Resumo: Over the last two decades, fostering and incentive to breastfeeding have been objects of study and widely publicized by the national and international scientific community. Skin-to-skin contact after childbirth and encouragement to breastfeeding in the labor environment have demonstrated to be favorable practices for keeping breastfeeding up to the age six months, in addition to offer essential nutritional elements to the child health. Therefore, breastfeeding is considered to be a political, economic and social practice. General Objective: Creating a protocol for breastfeeding during the first hour of life for a low-risk pregnancy. Specific objectives: Knowing scientific evidence on exclusive breastfeeding during the first hour of life; Knowing the social-demographic of professionals that assist a low-risk pregnancy, investigating current breastfeeding practices in the first hour of life for a low-risk pregnancy. Method: In order to achieve the first objective, an integrative review of the literature was carried out by searching national and international databases, which included fourteen articles overall. In order to achieve the second and the third objectives, a descriptive quantitative research was performed. A multi-professional team working on a low-risk pregnancy answered a questionnaire, which allowed us to identify a demographic profile that included occupation, years of career, positions occupied in the career and at institutions and, moreover, the following three dimensions: social-demographic profile, maternal dimension and professional dimension. Those dimensions covered the knowledge of the employees on the topic and their professional practices. Results: From the results of the literature integrative review, the topics that appeared on the articles studied focused on the incentive to skin-to-skin contact in the nursery room; breastfeeding as a benefit to the bond between mother and baby; consideration of opinions from puerperal women regarding breastfeeding and premature skin-to-skin contact. The results highlight the necessity of training of teams that work at the obstetric area towards promoting breastfeeding during the first hour of life, as well as the importance of public authorities support on the theme. Regarding the questionnaire application, it was divided into three dimensions and Dimension I referred to the social-demographic profile. Thirty-five professionals that work at the obstetric service have been interviewed. As Dimensions I and II were analyzed to achieve the third objective, it has been noticed that professional guidance regarding skin-to-skin contact and breastfeeding right after labor are 97% positive. Conclusion: In order to carry out this research, scientific evidence on the topic have been collected, as a questionnaire was applied to professionals working in low-risk pregnancies. Their answers were studied and then used as the base to create a protocol. Data analysis provided by the questionnaire allowed us to identify the social-demographic profile of professionals and their practices towards breastfeeding during the first hour of life. Thus, the protocol was based on their profile and practices developed in such context and supported by solid scientific evidence. Product and applicability Through this study, a Breastfeeding During The First Hour Protocol has been produced, which is a health care technology that enhances mother and baby quality assistance and can be applied in every health care setting. We reinforce that its applicability will be extremely important in low-risk pregnancy context, considering how necessary in the obstetric area it is.

O contato pele a pele logo após o nascimento, e o estímulo da amamentação ao RN em ambiente de parto, demonstra práticas favoráveis à manutenção do aleitamento materno até os seis meses de idade, além de oferecer elementos nutricionais essenciais à saúde da criança. Assim, o aleitamento materno é considerado uma prática política, econômica e social. Objetivo Geral: Criar protocolo de amamentação na primeira hora de vida para uma maternidade de risco habitual. Objetivos específicos: Conhecer as evidências científicas acerca do aleitamento materno na primeira hora de vida; Conhecer o perfil sócio demográfico dos profissionais que atuam em uma maternidade de risco habitual; Investigar as práticas atuais de amamentação na primeira hora de vida em uma maternidade de risco habitual. Método: Para atender o primeiro objetivo foi realizado uma revisão integrativa da literatura com busca em bases de dados nacional e internacional a fim de realizar uma revisão integrativa que contou com 14 artigos finais. Para atender o segundo e terceiro objetivo foi realizada uma pesquisa quantitativa exploratória e descritiva. Aplicou-se um questionário à equipe multiprofissional de uma maternidade de risco habitual, onde foi possível identificar o perfil demográfico contemplando a profissão, tempo de formação, atuação na profissão e na instituição e, ainda, três dimensões: perfil sócio demográfico; dimensão materna e dimensão profissional. Estas dimensões contemplaram os conhecimentos dos profissionais acerca do tema e as práticas por eles realizadas. Resultado: Dos dados resultantes da revisão integrativa da literatura, os temas que emergiram dos artigos analisados apresentam foco no estímulo ao contato pele a pele no alojamento conjunto; o aleitamento materno como benéfico para o vínculo entre o binômio; a consideração da opinião das puérperas em relação a amamentação e contato pele a pele oportuno. Destaca-se a necessidade de capacitações às equipes atuantes no cenário obstétrico para a promoção do aleitamento materno na primeira hora de vida, bem como a importância do apoio do poder público acerca do tema. Referente à aplicação do questionário, foram entrevistados 35 profissionais que atuam no serviço obstétrico, por meio de um questionário dividido em três dimensões sendo que a Dimensão I é referente ao perfil sócio demográfico. Ao se analisar a Dimensão II e III, para atender o terceiro objetivo, nota-se que quanto a atuação profissional frente às orientações de contato pele a pele e amamentação após o parto, em 97% são positivas. Considerações: Para criar o produto desta pesquisa foi realizado um estudo para conhecer as evidências científicas acerca do tema, com a possibilidade de construção do questionário a ser aplicado aos profissionais inseridos no atendimento obstétrico de risco habitual, onde a análise das respostas foram utilizadas como disparadores para a construção do protocolo. A partir da análise do questionário aplicado, foi possível identificar o perfil sócio- demográfico dos profissionais, e as suas práticas em relação a amamentação na primeira hora de vida. Desta forma, a elaboração do protocolo foi baseado no perfil dos profissionais, nas práticas que desenvolvem neste contexto e respaldadas na melhores evidências científicas. Produto e aplicabilidade: O produto desse estudo foi um protocolo de Amamentação na Primeira Hora de Vida o qual se configura como uma tecnologia de cuidado e corrobora com a qualificação do atendimento materno infantil e pode ser aplicado em âmbito hospitalar.
