Como patologias crônicas de grande prevalência, a hipertensão arterial e a diabetes mellitus requerem uma atenção maior por parte dos profissionais envolvidos. A necessidade do acompanhamento destes pacientes sugere a criação de mecanismos que facilitem a terapêutica. A Atenção Farmacêutica, há tempos, contribui para o controle dessas morbidades, utilizando formulários específicos, em cada consulta com o paciente O tradicional preenchimento manual destes formulários dificulta sua organização e torna o processo de recuperação e análise dos dados árduo. Visando diminuir estes problemas, neste trabalho, propõe-se o desenvolvimento de um software de acompanhamento para Atenção Farmacêutica, baseado no método Minesota Pharmaceutical Care. Este sistema, denominado SANUS, tem como objetivo a informatizar o preenchimento, armazenamento e consulta dos formulários, facilitando o acompanhamento farmacoterapêutico, otimizando o tempo de visita e melhorando a recuperação de informação e a comunicação entre os profissionais de saúde. As especificações e funcionalidades do software foram levantadas em conjunto com professores da área de Farmácia e Ciência da Computação. O modelo de dados (Esquema Relacional Estendido) foi concebido a partir dos requisitos levantados e vem sendo sucessivamente refinado. Também foram especificados os módulos do produto e construção de artefatos do sistema utilizando o banco de dados MySQL. Durante o processo de desenvolvimento são feitas reuniões quinzenais entre a equipe para revisão das funcionalidades e acompanhamento do projeto, e caso uma nova funcionalidade seja levantada, sua inclusão no escopo do projeto é avaliada, seguindo o modelo de desenvolvimento ágil SCRUM. O sistema foi testado por discentes que já realizavam Atenção Farmacêutica. O resultado foi bastante satisfatório principalmente quanto à facilidade de utilização, ao tempo para aprender a usar o sistema e à sequência estruturada das telas. O SANUS é um avanço importante na prática de Atenção Farmacêutica, na medida em que propõe inovação tecnológica para a documentação dos registros de acompanhamento de pacientes pelo farmacêutico.The growth of the access of the population to medication is one of the major challenges faced by the Brazilian Government. As widely spread chronic pathologies, hypertension and diabetes mellitus require more attention by the professionals involved. The need for monitoring the disease group demands the creation of mechanisms to make therapeutics easier. Pharmaceutical care, for a long time, have been contributing to the control of these diseases, using specific forms in every consultation with the patient. The traditional and manual completion of these forms makes your organization hard and the recovery process and data analysis becomes more difficult. In order to reduce these problems, in this work, it is proposed the development of a software that monitor the pharmaceutical care, based on Minnesota Pharmaceutical Care method. This system, called SANUS, aims to computerize the filling, the storage and the consultation of the forms, facilitating pharmacotherapy supervision, optimizing time and improving information retrieval and communication between health care professionals. The specifications and features of the software were raised in conjunction with teachers in the area of Pharmacy and computer science. The data model (relational schema extended) was designed according to the collected requirements and has been successively improved. The product modules and construction of system artifacts were also specified using the MySQL database. Meetings among the team are held fortnightly during the development process to review the features and the development of the project, and, if a new feature is considered, its inclusion in the scope of the project is evaluated by following the Agile SCRUM development model. The system was tested by students that are used to utilize pharmaceutical care. The result was quite satisfying, because it’s easy to use it, it’s easy to learn it and its screens have a structured sequence.
