupos e entre momentos foi utilizada ANOVA mista com post hoc de Bonferroni. Ainda, foi utilizada correlação linear de Pearson para dados paramétricos e de Spearman’s Rank’s para dados não paramétricos. O nível de atividade física (IPAQ) não diferiu entre os grupos (E: 1546 ± 1703; S: 1599 ± 1482; O: 2252 ± 3786 MET/min/sem). O grupo O apresentou maiores valores de pressão arterial diastólica (PAD) e pressão arterial média (PAM) (75,82 ± 8,04 e 90,36 ± 9,71 mmHg) comparado aos grupos E (70,44 ± 6,47 e 84,84 ± 7,09 mmHg) e S (72,42 ± 6,26 e 86,69 ± 4,91 mmHg). A FC e os valores dos índices de VFC em repouso não foram diferentes entre os grupos. O VO2pico foi menor no grupo O (28,94 ± 4,01 ml/kg/min) comparado aos demais (E: 37,25 ± 4,83; S: 36,97 ± 4,89 ml/kg/min). As variáveis antropométricas (MC, IMC, circunferência do pescoço, da cintura, do abdômen e a relação cintura/quadril) se associaram à PAS, à PAD e PAM (r: 0,25 a 0,50) e inversamente ao VO2pico (r: -0,45 a -0,68). Não houve diferenças nos valores absolutos, nos percentuais de redução da FC e nos índices de VFC nos 10’ de recuperação pós TPM entre os grupos. Não houve retomada vagal após 600” pós-TPM. Contudo, foi verificada retirada simpática após os cinco min da recuperação ativa, nos grupos E e O com oscilações no grupo S. O nível de atividade física (r: -0,25 a -0,38) e a PAD (r: 0,25 a 0,39) associaram-se a FC em diversos momentos da recuperação, bem como, com os índices da VFC (RMSSD e SDNN) na recuperação. A FCrep foi correlacionada com todos os momentos da recuperação (r: 0,30 a 0,49).
