Resumo: Introduction: The World Health Organization recommends exclusive breastfeeding during the first six months of life and its maintenance as a nutritional complement for up to two or more years so that children can receive all the benefits of breast milk for healthy growth and development. Therefore, studies on factors associated with weaning, as well as its consequences, may reveal important data for maintaining this important practice for the mother-baby binomial. Objectives: In this context, this dissertation contemplates two articles whose objectives were: (1) Identify the scientific productions published in the literature on the association between digital sucking and early weaning in children; (2) Verify the association between the type of breastfeeding and the development of the respiratory pattern in children. Article 1: In order to answer the first objective, an integrative review was performed which included searching the MEDLINE / PubMed, Scielo and Scopus databases for the combination of descriptors and free terms of the Mesh database "child OR children", "fingersucking OR thumbsucking" and "weaning ", Associated with each other by the Boolean operator AND. The search period comprised the months of August and September of 2018. After the removal of the duplicate articles, 106 articles were obtained whose abstracts and titles were read by two independent researchers. After the application of the eligibility criteria, 25 jobs remained and five others were found in their references. After reading the 30 articles in full, five were selected. Of these, only one found a statistically significant association between the achievement of the digital sucking habit with early weaning (p = 0.0000), while the others did not prove this association. Thus, to date there are few studies on the subject and there seems to be no clear association between digital sucking and early weaning. It is suggested to carry out more research with this theme, especially those of longitudinal design, which will better establish the cause-effect relationship between the study variables. Article 2: In order to answer the objective of the second article of this dissertation, a systematic review with meta-analysis, elaborated from searches in the electronic databases Cochrane Library, EMBASE, LILACS, MEDLINE / PubMed, Science Direct, Scopus and Web of Science, as well as like Google Scholar and Opengrey, are considered gray literature. For each one the search strategy used was adapted. Also, the references of the selected articles were considered. Observational studies evaluating the association between the type of breastfeeding and the development of the respiratory pattern in children were included. The selection of articles was carried out by two independent researchers, both in the first phase (reading titles and abstracts), and in the second (reading the articles selected in the first phase, in its entirety). To evaluate the quality of included studies, the MAStARI (Meta Analysis of Statistics and Review Instrument) tool was used, and a meta-analysis of the results was performed. After the duplicates were removed, the search resulted in 840 articles, of which seven were included. The overall prevalence of mouth breathers was 44% (95% CI, 38-49) (total sample = 912). Breast-fed infants presented a protective factor for the development of mouth breathing (OR = 0.62, 95% CI, 0.41-0.93) compared to non-breastfed infants, and those breastfed for more than 12 months and more that 24 months had respectively 41% and 11 34% less chance of developing it. There was no association between exclusive breastfeeding for less than or up to six months and the occurrence of mouth breathing (OR = 0.60; 95% CI, 0.31-1.18). Based on the limited evidence found, it was concluded that breastfeeding was considered a protective factor for the occurrence of mouth breathing. This, in turn, presented reductions with the increase in the time of breastfeeding, especially until the 2 years of breastfeeding. Assim, estudos sobre os fatores associados ao desmame, bem como suas consequências, podem revelar dados importantes para a manutenção desta prática tão importante para o binômio mãe-bebê. Objetivos: Neste contexto, esta dissertação contempla dois artigos cujos objetivos foram: (1)Identificar as produções científicas publicadas na literatura sobre a associação entre a sucção digital com o desmame precoce em crianças; (2)Verificar a associação entre o tipo de aleitamento e o desenvolvimento do padrão respiratório em crianças. Artigo 1: Para responder ao primeiro objetivo foi realizada uma revisão integrativa, que contemplou buscas nas bases de dados MEDLINE/PubMed, Scielo e Scopus, pela combinação de descritores e termos livres do Mesh Database ―child OR children‖, ―fingersucking OR thumbsucking‖ e ―weaning‖, associados entre si pelo operador booleano AND. O período de buscas compreendeu os meses de agosto e setembro de 2018. Após a remoção dos artigos duplicados, obtiveram-se 106 artigos cujos resumos e títulos foram lidos por duas pesquisadoras independentes. Após a aplicação dos critérios de elegibilidade, restaram 25 trabalhos e foram encontrados outros cinco em suas referências. Após a leitura dos 30 artigos na íntegra, cinco foram selecionados. Desses, apenas um encontrou associação estatisticamente significativa entre a realização do hábito de sucção digital com o desmame precoce (p=0,0000), enquanto os demais não comprovaram essa associação. Assim, até o momento existem poucos estudos sobre o tema e parece não haver associação clara entre a sucção digital e o desmame precoce. Sugere-se a realização de mais pesquisas com essa temática, especialmente os de delineamento longitudinal, que estabelecerão melhor a relação de causa-efeito entre as variáveis de estudo. Artigo 2: Para dar resposta ao objetivo do segundo artigo desta dissertação, foi realizada uma revisão sistemática com metanálise, elaborada a partir de buscas nas basesde dados eletrônicas Cochrane Library, EMBASE, LILACS, MEDLINE/PubMed, Science Direct, Scopus e Web of Science, bem como Google Scholar e Opengrey, estas consideradas literatura cinzenta. Para cada uma foi adaptada a estratégia de busca utilizada. Também, foram consideradas as referências dos artigos selecionados. Foram incluídos estudos observacionais avaliando a associação entre o tipo de aleitamento e o desenvolvimento do padrão respiratório em crianças. A seleção dos artigos foi realizada por dois pesquisadores independentes, tanto na primeira fase (leitura de títulos e abstracts), quanto na segunda (leitura dos artigos selecionados na primeira fase, na íntegra). Para avaliar a qualidade dos estudos incluídos foi utilizada a ferramenta MAStARI (Meta Analysis of Statistics Assessment and Review Instrument), e realizada metanálise dos resultados. Após a remoção dos duplicados, a busca resultou em 840 artigos, sendo que sete foram incluídos. A prevalência geral de respiradores bucais foi de 44% (IC95%, 38-49) (amostra total = 912). Crianças amamentadas no peito apresentaram um fator de proteção para o desenvolvimento da respiração bucal (OR=0,62; IC95%, 0,41-0,93) em comparação às crianças não amamentadas, e as amamentadas por mais 9 que 12 meses e mais que 24 meses tiveram respectivamente 41% e 34% menos chance de desenvolvê-la. Não houve associação entre o aleitamento materno exclusivo por menos ou até seis meses e a ocorrência da respiração bucal (OR=0,60; IC95%, 0,31-1,18). Baseado nas evidências limitadas encontradas, conclui-se que o aleitamento materno foi considerado um fator de proteção para a ocorrência da respiração bucal. Esta, por sua vez, apresentou reduções com o aumento no tempo de amamentação, especialmente até os 2 anos de aleitamento materno. Produto: O produto resultante desta dissertação foi a metanálise intitulada: ―Crianças amamentadas no peito têm menos chance de desenvolver respiração bucal? Revisão sistemática e metanálise‖. Breast-fed infants presented a protective factor for the development of mouth breathing (OR = 0.62, 95% CI, 0.41-0.93) compared to non-breastfed infants, and those breastfed for more than 12 months and more that 24 months had respectively 41% and 11 34% less chance of developing it. There was no association between exclusive breastfeeding for less than or up to six months and the occurrence of mouth breathing (OR = 0.60; 95% CI, 0.31-1.18). Based on the limited evidence found, it was concluded that breastfeeding was considered a protective factor for the occurrence of mouth breathing. This, in turn, presented reductions with the increase in the time of breastfeeding, especially until the 2 years of breastfeeding.
